Every article and webcast about covid-19 has the same message: \u201cWe are still learning; we don\u2019t know enough yet.\u201d However, out of this sea of information, protean patterns of illness are emerging. Doctors, governments, and countries have made huge efforts to treat patients and prevent disease spread, but given the novelty of this disease, we should consider what we can learn from the information we have so far, and what more can be done to help in the supportive management and prevention of new cases. <\/span><\/p>\n<p><span style=\"font-weight: 400\">So far, there has been much emphasis on the treatment of acute life-threatening manifestation of covid-19. However, we are seeing increasing evidence of a \u201clong-tail\u201d of covid-19 related illness, and we need to consider how to support patients with prolonged illness from covid-19. As a group of authors, we have become interested in this area from a personal and professional perspective. CR and AUL have both experienced a long-tail of covid-19 symptoms and MM through her experience treating people with these symptoms in general practice.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Many individuals not admitted to hospital are reporting a prolonged and debilitating course of illness. Doctors have highlighted their experience of prolonged symptoms. <\/span><a href=\"https:\/\/inews.co.uk\/news\/health\/coronavirus-covid-19-symptoms-uk-latest-fatigue-breathlessness-long-term-effects-2860360?utm_source=fb&amp;utm_medium=fb&amp;utm_campaign=ijp&amp;fbclid=IwAR2W1F1CPduV5OsmdFDBiPTYpOy1R2YMk8tBd9P1HdN1CvyAAMN2gFyZB9A\"><span style=\"font-weight: 400\">[1]<\/span><\/a> <a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/05\/05\/paul-garner-people-who-have-a-more-protracted-illness-need-help-to-understand-and-cope-with-the-constantly-shifting-bizarre-symptoms\/\"><span style=\"font-weight: 400\">[2]<\/span><\/a> <a href=\"https:\/\/www.sciencemag.org\/news\/2020\/05\/finally-virus-got-me-scientist-who-fought-ebola-and-hiv-reflects-facing-death-covid-19\"><span style=\"font-weight: 400\">[3]<\/span><\/a> <span style=\"font-weight: 400\">Patient led-research is already underway with the publication of data on protracted symptoms, such as the data from the Body Politic Covid-19 Slack support group. <\/span><a href=\"https:\/\/docs.google.com\/document\/d\/1KmLkOArlJem-PArnBMbSp-S_E3OozD47UzvRG4qM5Yk\/edit\"><span style=\"font-weight: 400\">[4]<\/span><\/a><span style=\"font-weight: 400\"> After sharing our recent BMJ rapid response on social media, many other people shared their experience of symptoms that have lasting up to 13 weeks or more. <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1470\/rr-4\"><span style=\"font-weight: 400\">[5]<\/span><\/a><span style=\"font-weight: 400\"> Among 61 reported symptoms, the most noted persistent symptoms were fatigue and shortness of breath, especially with exertion, chest tightness, cough, skin rash, elevated temperatures, and neurological disturbances. <\/span><a href=\"https:\/\/docs.google.com\/document\/d\/1KmLkOArlJem-PArnBMbSp-S_E3OozD47UzvRG4qM5Yk\/edit\"><span style=\"font-weight: 400\">[4]<\/span><\/a><span style=\"font-weight: 400\"> The Covid-19 symptom tracking app developed at King&#8217;s College London, which has been downloaded by 3.9 million people globally to date, has reported that 10% people had symptoms at 25 days and 5% were still ill one month later. <\/span><a href=\"https:\/\/covid.joinzoe.com\/\"><span style=\"font-weight: 400\">[6]<\/span><\/a><span style=\"font-weight: 400\"> The patterns within prolonged symptoms may not be noticed during acute hospital admissions, nor in the community if longitudinal patient symptom tracking and testing is not carried out. More formal evaluation is necessary and would be useful for clinicians and patients. Actively engaging patients and carers in research design and evaluation is crucial to ensure that people\u2019s experiences are being adequately captured.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Some patients have observed a\u00a0periodic <\/span><span style=\"font-weight: 400\">\u201ctertian\u201d or \u201cquartan\u201d pattern, where symptoms recur every 3<\/span><span style=\"font-weight: 400\">rd<\/span><span style=\"font-weight: 400\"> or 4<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> day<\/span><span style=\"font-weight: 400\">. <\/span><a href=\"https:\/\/docs.google.com\/document\/d\/1KmLkOArlJem-PArnBMbSp-S_E3OozD47UzvRG4qM5Yk\/edit\"><span style=\"font-weight: 400\">[4]<\/span><\/a> <span style=\"font-weight: 400\">As patients recover from covid-19, individuals say that they experience less intense recurrences of their original symptom \u201cclusters,\u201d<\/span> <span style=\"font-weight: 400\">predominantly at longer intervals. It is unclear if some of these recurrences represent re-activation viraemias or are a consequence of immune<\/span> <span style=\"font-weight: 400\">response.<\/span> <span style=\"font-weight: 400\">Current guidelines for self-isolation of \u201c7 days after start of symptoms\u201d in the UK may not be adequate in many individuals, <\/span><a href=\"https:\/\/www.nhs.uk\/conditions\/coronavirus-covid-19\/what-to-do-if-you-or-someone-you-live-with-has-coronavirus-symptoms\/staying-at-home-if-you-or-someone-you-live-with-has-coronavirus-symptoms\/\"><span style=\"font-weight: 400\">[7]<\/span><\/a><span style=\"font-weight: 400\"> particularly with reports of prolonged faecal viral shedding up to 1 month for some patients. <\/span><a href=\"https:\/\/www.ecdc.europa.eu\/sites\/default\/files\/documents\/covid-19-guidance-discharge-and-ending-isolation-first%20update.pdf\"><span style=\"font-weight: 400\">[8]<\/span><\/a> <a href=\"https:\/\/gut.bmj.com\/content\/69\/6\/973\"><span style=\"font-weight: 400\">[9]<\/span><\/a><span style=\"font-weight: 400\"> However, longitudinal assessments of serial PCR testing currently undertaken largely through oropharyngeal swabs, (or stool samples) are not routine community practice, apart from some selected settings such as care homes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Despite covid-19 testing being widely available, concerns remain around current limited sensitivity and specificity of Covid-19 PCR swab tests due to the timing and technique in collecting the swab samples. <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1403.long\"><span style=\"font-weight: 400\">[10]<\/span><\/a><span style=\"font-weight: 400\"> Highly specific IgG antibody tests have been developed, however uncertainty remains about the proportion of covid-19 patients that seroconvert and produce IgG antibodies after infection. Further uncertainty exists around level and duration of immunity and carrier risk after covid-19 recovery. <\/span><a href=\"https:\/\/www.sciencemediacentre.org\/expert-reaction-to-phe-laboratory-evaluations-of-roche-and-abbott-antibody-tests\/\"><span style=\"font-weight: 400\">[11]<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400\">UK prevalence studies suggest that 5% of the national population and 17% of London\u2019s population have developed IgG antibodies. <\/span><a href=\"https:\/\/www.gov.uk\/government\/news\/government-begins-large-scale-study-of-coronavirus-immunity\"><span style=\"font-weight: 400\">[12]<\/span><\/a> <a href=\"https:\/\/metro.co.uk\/2020\/05\/21\/17-londoners-5-rest-uk-have-coronavirus-antibodies-12739901\/\"><span style=\"font-weight: 400\">[13]<\/span><\/a><span style=\"font-weight: 400\"> For individuals with confirmed covid-19 infections, high initial antibody conversion rates have been found in small numbers of hospital patients. <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41586-020-2196-x\"><span style=\"font-weight: 400\">[14]<\/span><\/a> <span style=\"font-weight: 400\">\u00a0WHO guidance currently recommends return to work after 2 weeks of absence of symptoms. <\/span><a href=\"https:\/\/www.who.int\/publications-detail\/home-care-for-patients-with-suspected-novel-coronavirus-(ncov)-infection-presenting-with-mild-symptoms-and-management-of-contacts\"><span style=\"font-weight: 400\">[15]<\/span><\/a><span style=\"font-weight: 400\"> However, some staff who have returned to work during a quiescent phase are unable to continue work because of the resurgence of symptoms. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Prolonged covid-19 illness follows a distinct pattern, reflecting a cluster of symptoms of longer duration and of unknown cause. <\/span><span style=\"font-weight: 400\">The presentation of this periodic pattern of symptoms could help improve understanding of the nature of the virus-host interaction in covid-19 pathogenesis, and guide future targeted assessment, management, and treatment interventions.<\/span> <span style=\"font-weight: 400\">It is now apparent that this is a multi-system disorder with a far wider range of symptoms than currently identified on the NHS website. <\/span><a href=\"https:\/\/www.nhs.uk\/conditions\/coronavirus-covid-19\/symptoms\/\"><span style=\"font-weight: 400\">[16]<\/span><\/a><span style=\"font-weight: 400\"> Rather than solely relying on diagnostic tests, clinical acumen is crucial to unravel the covid-19 puzzle. Those with less severe relapsing symptoms may not present to general practice or may present at varying timepoints.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The dual hallmarks of prolonged illness with relapsing and remitting pattern of recurrence have significant implications for the individual, who needs care and advice. Consequences may include a prolonged sickness absence and multidisciplinary health needs. <\/span><span style=\"font-weight: 400\">A successful recovery requires a gradual rehabilitation and an individualised return to work<\/span> <span style=\"font-weight: 400\">plan. As our understanding develops on the length of time that symptoms persist, there may be further health implications relevant to immunity, infectivity, and return to work.
