{"id":47350,"date":"2020-04-30T11:05:45","date_gmt":"2020-04-30T10:05:45","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=47350"},"modified":"2020-05-04T16:25:22","modified_gmt":"2020-05-04T15:25:22","slug":"seth-holmes-societies-re-open-pandemic-need-social-solidarity-survive","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/30\/seth-holmes-societies-re-open-pandemic-need-social-solidarity-survive\/","title":{"rendered":"Seth Holmes: As societies re-open in this pandemic, we need social solidarity to survive the summer"},"content":{"rendered":"<p class=\"standfirst\">While we must take \u201cphysical distancing\u201d very seriously, we need the opposite of \u201csocial distancing\u201d to survive this pandemic, argues Seth Holmes<\/p>\n<p><!--more--><a href=\"https:\/\/www.wsj.com\/articles\/european-countries-move-closer-to-reopening-continent-11588095446\"><span style=\"font-weight: 400\"><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-47354 alignleft\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/04\/seth_holmes.png\" alt=\"\" width=\"136\" height=\"160\" \/>As more countries<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/world-us-canada-52435648\"><span style=\"font-weight: 400\">US states move to lift lockdown orders<\/span><\/a><span style=\"font-weight: 400\"> and re-open businesses and other public spaces, many <\/span><a href=\"https:\/\/www.arcgis.com\/apps\/opsdashboard\/index.html#\/bda7594740fd40299423467b48e9ecf6\"><span style=\"font-weight: 400\">people around the world<\/span><\/a><span style=\"font-weight: 400\">\u2014including health professionals and other essential workers\u2014are still being quarantined, hospitalized, and buried.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Public health experts have warned that lifting \u201cshelter in place\u201d and physical distancing measures <\/span><a href=\"https:\/\/www.latimes.com\/california\/story\/2020-04-15\/when-california-stay-at-home-order-can-relax-loosen\"><span style=\"font-weight: 400\">too early could lead to a second<\/span><\/a><span style=\"font-weight: 400\">, potentially greater, surge in infections and deaths in the summer or fall. However, some politicians have revealed how their immediate priorities rest on the <\/span><a href=\"https:\/\/time.com\/5821166\/gop-coronavirus-deaths-economy\/\"><span style=\"font-weight: 400\">health of the economy rather than human life<\/span><\/a><span style=\"font-weight: 400\">. As a physician and social anthropologist, I think it is clear that while we must take \u201cphysical distancing\u201d very seriously, we need the opposite of \u201csocial distancing\u201d to survive this pandemic. We must develop <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/03\/14\/opinion\/coronavirus-social-distancing.html\"><i><span style=\"font-weight: 400\">social solidarity<\/span><\/i><\/a><span style=\"font-weight: 400\"> to survive the summer and beyond.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the social sciences, we understand solidarity to be the <\/span><a href=\"http:\/\/fs2.american.edu\/dfagel\/www\/Class%20Readings\/Durkheim\/Division%20Of%20Labor%20Final%20Version.pdf\"><span style=\"font-weight: 400\">feeling and practice of interdependence<\/span><\/a><span style=\"font-weight: 400\"> between individuals and groups, which can change the ways in which societies are structured, <\/span><a href=\"https:\/\/www.worldcat.org\/title\/pedagogy-of-the-oppressed\/oclc\/439844182\"><span style=\"font-weight: 400\">as well as how each <\/span><\/a><span style=\"font-weight: 400\">individual views themself and acts in relation to other people<\/span><span style=\"font-weight: 400\">. Solidarity can occur <\/span><a href=\"http:\/\/abahlali.org\/wp-content\/uploads\/2011\/04\/Frantz-Fanon-The-Wretched-of-the-Earth-1965.pdf\"><span style=\"font-weight: 400\">across categories of social difference<\/span><\/a><span style=\"font-weight: 400\"> in the form of <\/span><a href=\"https:\/\/www.haymarketbooks.org\/books\/1108-how-we-get-free\"><span style=\"font-weight: 400\">coalition building<\/span><\/a><span style=\"font-weight: 400\">,<\/span><span style=\"font-weight: 400\"> which recognizes the connections between different forms of inequity and highlights the need to stand together for the good of all. Yet, this collective aspect of wellbeing is difficult to achieve in societies where <\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2020\/mar\/15\/america-public-health-system-coronavirus-trump\"><span style=\"font-weight: 400\">individualistic understandings<\/span><\/a><span style=\"font-weight: 400\"> of health and freedom prevail.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">As we approach another month of the covid-19 pandemic, we need social solidarity to protect our mental health.\u00a0Some people are sheltering in place alone, and some people will be feeling depressed or isolated.\u00a0Other people are avoiding their homes to protect loved ones at high risk. Some people are far from their families due to travel restrictions and border closures.\u00a0Other people are continuing to carry out essential work despite their intense fears. In this time of isolation, we need the healing properties of <\/span><a href=\"https:\/\/link.springer.com\/chapter\/10.1007\/978-0-387-71311-3_1\"><span style=\"font-weight: 400\">social cohesion<\/span><\/a><span style=\"font-weight: 400\">\u2014from a distance. Perhaps that means engaging with social media\u2014or avoiding it. Perhaps that includes sending letters, setting up virtual hangouts, or talking by phone. For many, this includes sharing a kind word or smile with neighbors, strangers, and essential workers\u2014from a distance.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">During this pandemic, we also need social solidarity to avoid the harm of anti-social practices, such as <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3682466\/\"><span style=\"font-weight: 400\">stigmatization<\/span><\/a><span style=\"font-weight: 400\">.\u00a0At a time when politicians make free use of racist language, other people are committing <\/span><a href=\"https:\/\/www.pbs.org\/newshour\/show\/what-anti-asian-attacks-say-about-american-culture-during-crisis\"><span style=\"font-weight: 400\">hate crimes<\/span><\/a><span style=\"font-weight: 400\"> against anyone perceived to be related to the entire, diverse continent of Asia.\u00a0President Trump and other politicians have stoked ethno-nationalism and xenophobia, <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/apr\/17\/us-asylum-seekers-coronavirus-law-un\"><span style=\"font-weight: 400\">violating<\/span><\/a><span style=\"font-weight: 400\"> UN conventions, global compacts, and the basic human rights of immigrants and asylum seekers.\u00a0As we approach the summer, we must resist division, come together, and learn to see ourselves and our health in relation to one another. We must confront politicians and corporate executives <\/span><a href=\"https:\/\/campaignlegal.org\/update\/trumps-conflicts-interest-resurface-administrations-response-coronavirus-pandemic\"><span style=\"font-weight: 400\">attempting to benefit from the crisis<\/span><\/a><span style=\"font-weight: 400\">.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">As we debate how we should live in the coming months, we need solidarity to build systems that benefit the health of all.<\/span> <span style=\"font-weight: 400\">What scares many frontline physicians and nurses in this pandemic is not so much the highly infectious virus itself, nor even the severe respiratory condition it causes, but rather <\/span><a href=\"https:\/\/www.salon.com\/2020\/03\/29\/in-a-defunded-health-system-doctors-and-nurses-suffer-near-impossible-conditions\/\"><span style=\"font-weight: 400\">the lack of essential equipment<\/span><\/a><span style=\"font-weight: 400\"> necessary to curb the pandemic and care for patients safely. There have also been reports of the <\/span><a href=\"https:\/\/www.kff.org\/news-summary\/reuters-examines-changes-to-cdc-nsc-epidemiology-pandemic-response-staff-under-trump-administration\/\"><span style=\"font-weight: 400\">US government previously disbanding<\/span><\/a><span style=\"font-weight: 400\"> roles that are critical to epidemic responsiveness. If we want to survive the summer, we must understand how critical our public systems are <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3701298\/\"><span style=\"font-weight: 400\">for all of our health<\/span><\/a><span style=\"font-weight: 400\">. As global health expert Paul Farmer explains, an effective epidemic response requires \u201c<\/span><a href=\"https:\/\/www.axios.com\/paul-farmer-coronavirus-covid19-interview-7750d0ae-7b19-4e9d-be65-16d985ebba1a.html\"><span style=\"font-weight: 400\">staff, stuff, space, and systems<\/span><\/a><span style=\"font-weight: 400\">\u201d\u2014including protection for frontline key workers, accessible testing, and contact tracing. We must build robust health systems to avoid thousands more avoidable infections and deaths in this and future pandemics.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">While in the US, it\u2019s been <\/span><a href=\"https:\/\/www.vox.com\/first-person\/2020\/4\/25\/21234774\/coronavirus-covid-19-protest-anti-lockdown\"><span style=\"font-weight: 400\">primarily white Americans<\/span><\/a><span style=\"font-weight: 400\"> with <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/04\/21\/us\/politics\/coronavirus-protests-trump.html\"><span style=\"font-weight: 400\">support from conservative groups<\/span><\/a><span style=\"font-weight: 400\"> who have been protesting shelter in place orders, <\/span><a href=\"https:\/\/www.theatlantic.com\/ideas\/archive\/2020\/04\/coronavirus-exposing-our-racial-divides\/609526\/\"><span style=\"font-weight: 400\">systematically more people from black and minoritized ethnic <\/span><\/a><span style=\"font-weight: 400\">populations<\/span><span style=\"font-weight: 400\"> have died. Well into the second month since the pandemic was declared by the World Health Organization, we need social solidarity to protect the health of all\u2014including the people most marginalized by society. Many hourly workers, <\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2020\/apr\/14\/will-we-have-food-coronavirus-pandemic\"><span style=\"font-weight: 400\">including those who plant and harvest produce<\/span><\/a><span style=\"font-weight: 400\">, have been left out of protections.\u00a0While we are ordered to physically distance, others have been <\/span><a href=\"https:\/\/ajph.aphapublications.org\/doi\/pdfplus\/10.2105\/AJPH.2020.305704\"><span style=\"font-weight: 400\">separated from their families<\/span><\/a><span style=\"font-weight: 400\"> and locked in <\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2020\/mar\/31\/ice-raids-coronavirus-n95-masks\"><span style=\"font-weight: 400\">overcrowded detention facilities<\/span><\/a><span style=\"font-weight: 400\"> and prisons that are ripe for viral spread. And while we are told to \u201cstay home,\u201d an unprecedented number of people lack any kind of safe housing, with nowhere to \u201cshelter.\u201d For all of us to survive the summer, we need to acknowledge and work to improve the <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(17)32848-9\/fulltext\"><span style=\"font-weight: 400\">social determinants of health for all<\/span><\/a><span style=\"font-weight: 400\"> in our societies.<\/span><\/p>\n<p><span style=\"font-weight: 400\">While covid-19 is more dangerous for certain groups, for any of us to be healthy in a pandemic, we must make sure that all of us are healthy. If we learn anything right now, we must learn that we are all in this together. We must stay connected. To survive the summer and beyond, we must fund public health and social care systems. We must confront racism, xenophobia, and division. Instead of allowing politicians and corporations to benefit from this crisis, we must provide people with safe places to live and healthcare for those who need it most.\u00a0Stop raiding, detaining, and deporting immigrant communities. Avoid causing chaos for those on whom our system relies\u2014including those who provide us all with food.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">This pandemic reminds us that each of us is dependent on the health of everyone else. As we enter the summer, we must have what physician scholar Frantz Fanon referred to as a \u201c<\/span><a href=\"http:\/\/kemetdevelopment.org\/wp-content\/uploads\/2018\/08\/Fanon-Frantz-Toward-the-African-Revolution.pdf\"><span style=\"font-weight: 400\">solidarity of fact, a solidarity of action, a solidarity concrete in men, in equipment, in money<\/span><\/a><span style=\"font-weight: 400\">.\u201d<\/span><\/p>\n<p><em><span style=\"font-weight: 400\"><strong>Seth M Holmes<\/strong> is a physician and social anthropologist at the University of California, Berkeley and University of California, San Francisco.
