If officials want people to buy into lifestyle changes, they need to use more inviting terminology<\/span><span style=\"font-weight: 400\"> say <\/span><span style=\"font-weight: 400\">Joanne Silberner and\u00a0<\/span><span style=\"font-weight: 400\">Howard Frumkin<\/span><\/em><\/p>\n<p><!--more--><\/p>\n<p><span style=\"font-weight: 400\">Staying home. Not hugging or touching friends or lovers. Closing businesses, avoiding recreational facilities, shutting down schools. It takes real commitment to slow the coronavirus pandemic.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">But across the English-speaking world, some of the language deployed in the fight against covid-19 is more daunting than motivational. Some changes in terminology might expedite the needed changes in behaviour. Call it covid-19\u2019s metamorphoses.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Here are our candidates:<\/span><\/p>\n<p><b>Social distancing<\/b><span style=\"font-weight: 400\">: The biggest problem with social distancing is that it\u2019s dangerous. Lonely people are more prone to heart disease, depression, and other maladies<sup>1<\/sup> Loneliness undermines immune function.<sup>2<\/sup> Without social contacts, older people are at risk of cognitive loss.<sup>3<\/sup> Lonely people die sooner.<sup>4<\/sup> Where social bonds are strong, people are more physically active, less obese, and less depressed.<sup>5<\/sup>\u00a0After all, we\u2019re social creatures. People know this, and across the world, they\u2019re connecting with friends, neighbours, and loved ones through phone calls, video conferences, on-line group activities, even singing out the window. So let\u2019s drop the term social distancing, when what we really mean is <\/span><b>physical distancing<\/b><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><b>Essential workers<\/b><span style=\"font-weight: 400\">: <\/span><span style=\"font-weight: 400\">We need doctors and nurses, first responders, hospital cleaners, delivery drivers, grocery store staff, and agricultural workers to keep working so the rest of us can safely cocoon. They are doing so, often heroically. (They are to be distinguished from the Easter bunny, named an essential worker over Easter weekend by the governor of the state of Maine.)<\/span><\/p>\n<p><span style=\"font-weight: 400\">But the term \u201cessential worker\u201d implies that everybody else is inconsequential. Teachers, construction workers, artists, IT staffers, grandparents, restaurant servers, journalists, and homemakers\u2014to name just a few\u2014are essential to a functioning society. So let\u2019s call the people we need to be on the job right now <\/span><b>lifeline workers<\/b><span style=\"font-weight: 400\">, reflecting the immediacy of their work, avoiding ugly implications about who is essential and who is not.<\/span><\/p>\n<p><b>Essential businesses: <\/b><span style=\"font-weight: 400\">In the US, the definition of \u201cessential businesses\u201d rests with individual states. The Arizona governor at first allowed hair and nail salons to open, but later <\/span><a href=\"https:\/\/www.kold.com\/2020\/04\/03\/gov-doug-ducey-orders-all-salons-barbershops-close-redefining-essential-businesses\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">reversed that decision<\/span><\/a><span style=\"font-weight: 400\">, leaving some state residents wondering if their leaders knew what they were doing. In late March, the Trump administration advised states that gun stores represent \u201cessential critical infrastructure\u201d and should remain open, a widely criticised move. The state of Washington includes liquor stores and recreational marijuana stores on its list. Whether such stores should be open is a matter of public health debate, but calling them \u201cessential businesses\u201d may be going a bit too far. The World Health Organization in early April <\/span><a href=\"http:\/\/www.euro.who.int\/en\/health-topics\/disease-prevention\/alcohol-use\/news\/news\/2020\/04\/alcohol-does-not-protect-against-covid-19-access-should-be-restricted-during-lockdown\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">advised governments<\/span><\/a><span style=\"font-weight: 400\"> to restrict access to alcohol during the pandemic, noting that that alcohol consumption at this time \u201ccan exacerbate health vulnerability, risk-taking behaviours, mental health issues and violence\u201d (and clarifying, for those in doubt, that drinking will not destroy the virus).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the interest of accuracy, and to shed unnecessary baggage, how about just specifying <\/span><b>businesses allowed to stay open<\/b><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><b>Lockdown<\/b><span style=\"font-weight: 400\">: Yes, we have to stay home, and yes, lots of businesses have to shutter. But do we have to use the language of a penitentiary?\u00a0 The UK announced <\/span><a href=\"http:\/\/www.legislation.gov.uk\/uksi\/2020\/350\/pdfs\/uksi_20200350_en.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">restrictions<\/span><\/a><span style=\"font-weight: 400\"> on leaving home, public gatherings, and other activities in late March. In the US such restrictions are determined by individual states. In both nations, compliance is generally voluntary, and police enforcement is generally proportionate.<\/span><\/p>\n<p><span style=\"font-weight: 400\">But from Kenya to Paraguay, from India to Liberia, police and soldiers have reportedly humiliated, beaten, and even shot people thought to be violating covid-19-related restrictions on movement (for example, <\/span><a href=\"https:\/\/www.theguardian.com\/global-development\/2020\/apr\/01\/extreme-coronavirus-lockdown-controls-raise-fears-for-worlds-poorest\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">here<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/04\/02\/world\/australia\/coronavirus-police-lockdowns.html\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">here<\/span><\/a> and<span style=\"font-weight: 400\">\u00a0<\/span><a href=\"https:\/\/www.reuters.com\/article\/us-health-coronavirus-liberia\/liberia-coronavirus-lockdown-off-to-chaotic-start-idUSKCN21U0QU\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">here<\/span><\/a><span style=\"font-weight: 400\">).\u00a0 These are \u201clockdowns\u201d complete with excessive state violence.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The word lockdown is not a welcoming one. \u201cShelter in place\u201d is far more engaging\u2014after all, everybody craves shelter in a storm\u2014but in the US, sadly, that term connotes what schools and businesses have to do when an active shooter shows up. How about telling people to keep \u201c<\/span><b>safe at home<\/b><span style=\"font-weight: 400\">\u201d rather than announce the imposition of a lockdown?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Language that is unclear\u2014or worse, that conveys inadvertent and counterproductive meanings\u2014undermines public discourse.<sup>6<\/sup> This is as true in public health as anywhere; successful behaviour change efforts, like those crucial to defeating covid-19, depend in part on accurate and compelling language.<sup>7-9<\/sup><\/span><\/p>\n<p><span style=\"font-weight: 400\">At times like these, we need solidarity, generosity, courage, and patience.
