Rose Olson looks to labor organizing efforts for some answers<\/span><\/p>\n<p><!--more--><span style=\"font-weight: 400\"><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2019\/09\/Olson-Rose.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-45622\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2019\/09\/Olson-Rose.jpg\" alt=\"\" width=\"179\" height=\"168\" \/><\/a>Not even the masks on our faces could hide our shared shock. The governor of New York, Andrew Cuomo, had just announced that <\/span><a href=\"https:\/\/www.governor.ny.gov\/news\/no-20210-continuing-temporary-suspension-and-modification-laws-relating-disaster-emergency\"><span style=\"font-weight: 400\">work hour restrictions for resident doctors would be suspended<\/span><\/a><span style=\"font-weight: 400\"> during the coronavirus pandemic. Just up the coast in nearby Boston, my fellow residents and I sat in silence. Would this same story unfold in our hospital?<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the midst of this crisis, more is being asked of resident doctors like myself and, for the most part, we are dutifully obliging. My co-residents and I understand that personal sacrifice is often a necessary part of this job. It\u2019s why we devoted the better part of our 20s to spending Saturday nights in silent libraries and 28 hour shifts in intensive care units. Resident doctors also share a deep sense of professional duty\u2013we meant every word we recited of the Hippocratic Oath. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Yet in the face of mounting demands and dwindling personal reserve, our capacity may reach its limit. <\/span><span style=\"font-weight: 400\">At what point do increased expectations become <\/span><a href=\"https:\/\/www.nytimes.com\/2019\/06\/08\/opinion\/sunday\/hospitals-doctors-nurses-burnout.html\"><span style=\"font-weight: 400\">exploitation<\/span><\/a><span style=\"font-weight: 400\"> of our professional ethic? And in times of crisis, how do we ensure that residents&#8217; rights are being protected?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Residents arguably supply the cheapest and most <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24743629\"><span style=\"font-weight: 400\">flexible physician labor<\/span><\/a> <span style=\"font-weight: 400\">in hospitals. Many residents have been redeployed to staff covid-19 floors where they face<\/span><a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6915e6.htm?s_cid=mm6915e6_x\"><span style=\"font-weight: 400\"> the risk of occupational exposure to SARS-Cov-2<\/span><\/a><span style=\"font-weight: 400\">, the virus that causes the coronavirus disease. Given the <\/span><a href=\"https:\/\/www.vox.com\/policy-and-politics\/2020\/3\/27\/21194402\/coronavirus-masks-n95-respirators-personal-protective-equipment-ppe\"><span style=\"font-weight: 400\">national shortage<\/span><\/a><span style=\"font-weight: 400\"> of personal protective equipment (PPE), many health professionals have had to work <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/03\/19\/health\/coronavirus-masks-shortage.html\"><span style=\"font-weight: 400\">without adequate protection<\/span><\/a><span style=\"font-weight: 400\">. With almost daily news stories of <\/span><a href=\"https:\/\/www.medscape.com\/viewarticle\/927976\"><span style=\"font-weight: 400\">physicians dying from covid-19<\/span><\/a><span style=\"font-weight: 400\">, residents worry for their own safety, and the potential impact on their families and patients.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Even before this pandemic, there was a high prevalence <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4866499\/\"><span style=\"font-weight: 400\">of depression and suicide<\/span><\/a><span style=\"font-weight: 400\"> among residents<\/span><span style=\"font-weight: 400\">. Yet now some <\/span><a href=\"https:\/\/www.ama-assn.org\/residents-students\/resident-student-health\/residents-share-fears-views-training-disruptions-during\"><span style=\"font-weight: 400\">residents have to live with the fear<\/span><\/a> <span style=\"font-weight: 400\">that they are vectors and will transmit the virus unknowingly to others. Many are also facing increased isolation, especially those who have <\/span><a href=\"https:\/\/www.nytimes.com\/2020\/03\/16\/us\/coronavirus-doctors-nurses.html\"><span style=\"font-weight: 400\">moved away from family<\/span><\/a><span style=\"font-weight: 400\"> and loved ones to prevent potential exposure.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Many educational opportunities have also been disrupted or cancelled, such as elective rotations, which are crucial for professional development and readiness for unsupervised practice. It is still unclear how the crisis will affect <\/span><a href=\"https:\/\/www.acgme.org\/Newsroom\/Newsroom-Details\/ArticleID\/10192\/The-American-Board-of-Medical-Specialties-and-Accreditation-Council-for-Graduate-Medical-Education-Joint-Principles-Physician-Training-During-the-Coronavirus-Disease-2019-Pandemic\"><span style=\"font-weight: 400\">national graduation dates and requirements<\/span><\/a><span style=\"font-weight: 400\"> for resident physicians.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Perhaps most concerning is that many of these rapidly changing expectations and safety hazards have not been met with recompense. Residents are the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24743629\"><span style=\"font-weight: 400\">lowest paid doctors<\/span><\/a> <span style=\"font-weight: 400\">and many work up to 80 hours per week, leaving them with less security in the event of an emergency. Additionally, there is an increased opportunity cost of coming to work\u2014for example, the price of commuting when public transportation is reduced or shut down, or the cost of childcare when work hours are extended, and older caretakers can no longer safely be around children. Some resident doctors have advocated for <\/span><a href=\"https:\/\/www.washingtonpost.com\/health\/health-care-workers-deserve-hazard-pay-during-covid-19-pandemic\/2020\/04\/03\/6d552ea2-742f-11ea-a9bd-9f8b593300d0_story.html\"><span style=\"font-weight: 400\">hazard pay<\/span><\/a><span style=\"font-weight: 400\">, although this has been <\/span><a href=\"https:\/\/theintercept.com\/2020\/04\/15\/coronavirus-hospital-medical-residents-hazard-pay\/\"><span style=\"font-weight: 400\">criticized by hospital leadership<\/span><\/a><span style=\"font-weight: 400\"> in some cases, with the professional ethics of residents even being called into question<\/span><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">And what if a resident contracts covid-19 at work and becomes critically ill? Or has a relapse of a psychiatric illness due to the overwhelming stress? What would fair compensation look like, then?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Some solutions may be just down the hospital corridor. Nursing professionals in the US have a long and successful <\/span><a href=\"https:\/\/www.jstor.org\/stable\/29768259?seq=1#metadata_info_tab_contents\"><span style=\"font-weight: 400\">history of <\/span><\/a><span style=\"font-weight: 400\">using organized labor<\/span><span style=\"font-weight: 400\"> to bring about changes in hospitals, often through unions. It is not entirely clear why physicians\u2019 efforts at labor organizing have been <\/span><a href=\"https:\/\/www.newyorker.com\/culture\/annals-of-inquiry\/why-doctors-should-organize\"><span style=\"font-weight: 400\">far more sporadic<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Perhaps physicians feel labor organizing should be reserved for blue collar workers, not more well compensated professions like medicine. Or maybe it\u2019s a shared physician value of service to others that makes advocating for physician rights seem selfish or unprofessional. Perhaps it\u2019s because doctors are by nature risk averse, and fear that these activities would negatively impact their professional reputation. Or it could be that doctors simply do not have the bandwidth to take on another task. Whatever the cause, the paucity of labor organizing may be a lost opportunity to improve physician and patient wellbeing.<\/span><\/p>\n<p><span style=\"font-weight: 400\">History shows us that, alt<\/span><span style=\"font-weight: 400\">hough infrequent, there have been well executed, successful examples of physician labor organizing efforts, largely led by residents and interns. In 1989, the Committee of Interns and Residents (CIR) helped establish one of the most prominent concessions in US physician organizing history by shaping New York limitations on <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/article-abstract\/195244\"><span style=\"font-weight: 400\">resident work hours<\/span><\/a><span style=\"font-weight: 400\"> to 80 hours per week. In 1975, resident doctors in Los Angeles, New York City, and Chicago <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/187575\"><span style=\"font-weight: 400\">organized strikes<\/span><\/a><span style=\"font-weight: 400\"> that ultimately resulted in improved resident wages and working conditions, as well increased attention to patient care concerns. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Resident unions have had other <\/span><a href=\"https:\/\/www.ibtimes.com\/medical-residents-use-apps-activism-fight-hospitals-bigger-salaries-better-benefits-2096453\"><span style=\"font-weight: 400\">notable successes<\/span><\/a><span style=\"font-weight: 400\"> in the negotiation of more equitable resident <\/span><a href=\"https:\/\/www.cirseiu.org\/new-contract-at-kingsbrook\/\"><span style=\"font-weight: 400\">parental leave policies<\/span><\/a><span style=\"font-weight: 400\">, improved <\/span><a href=\"https:\/\/www.cirseiu.org\/zsfg-residents-win-workspace-improvements-in-new-contract-with-the-city-of-san-francisco\/\"><span style=\"font-weight: 400\">pay parity<\/span><\/a><span style=\"font-weight: 400\">, and <\/span><a href=\"https:\/\/www.ibtimes.com\/medical-residents-use-apps-activism-fight-hospitals-bigger-salaries-better-benefits-2096453\"><span style=\"font-weight: 400\">patient advocacy<\/span><\/a><span style=\"font-weight: 400\"> for improved care. While some resident unions have been <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21646972\"><span style=\"font-weight: 400\">positively received<\/span><\/a><span style=\"font-weight: 400\"> by hospital leadership, this has not been true everywhere.<\/span> <span style=\"font-weight: 400\">Some residents active in unionizing efforts have been <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/187575\"><span style=\"font-weight: 400\">reprimanded<\/span><\/a><span style=\"font-weight: 400\"> by both hospital administrators, as well as respected physicians at their hospitals.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Despite the controversial reception that resident organizing efforts have sometimes had in the US, new resident unions continue to form. In 2017, residents from the University of California at San Francisco completed a months\u2019 long campaign and became the largest chapter of residents to join a union in CIR\u2019s history. Perhaps the time is ripe for more groups of resident physicians to follow suit.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Resident doctors love what we do, but it comes at a price. These increased expectations in the covid-19 era may be temporary, but their impact on residents\u2019 lives could be permanent.
