{"id":48013,"date":"2020-07-10T17:31:55","date_gmt":"2020-07-10T16:31:55","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=48013"},"modified":"2020-07-22T18:24:55","modified_gmt":"2020-07-22T17:24:55","slug":"patients-experiences-of-longcovid-are-missing-from-the-nhs-narrative","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/07\/10\/patients-experiences-of-longcovid-are-missing-from-the-nhs-narrative\/","title":{"rendered":"Patients&#8217; experiences of &#8220;longcovid&#8221; are missing from the NHS narrative"},"content":{"rendered":"<p class=\"standfirst\"><span class=\"il\">Patients and carers must be involved in any initiatives to explore the long term impacts of covid-19<\/span><\/p>\n<p><!--more--><\/p>\n<p><span style=\"font-weight: 400\">As a group of doctors who have chronic covid-19 symptoms, we have been digesting information on social media platforms from <\/span><a href=\"https:\/\/www.youtube.com\/watch?v=IIeOoS_A4c8&amp;feature=youtu.be&amp;fbclid=IwAR2r2Npabsmo5W7H3YllN63jOx9zWSzHAopUD5q-n94Eo2K6uXanZ1WeGck\"><span style=\"font-weight: 400\">thousands of individuals in the UK and worldwide<\/span><\/a><span style=\"font-weight: 400\"> affected by covid symptoms for 16 weeks or more. Support groups have rapidly sprung up on social media in which concerns have been raised about the lack of awareness among NHS doctors, nurses, paramedics and other healthcare professionals with regard to the prolonged, varied, and weird symptoms.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Some of these symptoms and patients\u2019 experiences have been summarised in a video <\/span><a href=\"https:\/\/www.youtube.com\/watch?v=IIeOoS_A4c8&amp;feature=youtu.be&amp;fbclid=IwAR2r2Npabsmo5W7H3YllN63jOx9zWSzHAopUD5q-n94Eo2K6uXanZ1WeGck\"><span style=\"font-weight: 400\">\u201cMessage in a bottle\u2014long covid SOS.\u201d<\/span><\/a><span style=\"font-weight: 400\">\u00a0Many individuals have been reporting symptoms that range from low oxygen saturations, breathlessness, chest pain, pericarditis\/myocarditis, pericardial \/ pleural effusions, fast heart rate with minimal exertion, hoarseness, skin manifestations, new onset dysphasia, acquired dyslexia, headaches, severe fatigue, relapsing fevers, lymphadenopathy, joint pains, sore throat and diarrhoea. Symptoms may arise through several mechanisms including direct organ damage and changes in immune function.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">These wide ranging, unusual, and potentially very serious symptoms can be anxiety provoking, particularly secondary to a virus that has only been known to the world for eight months and which we have barely begun to understand. However, it is dismissive to solely attribute such symptoms to anxiety in the thousands of patients like ourselves who have attended hospital or general practice with chronic covid-19. There is an enormous amount that we still do not know about covid-19. Many symptoms do not appear to be \u201cpost viral\u201d as they emerged with the first manifestations of covid, some symptoms appear later, and it is still uncertain if the relapses represent viraemias or not. Follow-up studies are urgently needed and we\u00a0 (im)patiently await epidemiological data on <\/span><a href=\"https:\/\/covid.joinzoe.com\/\"><span style=\"font-weight: 400\">long term sequelae of covid<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The stories from online support groups have revealed a frustrating lack of access to appropriate investigations for symptomatic individuals and their GPs due to lockdown and a reduction of services. But one thing has been striking to us when listening to the narratives of people trying to get their doctors to listen to their concerns: although some have excellent doctors who listen sympathetically, use their clinical acumen, and order the necessary investigations that they can from secondary care, many affected individuals have been dismissed with the label of \u201canxiety\u201d and have endured incredulity and a lack of sympathy or support. This concerns us greatly as clinicians and also as we are involved in undergraduate and postgraduate teaching of professional behaviours. It is unclear why some patients have struggled to get the necessary support.\u00a0 It may be partly due to a lack of knowledge, a lack of clinical guidance on how to manage chronic covid-19, as well as burnout or clinician anxiety when faced with an infection whose symptoms and treatment are poorly understood.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The announcement of an <\/span><a href=\"https:\/\/www.england.nhs.uk\/2020\/07\/nhs-to-launch-ground-breaking-online-covid-19-rehab-service\/\"><span style=\"font-weight: 400\">NHS portal<\/span><\/a><span style=\"font-weight: 400\"> for patients who have been admitted to hospital or dealt with the illness at home is a welcome signal that the problems of long standing covid symptoms are starting to be recognised. However, any such initiative should have input from individuals who have the symptoms and should not be a purely top-down venture. We are still not reassured about what form the assistance will take and concerned that the NHS coronavirus website still does not give a full and accurate list of symptoms. <\/span><a href=\"https:\/\/www.cochrane.org\/news\/new-cochrane-review-assesses-how-accurate-antibody-tests-are-detecting-covid-19\"><span style=\"font-weight: 400\">Antibody tests may be unreliable especially when measured after 5 weeks of infection<\/span><\/a><span style=\"font-weight: 400\"> and thus many diagnostic uncertainties remain. Hence clinical acumen is required in the diagnosis and individualised management of the varied manifestation of long covid. We know from research that this virus is not just a \u201crespiratory virus.\u201d Evidence suggests that it affects the <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/laneur\/article\/PIIS1474-4422(20)30221-0\/fulltext\"><span style=\"font-weight: 400\">brain<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCRESAHA.120.317055\"><span style=\"font-weight: 400\">heart<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMc2018688\"><span style=\"font-weight: 400\">pancreas<\/span><\/a><span style=\"font-weight: 400\">, <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/www.sciencedaily.com\/releases\/2020\/07\/200702113716.htm\">skin<\/a>, <\/span><a href=\"https:\/\/www.medscape.com\/viewarticle\/931197\"><span style=\"font-weight: 400\">thyroid<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/gut.bmj.com\/content\/69\/6\/973\"><span style=\"font-weight: 400\">gut<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.england.nhs.uk\/coronavirus\/wp-content\/uploads\/sites\/52\/2020\/03\/c0299-acute-kidney-injury-in-covid-outside-the-icu-version-v1-1.pdf\"><span style=\"font-weight: 400\">kidneys<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7232908\/\"><span style=\"font-weight: 400\">musculoskeletal<\/span><\/a><span style=\"font-weight: 400\"> system. So, we would urge that this new portal should be co-created with by patients with covid-19 and carers. There needs to be some bidirectionality in the creation of this service and subsequent research to avoid institutional \u201ctop down\u201d blind spots about the condition. We have been encouraged to find really interesting patient led research about long covid and antibody testing e.g. <\/span><a href=\"https:\/\/patientresearchcovid19.com\/research\/report-1\/\"><span style=\"font-weight: 400\">Slack Body Politic group<\/span><\/a><span style=\"font-weight: 400\">, and all of this patient led data could be the foundation for supporting the rehabilitation of those affected by prolonged symptoms of covid-19.
