{"id":47717,"date":"2020-06-08T08:54:29","date_gmt":"2020-06-08T07:54:29","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=47717"},"modified":"2020-06-10T17:38:12","modified_gmt":"2020-06-10T16:38:12","slug":"structural-racism-in-society-and-the-covid-19-stress-test","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/06\/08\/structural-racism-in-society-and-the-covid-19-stress-test\/","title":{"rendered":"Structural racism in society and the covid-19 \u201cstress test\u201d"},"content":{"rendered":"<p><span style=\"font-weight: 400\">Hot on the heels of <\/span><i><span style=\"font-weight: 400\">The <\/span><\/i><i><span style=\"font-weight: 400\">BMJ<\/span><\/i><span style=\"font-weight: 400\">\u2019s special edition on <a href=\"https:\/\/www.bmj.com\/racism-in-medicine\">Racism in Medicine<\/a>, the covid-19 pandemic has produced a \u201cstress test\u201d for everyone concerned about structural racism in healthcare. The higher mortality rates from covid-19 for people from ethnic minority backgrounds across the general population and in health and social care roles has been widely documented. Scrutinising the causes of health and social inequality is critical to mitigating risk and preventing further deaths. It is 20 years since the <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/277111\/4262.pdf\"><span style=\"font-weight: 400\">Macpherson<\/span><\/a><span style=\"font-weight: 400\"> inquiry into the <\/span><a href=\"https:\/\/www.theguardian.com\/uk-news\/2019\/feb\/22\/macpherson-report-what-was-it-and-what-impact-did-it-have\"><span style=\"font-weight: 400\">racially motivated death of Stephen Lawrence<\/span><\/a><span style=\"font-weight: 400\">, which brought attention to the problem of institutional racism. Our confidence to identify, name, and act on racism appears to be dwindling. <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/889861\/disparities_review.pdf\"><span style=\"font-weight: 400\">Public Health England\u2019s<\/span><\/a><span style=\"font-weight: 400\"> recent inquiry into the disproportionate impact of covid- 19 on ethnic minority communities served to confirm<\/span><span style=\"font-weight: 400\"> data, <\/span><span style=\"font-weight: 400\">but was a missed opportunity. Unless we overcome the persistent fragility and discomfort surrounding discussions of discrimination at institutional and individual levels, findings, recommendations, and actions will be limited.<\/span><\/p>\n<p><span style=\"font-weight: 400\">We must avoid inquiries based solely on biological determinism. Race is a <\/span><a href=\"https:\/\/jech.bmj.com\/content\/55\/10\/693\"><span style=\"font-weight: 400\">social<\/span><\/a><span style=\"font-weight: 400\">, not biological construct.\u00a0 While biological factors may in part contribute to the disproportionate burden of covid- 19 mortality in people from ethnic minorities, we must recognise many underlying factors are linked to social determinants that in turn are directly influenced by racism. If we regard race as a social construct, we will move away from reductionist assumptions, to ways of reasoning which account for historical and contextual legacies of power, privilege, discrimination and oppression in UK society as well as in our healthcare and social systems.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/277111\/4262.pdf\"><span style=\"font-weight: 400\">Macpherson<\/span><\/a><span style=\"font-weight: 400\"> defined institutional racism as \u201cThe collective failure of an organisation to provide an appropriate and professional service to people because of their colour, culture, or ethnic origin. It can be seen or detected in processes, attitudes and behaviour which amount to discrimination through unwitting prejudice, ignorance, thoughtlessness and racist stereotyping which disadvantage minority ethnic people.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There is well documented evidence of <\/span><a href=\"https:\/\/www.bmj.com\/racism-in-medicine\"><span style=\"font-weight: 400\">racism<\/span><\/a><span style=\"font-weight: 400\"> in healthcare, the NHS, and medical education. Within the population there is inequitable access to healthcare, provision, treatment and outcomes across a number of areas by <\/span><a href=\"https:\/\/www.bmj.com\/content\/368\/bmj.m530\"><span style=\"font-weight: 400\">ethnicity<\/span><\/a><span style=\"font-weight: 400\">. Healthcare workers from ethnic minority backgrounds face differential training and career progression, pay levels and complaints processes, as well as bullying and harassment <\/span><a href=\"https:\/\/www.bmj.com\/content\/368\/bmj.m530\"><span style=\"font-weight: 400\">at work<\/span><\/a><span style=\"font-weight: 400\">. There is also differential attainment between students from ethnic minorities and white <\/span><a href=\"https:\/\/www.bmj.com\/content\/368\/bmj.m339\"><span style=\"font-weight: 400\">medical students<\/span><\/a><span style=\"font-weight: 400\">, with questions raised whether some medical schools are \u201cturning a blind eye to <\/span><a href=\"https:\/\/www.bmj.com\/content\/368\/bmj.m420\"><span style=\"font-weight: 400\">racism<\/span><\/a><span style=\"font-weight: 400\">\u201d. Chaand <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/20\/chaand-nagpaul-the-disproportionate-impact-of-covid-19-on-ethnic-minority-healthcare-workers\/\"><span style=\"font-weight: 400\">Nagpaul<\/span><\/a><span style=\"font-weight: 400\">, chair of BMA Council, <\/span><span style=\"font-weight: 400\">highlighted data that show \u201cdoctors from an ethnic minority are nearly twice as likely to say that they would not feel confident in raising workplace safety concerns, and fear being unfairly blamed or suffering adverse consequences if they did so.\u201d\u00a0Many healthcare workers from ethnic minorities are operating in a culture of fear.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The current protests in the US, and across the world, and the focus on \u201cBlack Lives Matter\u201d after the tragic death of <\/span><a href=\"https:\/\/www.newyorker.com\/news\/daily-comment\/the-death-of-george-floyd-in-context\"><span style=\"font-weight: 400\">George Floyd<\/span><\/a><span style=\"font-weight: 400\"> more than ever demands that we look beyond inadequate specious responses when structural racism is laid bare. To be successful we need to name racism as a determinant of health and invest significant effort to identify and dismantle bias and discrimination in our health systems and institutions, as well as at an individual level. Racism has and continues to drive health and social inequality in the UK and is, we believe, a likely co-factor contributing to deaths in ethnic minority communities in this crisis. We must widen our investigative frameworks beyond biological determinism, and avoid unintended scientific racism.
