Writing under the headline \u201c<\/span><span style=\"font-weight: 400\">Med School Needs an Overhaul: Doctors Should Learn to Fight Pandemics, Not Injustice,\u201d<\/span><span style=\"font-weight: 400\"> Stanley Goldfarb advocates for gutting \u201csocial and organizational topics\u201d from medical school curricula. He argues that teaching students about these subjects distracts from necessary training in areas like pandemic response and critical care. <\/span><span style=\"font-weight: 400\">\u201cThe medical profession,\u201d Goldfarb maintains, \u201cshould abandon the fantasy that physicians can be trained to solve the problems of poverty, food insecurity, and racism.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We have heard the argument that doctors need to \u201c<\/span><a href=\"https:\/\/www.npr.org\/2018\/11\/11\/666762890\/after-nra-mocks-doctors-physicians-reply-this-is-our-lane\"><span style=\"font-weight: 400\">stay in their lane<\/span><\/a><span style=\"font-weight: 400\">\u201d before. And it has never been more wrong than now.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Covid-19 has acted for months as \u201cthe great unequalizer,\u201d <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/05\/18\/nyregion\/coronavirus-deaths-nyc.html\"><span style=\"font-weight: 400\">ravaging the world\u2019s most vulnerable communities<\/span><\/a><span style=\"font-weight: 400\">. More recently in the US, the long history and ongoing reality of racism against black people has once again resulted in violence with the brutal killings of <\/span><a href=\"https:\/\/www.courier-journal.com\/story\/opinion\/2020\/05\/14\/breonna-taylor-ahmaud-arbery-deaths-demonstrate-public-heath-crisis\/3121788001\/\"><span style=\"font-weight: 400\">Breonna Taylor<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/undark.org\/2020\/05\/12\/health-lesson-ahmaud-arbery-murder\/\"><span style=\"font-weight: 400\">Ahmaud Arbery<\/span><\/a><span style=\"font-weight: 400\">, and <\/span><a href=\"https:\/\/www.apha.org\/topics-and-issues\/health-equity\/racism-and-health\"><span style=\"font-weight: 400\">George Floyd<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Critical care textbooks do not contain solutions to the interpersonal and systemic injustices that create unequal health outcomes for black and brown people. If we wish to effectively tackle emergencies like those facing us today, we must strengthen\u2014not curtail\u2014doctors\u2019 understanding of the social, economic, and political factors that shape health. Medicine will always be social, and even more so in times of crisis.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The call to pare back these subjects in medical school rests on the assumption that medicine is entirely applied biology. In this view, doctors should operate exclusively as scientists and technicians, avoiding the muddy world of lived reality. But<\/span><span style=\"font-weight: 400\"> it is foolish to imagine we can separate a person\u2019s individual biology from their connections to their family, community, and environment. When we sever individuals from their surroundings, we end up ignoring the <\/span><a href=\"https:\/\/www.annualreviews.org\/doi\/abs\/10.1146\/annurev-publhealth-031210-101218\"><span style=\"font-weight: 400\">social determinants of health<\/span><\/a><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">factors like economic stability, physical environment, education, food security, and social support<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">that in fact determine <\/span><a href=\"https:\/\/www.countyhealthrankings.org\/explore-health-rankings\/measures-data-sources\"><span style=\"font-weight: 400\">a majority of variation in health outcomes<\/span><\/a><span style=\"font-weight: 400\">. <\/span><\/p>\n<p><span style=\"font-weight: 400\">As physicians, we must recognize social determinants as powerful risk factors for poor health outcomes\u2014as important as any biomarker or abnormal lab result. We must learn to treat social determinants as active dangers to health, in the same way we treat smoking cigarettes as harmful to the body. Above all, we must learn the tools to heal the wounds they cause.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Covid-19 has confounded any claim that social problems are not the problems of the healthcare workforce and the larger healthcare system. Like HIV\/AIDS, Ebola, and other infectious epidemics, the novel coronavirus has <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/03\/15\/world\/europe\/coronavirus-inequality.html\"><span style=\"font-weight: 400\">preyed on and exacerbated underlying inequality<\/span><\/a><span style=\"font-weight: 400\">.<\/span> <a href=\"https:\/\/www.apmresearchlab.org\/covid\/deaths-by-race\"><span style=\"font-weight: 400\">Race and ethnicity<\/span><\/a><span style=\"font-weight: 400\"> profoundly influence who gets sick, and how severely. Black and Latinx people bear a <\/span><a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2020\/05\/30\/865413079\/what-do-coronavirus-racial-disparities-look-like-state-by-state\"><span style=\"font-weight: 400\">far greater burden<\/span><\/a><span style=\"font-weight: 400\"> of illness and death than their white counterparts. Across the US, residential segregation makes a person\u2019s <\/span><a href=\"https:\/\/www.buzzfeednews.com\/article\/johntemplon\/detailed-coronavirus-maps-by-zip-code-major-cities\"><span style=\"font-weight: 400\">postal code<\/span><\/a><span style=\"font-weight: 400\"> a significant predictor of morbidity. Racial and economic inequity means that patients from the communities hardest hit by coronavirus paradoxically face <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/05\/10\/us\/coronavirus-african-americans-bias.html\"><span style=\"font-weight: 400\">more challenges<\/span><\/a><span style=\"font-weight: 400\"> in accessing testing, leading to <\/span><a href=\"https:\/\/abcnews.go.com\/Health\/beloved-brooklyn-teacher-30-dies-coronavirus-denied-covid\/story?id=70376445\"><span style=\"font-weight: 400\">misdiagnosis and preventable death<\/span><\/a><span style=\"font-weight: 400\">.<\/span> <a href=\"https:\/\/www.theatlantic.com\/ideas\/archive\/2020\/05\/essentials-meatpeacking-coronavirus\/611437\/\"><span style=\"font-weight: 400\">Blue collar jobs<\/span><\/a><span style=\"font-weight: 400\"> like meat packing carry serious risks for workers, who often have little choice in matters of distancing or other precautions.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Physicians can only provide good care if they can anticipate and address these inequalities. Awareness of systemic injustice allows us to empathically approach the critically ill breadwinning grandfather of a large immigrant family or the undomiciled patient who fears a decline when he returns to his homeless shelter. It permits us to recognize and address our own biases. It motivates us to look for solutions, like free access to and equitable allocation of <\/span><a href=\"https:\/\/www.chcf.org\/blog\/striving-equity-covid-19-testing\/\"><span style=\"font-weight: 400\">essential testing and treatment<\/span><\/a><span style=\"font-weight: 400\">, greater protections for <\/span><a href=\"https:\/\/nationalcosh.org\/coronavirus\"><span style=\"font-weight: 400\">laborers at risk<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/ajph.aphapublications.org\/doi\/pdf\/10.2105\/AJPH.2020.305707\"><span style=\"font-weight: 400\">prison health reform<\/span><\/a><span style=\"font-weight: 400\">, policies <\/span><a href=\"https:\/\/www.latimes.com\/politics\/story\/2020-05-22\/coronavirus-shutdowns-consumers-unpaid-utility-bills-loom-as-costly-problem\"><span style=\"font-weight: 400\">preventing utility suppliers from cutting off<\/span><\/a><span style=\"font-weight: 400\"> people who are vulnerable, funding for <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp2005638\"><span style=\"font-weight: 400\">nutrition programs<\/span><\/a><span style=\"font-weight: 400\">, and guarantees of <\/span><a href=\"https:\/\/www.citylab.com\/equity\/2020\/05\/pay-rent-eviction-ban-coronavirus-housing-crisis-landlord\/612277\/\"><span style=\"font-weight: 400\">secure housing<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the end, if we have fallen short in our care of covid-19 patients, it will not be because we lacked training in emergency intubation or ventilator management. It will be because we failed to confront the pandemic\u2019s social and ethical challenges\u2014precisely the kinds of challenges that medical education needs to prepare us for.<\/span><\/p>\n<p><span style=\"font-weight: 400\">More broadly, social education in medicine can sensitize medical trainees to public health crises like the epidemics of <\/span><a href=\"https:\/\/mappingpoliceviolence.org\/\"><span style=\"font-weight: 400\">police violence<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.sentencingproject.org\/criminal-justice-facts\/\"><span style=\"font-weight: 400\">mass incarceration<\/span><\/a><span style=\"font-weight: 400\"> in the US. Police use of force is a <\/span><a href=\"https:\/\/www.pnas.org\/content\/116\/34\/16793\"><span style=\"font-weight: 400\">leading cause of death<\/span><\/a><span style=\"font-weight: 400\"> for young black men, as well as a major contributor to <\/span><a href=\"https:\/\/spssi.onlinelibrary.wiley.com\/doi\/epdf\/10.1111\/josi.12251\"><span style=\"font-weight: 400\">individual and collective trauma<\/span><\/a><span style=\"font-weight: 400\">. Compared with white men, black men face <\/span><a href=\"https:\/\/www.sentencingproject.org\/publications\/color-of-justice-racial-and-ethnic-disparity-in-state-prisons\/\"><span style=\"font-weight: 400\">higher rates of imprisonment<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.ussc.gov\/research\/research-reports\/demographic-differences-sentencing\"><span style=\"font-weight: 400\">longer sentences for the same crimes<\/span><\/a><span style=\"font-weight: 400\">, producing greater exposure to the <\/span><a href=\"https:\/\/theconversation.com\/how-mass-incarceration-harms-u-s-health-in-5-charts-90674\"><span style=\"font-weight: 400\">detrimental health effects of incarceration<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Physicians with a critical approach to the social world are more likely to <\/span><a href=\"https:\/\/www.statnews.com\/2020\/06\/05\/physician-advocacy-against-police-brutality-thisisourlanetoo\/\"><span style=\"font-weight: 400\">speak up<\/span><\/a><span style=\"font-weight: 400\"> about the health effects of violent law enforcement. Following the death of George Floyd, the American Medical Association <\/span><a href=\"https:\/\/www.ama-assn.org\/about\/leadership\/police-brutality-must-stop\"><span style=\"font-weight: 400\">denounced racialized police brutality<\/span><\/a><span style=\"font-weight: 400\">, and the American Academy of Ophthalmology <\/span><a href=\"https:\/\/www.aao.org\/newsroom\/news-releases\/detail\/nation-s-ophthalmologists-condemn-use-of-tear-gas-\"><span style=\"font-weight: 400\">condemned the use of rubber bullets<\/span><\/a><span style=\"font-weight: 400\"> to quash peaceful protests. These are just two recent examples. Work by the student-led group <\/span><a href=\"https:\/\/whitecoats4blacklives.org\/\"><span style=\"font-weight: 400\">White Coats for Black Lives<\/span><\/a><span style=\"font-weight: 400\"> illustrates how medical student advocacy for racial justice can be impactful and enduring.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Without an understanding of the <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27680318\/\"><span style=\"font-weight: 400\">personal, interpersonal, and structural dimensions<\/span><\/a><span style=\"font-weight: 400\"> of racial inequality, clinicians cannot adequately address what ails our patients and our communities. We cannot comprehend why suffering follows such predictable patterns, or how we must go beyond individual technical solutions to heal systemic pathologies. At a time of widespread public health crisis, as leaders at the highest levels sow civic discord, medical schools should teach students to fight for care and safety for all.<\/span><\/p>\n<p><span style=\"font-weight: 400\">This is why the Centers for Disease Control and Prevention is compiling <\/span><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/need-extra-precautions\/racial-ethnic-minorities.html\"><span style=\"font-weight: 400\">resources on covid-19 in minority groups<\/span><\/a><span style=\"font-weight: 400\">. It is why academics are insisting we <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp2012910\"><span style=\"font-weight: 400\">contextualize the pandemic\u2019s racial disparities<\/span><\/a><span style=\"font-weight: 400\"> in relation to social factors like poverty, chronic stress, and location. It is why <\/span><a href=\"https:\/\/www.statnews.com\/2020\/06\/05\/physician-advocacy-against-police-brutality-thisisourlanetoo\/\"><span style=\"font-weight: 400\">physicians<\/span><\/a><span style=\"font-weight: 400\"> are calling for an end to police brutality, and why <\/span><a href=\"https:\/\/twitter.com\/ZarinIRahman\/status\/1266025461370556416?s=20\"><span style=\"font-weight: 400\">medical students<\/span><\/a><span style=\"font-weight: 400\"> are demanding we address it directly in medical education.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Why should doctors bother grappling with injustice? Because doing so will save lives.
