Mahtani and Sean Heneghan<\/b><\/p>\n<p><i><span style=\"font-weight: 400\">\u201cwithout trust we cannot stand\u201d \u2014Confucius<\/span><\/i><\/p>\n<p><span style=\"font-weight: 400\">The UK is now entering its seventh week of lockdown, with no immediate signs of respite. At his first <\/span><a href=\"https:\/\/www.gov.uk\/government\/speeches\/pm-statement-in-downing-street-27-april-2020\"><span style=\"font-weight: 400\">briefing<\/span><\/a><span style=\"font-weight: 400\"> since recovering from his illness, Boris Johnson the UK&#8217;s prime minister made it clear that the nation should \u201ccontain your impatience\u201d as we approach \u201cthe end of the first phase of this conflict.\u201d The prime minister spoke of the need to meet <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/health-52374513\"><span style=\"font-weight: 400\">five tests<\/span><\/a><span style=\"font-weight: 400\"> before moving to a \u201csecond phase\u201d\u2014whatever that may be.<\/span><\/p>\n<p><span style=\"font-weight: 400\">At the daily <\/span><a href=\"https:\/\/www.youtube.com\/watch?v=cZjwM6B1bZo\"><span style=\"font-weight: 400\">Coronavirus briefing<\/span><\/a><span style=\"font-weight: 400\">s, the UK government&#8217;s advisors have, thus far, confirmed that the vast majority of the public is listening, citing the rapid fall and continued lower level of daily use of transport as a surrogate for public adherence to social distancing.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The high adherence is probably influenced by several factors, including <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/may\/02\/fearful-britons-oppose-lifting-lockdown-schools-pubs-restaurants-opinium-poll\"><span style=\"font-weight: 400\">fear<\/span><\/a><span style=\"font-weight: 400\"> of catching or spreading the virus, law abidance and trust that the government is acting in the best interests of the nation. In return, the government has often justified its actions on the basis that they are being \u201cled by the science\u201d\u2014a phrase we have become accustomed to hearing from politicians in recent weeks. And who could argue with this approach? <\/span><a href=\"https:\/\/sciencecouncil.org\/about-science\/our-definition-of-science\/\"><span style=\"font-weight: 400\">Science<\/span><\/a><span style=\"font-weight: 400\">, by its very nature, is built on systematic methods, objectivity, transparency, reproducibility, and critical review\u2014all trustworthy characteristics.<\/span><\/p>\n<p><span style=\"font-weight: 400\">However, the \u201cled by the science\u201d rhetoric has not been without <\/span><a href=\"https:\/\/www.hsj.co.uk\/coronavirus\/the-political-science-of-choices-and-consequences\/7027495.article\"><span style=\"font-weight: 400\">criticism<\/span><\/a><span style=\"font-weight: 400\">. Some of the <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/apr\/24\/coronavirus-whos-who-on-secret-scientific-group-advising-uk-government-sage\"><span style=\"font-weight: 400\">criticisms <\/span><\/a><span style=\"font-weight: 400\">include concern over a lack of transparency about who makes up the membership of the Scientific Advisory Group for Emergencies (SAGE), which provides independent <\/span><a href=\"https:\/\/www.gov.uk\/government\/groups\/scientific-advisory-group-for-emergencies-sage-coronavirus-covid-19-response\"><span style=\"font-weight: 400\">scientific and technical advice to support Government decision-making during emergencies<\/span><\/a><span style=\"font-weight: 400\">. The <a href=\"https:\/\/www.gov.uk\/government\/publications\/scientific-advisory-group-for-emergencies-sage-coronavirus-covid-19-response-membership?utm_source=POLITICO.EU&amp;utm_campaign=56a78425fd-EMAIL_CAMPAIGN_2020_05_04_05_29&amp;utm_medium=email&amp;utm_term=0_10959edeb5-56a78425fd-189845981\">membership of this group<\/a> has now been revealed. Others have criticised what they see as narrow views from the membership, with a lack of involvement from broader public health expertise, as well as <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/apr\/23\/scientists-criticise-uk-government-over-following-the-science\"><span style=\"font-weight: 400\">over-dependence on modelling<\/span><\/a><span style=\"font-weight: 400\"> studies, which may not yet have been subject to critical review and have known <\/span><a href=\"https:\/\/www.the-tls.co.uk\/articles\/problem-modelling-public-policy-coronavirus-paul-collier\/\"><span style=\"font-weight: 400\">limitations<\/span><\/a><span style=\"font-weight: 400\">. In some cases, \u201c<\/span><a href=\"https:\/\/www.thetimes.co.uk\/edition\/news\/coronavirus-lockdown-scientists-challenge-no-10-with-rival-advice-on-covid-19-ndzwlnwvp\"><span style=\"font-weight: 400\">rival<\/span><\/a><span style=\"font-weight: 400\">\u201d groups of scientific experts are coming together to offer advice on easing the lockdown.\u00a0<\/span><\/p>\n<p><b>Public trust in science\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">Reports of such tensions are unlikely to enhance public trust. If anything, recent history should have taught us how fragile the relationship between public trust and science can be. In June 2015, the then UK Chief Medical Officer asked the Academy of Medical Sciences to investigate how to restore public trust in scientific evidence for decision making. At the time, there was concern that widespread debate over government policies for the management of the 2009 swine-flu epidemic, including the impact of stockpiling antiviral drugs, could affect public trust negatively. In their <\/span><a href=\"https:\/\/acmedsci.ac.uk\/file-download\/44970096\"><span style=\"font-weight: 400\">report<\/span><\/a><span style=\"font-weight: 400\">, the Academy recommended \u201copenness in decision-making processes to allow wider society to judge whether decisions are made based on sufficiently robust and relevant evidence.\u201d In 2019, the NHS Health Research Authority (HRA) launched the \u201c<\/span><a href=\"https:\/\/www.hra.nhs.uk\/about-us\/consultations\/make-it-public\/\"><span style=\"font-weight: 400\">Make it Public<\/span><\/a><span style=\"font-weight: 400\">\u201d campaign with the remit of \u201cenhancing public trust in research evidence and enhancing public accountability.\u201d<\/span><\/p>\n<p><b>Losing trust has consequences<\/b><\/p>\n<p><span style=\"font-weight: 400\">Trust in scientific evidence allows the public to make more balanced judgements on the treatments they are offered. Such thinking is imperative now, as the public is faced with significant public health interventions to tackle the covid-19 pandemic. However, despite the public-facing \u201cled by the science\u201d approach, a recent <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/apr\/26\/trust-wanes-in-uk-ministers-handling-of-coronavirus-pandemic-poll\"><span style=\"font-weight: 400\">opinion poll<\/span><\/a><span style=\"font-weight: 400\"> reported a fall in public trust in the UK\u2019s handling of the pandemic. This is worrying; as doubts and a loss of confidence during an outbreak may influence public <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4063987\/\"><span style=\"font-weight: 400\">behavioural<\/span><\/a><span style=\"font-weight: 400\"> responses.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Falling opinion polls may not be the only negative consequences to consider. A new pre-print <\/span><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2020.04.10.20061267v1\"><span style=\"font-weight: 400\">study<\/span><\/a><span style=\"font-weight: 400\"> reports on how the UK public currently perceives and experiences the government\u2019s pandemic strategy. Online focus groups were formed from members of the public, representing a range of genders, ethnicity, ages, and occupational backgrounds. Despite reporting adherence to social distancing measures, the participants reported a loss of meaning and self-worth, possibly explained by a loss of control. They also said a lack of trust in the government, which some accused of \u201cpoliticising\u201d the pandemic. The authors suggested that increasing the public\u2019s sense of control should be part of future policy.\u00a0<\/span><\/p>\n<p><b>A stronger relationship\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">Building trust with the public should be a core theme of the government\u2019s ongoing pandemic strategy, particularly as we look to ease lockdown restrictions. The public will need to trust the government that ministers are sensitive to the fears and anxieties people are experiencing, and have the right strategy to respond. In turn, the government will need to trust the public to implement the next phases of their plan\u2014an example being the high uptake (<\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/may\/03\/work-times-could-be-staggered-to-help-end-covid-19-lockdown-says-shapps\"><span style=\"font-weight: 400\">50-60%<\/span><\/a><span style=\"font-weight: 400\"> of the population) of a mobile app that\u2019s needed to make digital contact tracing possible.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Greater access to information on <\/span><a href=\"https:\/\/www.gov.uk\/government\/groups\/scientific-advisory-group-for-emergencies-sage-coronavirus-covid-19-response\"><span style=\"font-weight: 400\">Government websites<\/span><\/a><span style=\"font-weight: 400\">, daily briefings &#8211; which now include questions from the public\u2014and balanced ministerial media appearances are steps in the right direction to building that trust. But higher levels of engagement, communication that is <\/span><a href=\"https:\/\/www.bbc.co.uk\/radio4\/reith2002\/lecture5.shtml\"><span style=\"font-weight: 400\">open to assessment<\/span><\/a><span style=\"font-weight: 400\"> and high-quality public health education\u2014including the potential strengths and limitations of the \u201cled by science\u201d strategy\u2014may be needed to strengthen the relationship. Despite the ethical and logistical challenges this may present, such efforts may not only enhance trust but also <\/span><a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/15265160903197531?casa_token=TKjGalMU_wIAAAAA%3ACzZkvmAVyzm7xhVeZXJ7URpKRfBopyl5uOSu9t_5y6U-zNQ-_NlwvKl-BqrOkb69Yt5X4g_NgoDjRj4\"><span style=\"font-weight: 400\">contribute<\/span><\/a><span style=\"font-weight: 400\"> to mutually beneficial outcomes and a stronger relationship in the future.<\/span><\/p>\n<p><strong><a href=\"https:\/\/www.phc.ox.ac.uk\/team\/kamal-mahtani\"><i>Kamal R.
