{"id":48116,"date":"2020-07-22T17:15:33","date_gmt":"2020-07-22T16:15:33","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=48116"},"modified":"2020-07-28T13:31:34","modified_gmt":"2020-07-28T12:31:34","slug":"jenny-nguyen-and-ina-ko-teach-me-about-death","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/07\/22\/jenny-nguyen-and-ina-ko-teach-me-about-death\/","title":{"rendered":"Jenny Nguyen and Ina Ko: Teach me about death"},"content":{"rendered":"<p class=\"standfirst\">Medical schools aren\u2019t adequately preparing future doctors for the reality of having conversations about death, say Jenny Nguyen and Ina Ko<\/p>\n<p><!--more--><span style=\"font-weight: 400\">Medicine has come on leaps and bounds and, as a result, in the UK we have become what has been called a <\/span><span style=\"font-weight: 400\">&#8220;<a href=\"https:\/\/books.google.co.uk\/books?id=3be2764T3-4C&amp;printsec=frontcover&amp;source=gbs_ge_summary_r&amp;cad=0#v=onepage&amp;q=%E2%80%98death-free%20generation%E2%80%99&amp;f=false\">death free generation<\/a>.&#8221; Many people are largely unaffected by deaths from war, childbirth, or infection and can live for decades without experiencing the death of a loved one<\/span><span style=\"font-weight: 400\">. It has almost become an expectation that we will all live well into old age. However, death is inevitable and plays some part in every clinical specialty.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Public health figures indicate that <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/end-of-life-care-profiles-february-2018-update\/statistical-commentary-end-of-life-care-profiles-february-2018-update#:~:targetText=In%202016%2046.9%25%20of%20people,46.9%25%20(Figure%202).\"><span style=\"font-weight: 400\">46.9% of people in England will die in hospital<\/span><\/a><span style=\"font-weight: 400\">, making death an important part of hospital life that both medical students and doctors will be exposed to and required to address. Junior doctors, who commonly staff inpatient wards, are likely to be involved in prognosis counselling and end of life conversations with patients, carers, and loved ones from the very beginning of their careers. However, medical schools aren\u2019t preparing us for the reality of having these conversations.<\/span><\/p>\n<p><span style=\"font-weight: 400\">A UK survey of medical schools found that on average, <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/pdf\/10.1046\/j.1365-2923.2002.01232.x\">only 20 hours of the curriculum were dedicated to palliative care<\/a>, with a large proportion of these occurring in the later years of training<\/span><span style=\"font-weight: 400\">.<\/span><span style=\"font-weight: 400\"> A study by the Royal College of Physicians of Edinburgh (RCPE) <a href=\"https:\/\/www.rcpe.ac.uk\/sites\/default\/files\/bowden.pdf\">admitted that it was &#8220;unrealistic&#8221; to expect<\/a> any graduating medical student to be fully prepared for palliative medicine. Yet this failure to equip junior doctors with adequate knowledge of end of life care risks patients and their loved ones being left without the support they require.<\/span><\/p>\n<p><span style=\"font-weight: 400\">These topics should be addressed early in medical education. Pre-clinical teaching may include formal teaching on the process of death, such as common signs of a patient approaching death. During the clinical years, students should have protected time specifically with the palliative care team, as it can be uncomfortable and challenging for medical students to separate themselves from participating in the medical team\u2019s active treatment of other patients to observe the palliative care team. An annual week&#8217;s rotation in palliative care would further students\u2019 understanding of the services available and how best to support patients with varying needs.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Palliative care teams involve multidisciplinary staff that students may not encounter elsewhere, such as bereavement nurses. With greater diversity of experience in palliative care, students may be able to better refer to services available and explain what will happen as treatment and life comes to an end.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">A major challenge to education about death is its inherent emotionality. It is essential to remove the stigma and taboo around death. Medical schools do not always provide space for discussions about the sense of failure, guilt, or sadness doctors feel when nothing else can be done. This leaves future doctors vulnerable to significant emotional stress. The RCPE found that <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/www.rcpe.ac.uk\/sites\/default\/files\/bowden.pdf\">79% of FY1 doctors felt out of their depth<\/a> when treating patients who were dying\u2014particularly where it concerns their psychological distress, spiritual distress, and social issues<\/span><span style=\"font-weight: 400\">. An inability to address these issues led to <\/span><a href=\"https:\/\/www.rcpe.ac.uk\/sites\/default\/files\/bowden.pdf\"><span style=\"font-weight: 400\">two thirds of FY1 doctors feeling distressed themselves<\/span><\/a><span style=\"font-weight: 400\"> when caring for people who are dying.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.thelancet.com\/pdfs\/journals\/laneur\/PIIS1474-4422(17)30203-X.pdf\"><span style=\"font-weight: 400\">In 2004, the first Death Caf\u00e9 was organised in Switzerland<\/span><\/a><span style=\"font-weight: 400\"> in order to encourage open conversations about people\u2019s opinions and perceptions of death. <\/span><a href=\"https:\/\/www.thelancet.com\/pdfs\/journals\/laneur\/PIIS1474-4422(17)30203-X.pdf\"><span style=\"font-weight: 400\">The groups have now been set up in 48 countries with more than 4300 members globally.<\/span><\/a><span style=\"font-weight: 400\"> Healthcare professionals who have attended these Death Cafes have described how the <\/span><span style=\"font-weight: 400\">sessions have been significantly different from the usual medical narrative and have allowed them insight into how the public understands death<\/span><span style=\"font-weight: 400\">.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Many organisations providing end of life care ensure their staff and volunteers have regular supervision sessions, which facilitate discussion of their experiences. This dedicates time to reflect on the process of grieving and on any challenging incidents that can be learnt from for future cases.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The Death Caf\u00e9s and hospice supervisions provide models of how healthcare professionals can engage with death in both an emotionally supportive and educational manner. Along with the introduction of regular palliative care rotations, medical schools should incorporate small group sessions to facilitate reflection and learning.
