Many routine and necessary health services were<\/span>\u00a0<a href=\"https:\/\/www.gov.scot\/news\/nhs-scotland-placed-on-emergency-footing\/\"><span style=\"font-weight: 400\">put<\/span><\/a> <a href=\"https:\/\/www.bbc.co.uk\/news\/uk-wales-51877667\"><span style=\"font-weight: 400\">on<\/span><\/a> <a href=\"https:\/\/www.health-ni.gov.uk\/news\/services-will-be-significantly-curtailed-due-covid-19-minister\"><span style=\"font-weight: 400\">hold<\/span><\/a><span style=\"font-weight: 400\">. Retired nurses and doctors <\/span><a href=\"https:\/\/www.england.nhs.uk\/2020\/04\/thousands-of-former-nhs-staff-are-back-on-the-front-line-in-the-nhs-fight-against-coronavirus\/\"><span style=\"font-weight: 400\">returned to the frontline<\/span><\/a><span style=\"font-weight: 400\">, clinical staff were redeployed to unfamiliar departments, and <\/span><a href=\"https:\/\/www.hsj.co.uk\/policy-and-regulation\/nhs-block-books-almost-all-private-hospital-sector-capacity-to-fight-covid-19\/7027196.article\"><span style=\"font-weight: 400\">independent hospitals<\/span><\/a><span style=\"font-weight: 400\"> were tapped to supplement care, including urgent surgery and cancer treatment. In England, <\/span><a href=\"https:\/\/www.england.nhs.uk\/2020\/03\/new-nhs-nightingale-hospital-to-fight-coronavirus\/\"><span style=\"font-weight: 400\">entire hospitals<\/span><\/a><span style=\"font-weight: 400\">,<\/span><span style=\"font-weight: 400\"> known as Nightingales, were built and staffed in a matter of weeks, in anticipation of a wave of cases and in an attempt to shield the NHS from being overwhelmed. This rapid ingenuity paid off: the <\/span><a href=\"https:\/\/www.england.nhs.uk\/2020\/03\/new-nhs-nightingale-hospital-to-fight-coronavirus\/\"><span style=\"font-weight: 400\">Nightingales<\/span><\/a><span style=\"font-weight: 400\"> never reached capacity and the health service crisis was, in many ways, averted.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Now that we have passed the peak, what is our plan for stepdown? How do we &#8220;extubate&#8221; our health service and reanimate our daily work?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">To safely restart routine services, it is critical that patient needs and perspectives are given paramount consideration.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\"><strong>Patient-centred care is needed now more than ever<\/strong><\/p>\n<p><\/span><span style=\"font-weight: 400\">Covid-19 has underscored the need for <\/span><a href=\"https:\/\/www.health.org.uk\/sites\/default\/files\/PersonCentredCareMadeSimple.pdf\"><span style=\"font-weight: 400\">patient centricity<\/span><\/a><span style=\"font-weight: 400\">. The diversity of covid disease progressions is such that <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1684118220300943\"><span style=\"font-weight: 400\">treatments<\/span><\/a><span style=\"font-weight: 400\"> that work for one patient cannot be relied upon to do the same for others. The term &#8220;respiratory virus&#8221; fails to capture the <\/span><a href=\"https:\/\/www.who.int\/health-topics\/coronavirus#tab=tab_3\"><span style=\"font-weight: 400\">ever-evolving range<\/span><\/a><span style=\"font-weight: 400\"> of symptoms reported. The novelty of this disease has reminded us that the patient perspective is essential; we still know very little about this virus, but patients know what is and isn\u2019t normal for them.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Recovering covid patients from all backgrounds are discovering that their health challenges may not be over. Long-term damage to the kidneys, lungs, and heart seem to be an <\/span><a href=\"https:\/\/www.vox.com\/2020\/5\/8\/21251899\/coronavirus-long-term-effects-symptoms\"><span style=\"font-weight: 400\">unfortunate potential legacy<\/span><\/a><span style=\"font-weight: 400\">. And whilst we have high hopes for a <\/span><a href=\"https:\/\/www.healthline.com\/health-news\/heres-exactly-where-were-at-with-vaccines-and-treatments-for-covid-19\"><span style=\"font-weight: 400\">vaccine<\/span><\/a><span style=\"font-weight: 400\"> and antiviral treatment, it is clear that we will be living with the Sars-CoV-2 virus for some time. In this next phase, we must contend with meeting the needs of the new covid patients who will continue to arrive, along with the traditional range of acutely and chronically ill patients.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">As ever, there is no such thing as an &#8220;average patient&#8221;, and we design services for a prototype at our peril. Even in the covid context, the key to patient safety is patient centricity: the extent to which the health service is responsive is to diverse patient needs, how it enables patients to voice their concerns, and how it tailors care to individual clinical needs.\u00a0<\/span><\/p>\n<p><strong>Covid-19 is a health issue, but also a wider social issue<\/strong><\/p>\n<p><span style=\"font-weight: 400\">NHS business as usual halted during the crisis, but life as usual did not: people still had babies, heart attacks, accidents, and <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/bulletins\/coronavirusandshieldingofclinicallyextremelyvulnerablepeopleinengland\/28mayto3june2020\"><span style=\"font-weight: 400\">3 percent<\/span><\/a><span style=\"font-weight: 400\"> of our population still had severe long-term conditions, which during covid required them to &#8220;<\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/guidance-on-shielding-and-protecting-extremely-vulnerable-persons-from-covid-19\/guidance-on-shielding-and-protecting-extremely-vulnerable-persons-from-covid-19\"><span style=\"font-weight: 400\">shield&#8221;<\/span><\/a><span style=\"font-weight: 400\"> at home without regular in-person care. Furthermore, the <\/span><a href=\"https:\/\/www.centreformentalhealth.org.uk\/coronavirus-update\"><span style=\"font-weight: 400\">mental health consequences<\/span><\/a><span style=\"font-weight: 400\"> associated with isolation, illness, and challenging working environments compound the burden of disease and pressure on the health service.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Covid-19 is proving to be society\u2019s very own long-term condition, wreaking havoc on its vital organ systems\u2014education, housing, industry\u2014and exacerbating existing inequalities. The <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2766098\"><span style=\"font-weight: 400\">disproportionate<\/span><\/a> <a href=\"https:\/\/www.cebm.net\/covid-19\/bame-covid-19-deaths-what-do-we-know-rapid-data-evidence-review\/\"><span style=\"font-weight: 400\">impact<\/span><\/a><span style=\"font-weight: 400\"> of covid-19 on marginalised communities intersects with other inequities, such that certain communities find themselves facing multiple public health crises simultaneously. Recognition of these layered inequalities was evidenced by the <\/span><a href=\"https:\/\/www.economist.com\/international\/2020\/06\/08\/how-george-floyds-death-reverberates-around-the-world\"><span style=\"font-weight: 400\">global outpouring of support<\/span><\/a><span style=\"font-weight: 400\"> for the Black Lives Matter protests sparked by the death of George Floyd, and the structural racism that continues to place people from ethnic minorities at higher risk of poor health and adverse outcomes.\u00a0<\/span><\/p>\n<p><strong>The pandemic is an opportunity to innovate to for safer care<\/strong><\/p>\n<p><span style=\"font-weight: 400\">Whilst some clinical resources must be ringfenced for ongoing covid-19 endeavours, this is an opportunity to innovate approaches for continuing quality improvement. Instead of suspending projects, now is the time to demonstrate <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp2007781\"><span style=\"font-weight: 400\">adaptability<\/span><\/a><span style=\"font-weight: 400\">, resilience, teamwork, and <\/span><a href=\"https:\/\/bjgplife.com\/2020\/04\/15\/the-technology-legacy-of-covid-19-in-primary-care\/\"><span style=\"font-weight: 400\">technological innovation<\/span><\/a><span style=\"font-weight: 400\">. By using apps to <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41591-020-0916-2\"><span style=\"font-weight: 400\">crowdsource<\/span><\/a><span style=\"font-weight: 400\"> covid-19 symptoms, <\/span><a href=\"https:\/\/participate.mqmentalhealth.org\/studies\/ccopey-young-peoples-mental-health-and-coping-styles-during-and-after-covid-19-lockdown\/\"><span style=\"font-weight: 400\">partnering with young people<\/span><\/a><span style=\"font-weight: 400\"> to support the mental health of their peers, and monitoring patients <\/span><a href=\"https:\/\/www.imperial.ac.uk\/news\/196973\/wearable-sensor-trialled-remote-covid-19-monitoring\/\"><span style=\"font-weight: 400\">remotely<\/span><\/a><span style=\"font-weight: 400\">, we can find new ways to connect with patients at the same time that we curtail the spread of the disease.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Despite the unprecedented challenges faced by the NHS in recent months, the pandemic has also accelerated innovation in fields such as technology, policy, and healthcare service design. Efforts to expedite covid-19 related research\u2014from <\/span><a href=\"https:\/\/www.hra.nhs.uk\/covid-19-research\/fast-track-review-guidance-covid-19-studies\/\"><span style=\"font-weight: 400\">fast-tracking study approval<\/span><\/a><span style=\"font-weight: 400\"> processes to <\/span><a href=\"https:\/\/www.nihr.ac.uk\/news\/141-million-funding-announced-for-new-covid-19-research-on-treatments-vaccines-and-epidemiology\/24628\"><span style=\"font-weight: 400\">massive funding<\/span><\/a><span style=\"font-weight: 400\"> boosts, to the popularisation of <\/span><a href=\"https:\/\/www.natureindex.com\/news-blog\/how-coronavirus-is-changing-research-practices-and-publishing\"><span style=\"font-weight: 400\">preprints\u2014<\/span><\/a><span style=\"font-weight: 400\">have been unprecedented. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK379320\/\"><span style=\"font-weight: 400\">Telehealth<\/span><\/a><span style=\"font-weight: 400\">, a technology that has long been waiting in the wings, has been widely adopted virtually overnight. The pandemic has seen the <\/span><a href=\"https:\/\/www.hsj.co.uk\/policy-and-regulation\/nhs-block-books-almost-all-private-hospital-sector-capacity-to-fight-covid-19\/7027196.article\"><span style=\"font-weight: 400\">public and private sectors<\/span><\/a><span style=\"font-weight: 400\"> work together to tackle demands for patient beds and clinical equipment, and new hospitals were built in conference centres in a matter of weeks.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The pandemic has reinvigorated a culture of innovation and ingenuity within the NHS. This willingness to adapt quickly, assimilate new teams, share information, and experiment with novel interventions should be maintained. Past the peak of the acute emergency, it is time to define our new normal.
