{"id":47487,"date":"2020-05-12T13:00:47","date_gmt":"2020-05-12T12:00:47","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=47487"},"modified":"2020-05-18T17:18:22","modified_gmt":"2020-05-18T16:18:22","slug":"public-health-inequalities-and-covid-19-looking-beyond-the-lockdown","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/05\/12\/public-health-inequalities-and-covid-19-looking-beyond-the-lockdown\/","title":{"rendered":"Covid-19: Public health policies can reduce inequalities as we come out of lockdown"},"content":{"rendered":"<p class=\"standfirst\"><span style=\"font-weight: 400\">Population-level public health policies have an important part in supporting the transition out of lockdown<\/span><\/p>\n<p><!--more--><span style=\"font-weight: 400\">As the UK moves beyond the first-wave peak, the national conversation surrounding covid-19 has shifted to how to safely lift the lockdown. When should restrictions be relaxed? Who can go back to school or work first? Which limits should stay in place? And how can this be done in a way that\u2019s safe, transparent, data-led, and adaptable as new evidence emerges?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Straight-forward population-level public health policies have an important part in supporting this transition, reducing the impact of covid-19 on inequalities, and ensuring that the health and care system can provide care to people as and when they need it.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/03\/24\/can-we-improve-the-nhss-ability-to-tackle-covid-19-through-emergency-public-health-interventions\/\"><span style=\"font-weight: 400\">We weren\u2019t alone<\/span><\/a><span style=\"font-weight: 400\"> in <\/span><a href=\"https:\/\/www.telegraph.co.uk\/global-health\/climate-and-people\/use-lockdown-break-old-habits-start-afresh-healthier-new-ones\/\"><span style=\"font-weight: 400\">writing about this<\/span><\/a><span style=\"font-weight: 400\"> topic earlier in the pandemic, but progress has been disappointing. We argued for lower speed limits in light of reports <\/span><a href=\"http:\/\/news.met.police.uk\/news\/police-urge-drivers-to-slow-down-as-extreme-speeding-enforcement-soars-401835\"><span style=\"font-weight: 400\">of terrifying driving<\/span><\/a><span style=\"font-weight: 400\">. We suggested raising the legal age for buying tobacco and providing free nicotine replacement. We proposed limiting the amount of alcohol that can be bought per transaction alongside accelerating the introduction of minimum unit pricing in England.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Such policies weren\u2019t chosen at random. As we <\/span><a href=\"https:\/\/www.telegraph.co.uk\/global-health\/climate-and-people\/use-lockdown-break-old-habits-start-afresh-healthier-new-ones\/\"><span style=\"font-weight: 400\">wrote at the time<\/span><\/a><span style=\"font-weight: 400\">, they were selected based on evidence that they<\/span><i><span style=\"font-weight: 400\"> \u201ccould free up NHS capacity and save lives in the short term, as well as improve population health and wellbeing in the medium to long term.\u201d <\/span><\/i><span style=\"font-weight: 400\">It feels as though much of the world has moved on with these discussions, but the UK has largely lagged behind.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.telegraph.co.uk\/news\/2020\/04\/23\/uk-urged-follow-europes-lead-expanding-bike-lanes-help-social\/\"><span style=\"font-weight: 400\">Germany, France, and Italy<\/span><\/a><span style=\"font-weight: 400\"> have all made significant moves to ensure people can use active travel to get to work safely by initiatives ranging from <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/apr\/13\/pop-up-bike-lanes-help-with-coronavirus-social-distancing-in-germany\"><span style=\"font-weight: 400\">temporary cycle lanes<\/span><\/a><span style=\"font-weight: 400\"> to <\/span><a href=\"https:\/\/www.weforum.org\/agenda\/2020\/05\/france-air-pollution-cycling-public-transport-bike-coronavirus\/\"><span style=\"font-weight: 400\">free bike repairs<\/span><\/a><span style=\"font-weight: 400\">. And on the 9<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> May the government somewhat belatedly followed suit when Transport Secretary Grant Shapps announced the first \u00a3250 million of a <\/span><a href=\"https:\/\/www.gov.uk\/government\/news\/2-billion-package-to-create-new-era-for-cycling-and-walking\"><span style=\"font-weight: 400\">\u00a32 billion investment in cycling and walking infrastructure<\/span><\/a><span style=\"font-weight: 400\"> in England. The announcement included the welcome publication of <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/reallocating-road-space-in-response-to-covid-19-statutory-guidance-for-local-authorities\"><span style=\"font-weight: 400\">new local authority statutory guidance<\/span><\/a><span style=\"font-weight: 400\"> on reallocating road space, compelling local government to repurpose roads away from cars and towards bikes and pedestrians, and recommending lower speed limits in built up areas. Such investment and guidance are crucial to ensuring that people don\u2019t crowd public transport or resort to private vehicles when returning to work. But when you consider that <\/span><a href=\"https:\/\/www.itv.com\/news\/granada\/2020-01-28\/greater-manchester-cycling-and-walking-network-revolutionary-report-claims\/\"><span style=\"font-weight: 400\">Greater Manchester\u2019s 1,800 mile-long cycling and walking network<\/span><\/a><span style=\"font-weight: 400\"> alone is estimated at \u00a31.5bn, the promised \u00a32bn is unlikely to fund the necessary long-term shift towards active travel.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Recent reports suggest that as <\/span><a href=\"https:\/\/www.theguardian.com\/society\/2020\/may\/04\/more-than-300000-uk-smokers-may-have-quit-owing-to-covid-19-fears\"><span style=\"font-weight: 400\">many as 300,000 smokers<\/span><\/a><span style=\"font-weight: 400\"> may have quit during the lockdown, with four times as many people actively trying to follow suit. Covid-19 has helped to alter the <\/span><i><span style=\"font-weight: 400\">decisional balance <\/span><\/i><span style=\"font-weight: 400\">in Prochaska and di Clemente\u2019s famous <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10170434\"><span style=\"font-weight: 400\">transtheoretical model of behaviour change\u2014<\/span><\/a><span style=\"font-weight: 400\">shifting people from contemplation to preparation, and from preparation to action with potentially important implications for inequalities. Smoking rates among <\/span><a href=\"https:\/\/digital.nhs.uk\/data-and-information\/publications\/statistical\/statistics-on-smoking\/statistics-on-smoking-england-2019\/part-3-smoking-patterns-in-adults-copy\"><span style=\"font-weight: 400\">unemployed adults are 29% compared with just 15% among those with jobs<\/span><\/a><span style=\"font-weight: 400\">, and people in routine or manual work are over twice as likely to smoke as managers or professionals. One wonders how many additional smokers would have stopped if more support had been made available.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Drinking habits have also changed. <\/span><a href=\"https:\/\/alcoholchange.org.uk\/blog\/2020\/covid19-drinking-during-lockdown-headline-findings\"><span style=\"font-weight: 400\">A survey from the charity, Alcohol Change UK<\/span><\/a><span style=\"font-weight: 400\">, suggests that up to a third of UK adults have reduced how much they drink compared to a fifth that have increased consumption. Whilst welcome, closer reading of the data tell us that those who drink most often are the least likely to have cut back\u2014those dependent drinkers who are most likely to respond to policies such as <\/span><a href=\"https:\/\/www.bmj.com\/content\/366\/bmj.l5274\"><span style=\"font-weight: 400\">minimum unit pricing<\/span><\/a><span style=\"font-weight: 400\">. And alcohol doesn\u2019t just harm the physical and mental health of the individual, it impacts the entire home including the <\/span><a href=\"https:\/\/www.who.int\/violence_injury_prevention\/violence\/world_report\/factsheets\/ft_intimate.pdf\"><span style=\"font-weight: 400\">risk and severity of domestic abuse<\/span><\/a><span style=\"font-weight: 400\"> against women and children.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In addition to the inequalities in covid-19 outcomes <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/deaths\/articles\/coronavirusrelateddeathsbyethnicgroupenglandandwales\/2march2020to10april2020\"><span style=\"font-weight: 400\">by ethnicity<\/span><\/a><span style=\"font-weight: 400\">, there is growing evidence that covid-19 is having a much greater impact on those least able to cope\u2014people who are more deprived and have lower incomes. <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/deaths\/bulletins\/deathsinvolvingcovid19bylocalareasanddeprivation\/deathsoccurringbetween1marchand17april\"><span style=\"font-weight: 400\">Mortality data<\/span><\/a><span style=\"font-weight: 400\"> show that most deprived populations have over double the covid-19 death rate of the least deprived. Economic data suggest that <\/span><a href=\"https:\/\/www.resolutionfoundation.org\/publications\/risky-business\/\"><span style=\"font-weight: 400\">those working in the most insecure and lowest paid jobs<\/span><\/a><span style=\"font-weight: 400\"> are least likely to have been able to work from home, are more affected by loss of employment, and are more vulnerable to housing insecurity. The covid-inspired growth of informal community networks is weighted towards the <\/span><a href=\"https:\/\/www.bennettinstitute.cam.ac.uk\/blog\/social-capital-and-response-covid-19\/\"><span style=\"font-weight: 400\">most socio-economically advantaged parts of the country<\/span><\/a><span style=\"font-weight: 400\">. And the forthcoming global economic recession may mean further <\/span><a href=\"https:\/\/www.health.org.uk\/news-and-comment\/news\/urgent-call-for-1-billion-a-year-to-reverse-cuts-to-public-health-grant\"><span style=\"font-weight: 400\">cuts to national and local government services<\/span><\/a><span style=\"font-weight: 400\"> that protect the most vulnerable in our society.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The repercussions of covid-19 will be felt for years to come. It can\u2019t be used as an excuse for worsening population health and widening inequalities, but instead needs to be an opportunity for a healthier and more equal society. Public health policies taken now can reduce short-term NHS demand, improve population health, and limit the impact on inequalities. And beyond these immediate measures, we need <\/span><a href=\"https:\/\/www.health.org.uk\/publications\/long-reads\/will-covid-19-be-a-watershed-moment-for-health-inequalities\"><span style=\"font-weight: 400\">a long-term government inequalities strategy<\/span><\/a><span style=\"font-weight: 400\"> to support those most acutely affected by covid-19 both now and in the future.<\/span><\/p>\n<p><em><span style=\"font-weight: 400\"><strong>Adam Briggs<\/strong>, Associate Clinical Professor and Honorary Consultant in Public Health, University of Warwick.
