{"id":47630,"date":"2020-05-27T17:40:55","date_gmt":"2020-05-27T16:40:55","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=47630"},"modified":"2020-05-28T14:24:05","modified_gmt":"2020-05-28T13:24:05","slug":"guillermo-d-pineda-why-the-us-needs-immigrants-in-medicine-more-than-ever","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/05\/27\/guillermo-d-pineda-why-the-us-needs-immigrants-in-medicine-more-than-ever\/","title":{"rendered":"Guillermo D Pineda: Why the US needs immigrants in medicine more than ever"},"content":{"rendered":"<p class=\"standfirst\">The covid-19 pandemic has underscored the contributions of immigrant physicians to US healthcare, says Guillermo D Pineda<\/p>\n<p><!--more--><span style=\"font-weight: 400\"><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-47637 alignleft\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/05\/Guillermo-Pineda.jpg\" alt=\"\" width=\"137\" height=\"126\" \/>Last semester\u2014my first as a medical student\u2014I shadowed a family medicine doctor in Northwest Houston, Texas. His patients were mostly immigrants from Mexico or Central America, many of whom only spoke Spanish. As I helped to translate for them, I saw how important it was for doctors to share linguistic and cultural commonality with their patients. Patients could better express what was ailing them and were more likely to trust our recommendations. <\/span><\/p>\n<p><span style=\"font-weight: 400\">I think about those patients a lot these days. Most of them work in<\/span><a href=\"https:\/\/research.newamericaneconomy.org\/report\/undocumented-immigrants-covid-19-crisis\/\"> <span style=\"font-weight: 400\">high risk jobs<\/span><\/a><span style=\"font-weight: 400\"> like construction, delivery, or cleaning. They don\u2019t have the luxury to<\/span><a href=\"https:\/\/www.houstonchronicle.com\/news\/houston-texas\/houston\/article\/job-loss-coronavirus-aid-pandemic-covid-immigrants-15182005.php\"> <span style=\"font-weight: 400\">stay at home<\/span><\/a><span style=\"font-weight: 400\"> right now<\/span><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Every community across the country needs more doctors right now, but immigrant and Latino physicians are particularly important in the county of Texas that I live and study in, where<\/span><a href=\"https:\/\/www.census.gov\/quickfacts\/harriscountytexas\"> <span style=\"font-weight: 400\">43% of the population is Hispanic<\/span><\/a><span style=\"font-weight: 400\">. It\u2019s one of the reasons why I want to become a doctor.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">As a second generation immigrant from Venezuela and a member of the Latino Medical Student Association, I am inspired by other immigrant and Latino physicians. This includes my own parents, who right now are putting their own health at risk to care for their patients. The contributions of immigrant physicians to US healthcare were already crucial before covid-19. According to<\/span><a href=\"https:\/\/research.newamericaneconomy.org\/report\/immigration-and-covid-19\/\"> <span style=\"font-weight: 400\">New American Economy<\/span><\/a><span style=\"font-weight: 400\"> (NAE)<\/span><span style=\"font-weight: 400\">, 16.5% of all US healthcare workers were born in a different country. Immigrants account for nearly a third of all physicians in the US and we need them now more than ever. When we come through this crisis, we will need policies that bolster our immigrant healthcare workforce far into the future.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The Association of American Medical Colleges estimate that the US could face a shortage of<\/span><a href=\"https:\/\/www.aamc.org\/news-insights\/press-releases\/new-findings-confirm-predictions-physician-shortage\"> <span style=\"font-weight: 400\">122 000 physicians<\/span><\/a><span style=\"font-weight: 400\"> by 2032. In<\/span><a href=\"http:\/\/research.newamericaneconomy.org\/wp-content\/uploads\/2015\/09\/lifesupport929-1.pdf\"> <span style=\"font-weight: 400\">Texas, 20 of its 254 counties didn\u2019t have a single physician<\/span><\/a><span style=\"font-weight: 400\"> in 2015\u2014and other states face the same drought. This is largely why states across the country are desperate for medical professionals. Michigan\u2019s<\/span><a href=\"https:\/\/www.metrotimes.com\/news-hits\/archives\/2020\/03\/31\/gov-whitmer-asks-medical-personnel-to-come-to-michigan-to-help-fight-coronavirus\"> <span style=\"font-weight: 400\">governor recently pleaded<\/span><\/a><span style=\"font-weight: 400\">, \u201cIf you\u2019re a health professional anywhere in America, Michigan needs you. We\u2019re calling on doctors, and nurses, and respiratory therapists, and other health professionals to sign up and help us fight covid-19 and save lives.\u201d New York\u2019s governor Andrew Cuomo has <a href=\"https:\/\/www.npr.org\/sections\/coronavirus-live-updates\/2020\/03\/30\/823970536\/cuomo-makes-plea-to-medical-workers-nationwide-please-come-help-us-in-new-york\">also called on healthcare workers<\/a> to travel to the state to help.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Unfortunately, current immigration policies limit the ability of international healthcare professionals to practice in the US. The<\/span><a href=\"https:\/\/www.uscis.gov\/working-united-states\/students-and-exchange-visitors\/conrad-30-waiver-program\"> <span style=\"font-weight: 400\">Conrad 30 Waiver Program<\/span><\/a><span style=\"font-weight: 400\">, for example, gives only 30 waivers a year to each state for immigrant physicians who have completed a US residency. Other waivers, such as those obtained through the federal government\u2019s Department of Veterans Affairs, are particularly cumbersome, requiring extensive paperwork, approvals, and months of waiting. Without a waiver, medical graduates must return home for two years before applying for an H-1B visa sponsored by their employer.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">To make this convoluted process even more discouraging, under the Trump administration, there have been reports of a growing number of international medical residents experiencing visa<\/span><a href=\"https:\/\/time.com\/5299488\/international-medical-graduates\/\"> <span style=\"font-weight: 400\">rejections or delays<\/span><\/a><span style=\"font-weight: 400\">. This prevents highly qualified physicians from serving our rapidly growing patient population, especially in a state like Texas with 29 million residents.<\/span><\/p>\n<p><span style=\"font-weight: 400\">My parents immigrated to the US from Venezuela after medical school in the early 1990s. The US Educational Commission for Foreign Medical Graduates sponsored them for a J-1 visa to attend residency. They then received a waiver to work at a Veterans Administration (VA) Hospital in Columbia, South Carolina, which had an acute need for physicians. This qualified them for H-1B visas and, three years later, green cards.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Today, my dad is an interventional cardiologist and my mom still works for the VA as a general internist. Every day that they&#8217;ve gone in to work during this pandemic, they have known that they\u2019re risking their own lives. Growing up, I witnessed my parents\u2019 passion and dedication to the wellbeing of their patients. I don\u2019t know which type of medicine I\u2019ll practice yet, but I hope to continue working with immigrant populations, giving back to that community whenever possible.<\/span><\/p>\n<p><span style=\"font-weight: 400\">I look forward to joining the ranks of licensed physicians when I graduate in 2023. The threat of this pandemic will be over by then, but the next one could be just around the corner and it\u2019s vital that we\u2019re more prepared. Because ultimately it doesn\u2019t matter where we come from: contagious viruses don\u2019t recognize borders.\u00a0<\/span><\/p>\n<p><i><span style=\"font-weight: 400\"><strong>Guillermo D Pineda<\/strong> is a first year medical student at Baylor College of Medicine in Houston, Texas.
