You can read a further list of comments at the end of this piece.<\/em><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400\">I get a call from the daughter of an old friend. She is choking back tears.<\/span><\/p>\n<p><span style=\"font-weight: 400\">&#8220;Mum&#8217;s in hospital, and she has just rung to say she&#8217;s been told she has cancer on the lung and there is nothing to be done. They can&#8217;t do tests because of \u201ccorona.\u201d Dad and I are desperate to visit her but it&#8217;s not allowed.&#8221;\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Three weeks later, the picture is clearer. After a stressful few days in hospital during which &#8220;Mum&#8221; was moved from ward to ward, told different things by different doctors, and at one point challenged to explain &#8220;why on earth&#8221; she had been admitted (a decision by staff in the emergency department, triggered by a radiological alert about an abnormal follow up X ray) this active 58 year old lady is back at home.<\/span><\/p>\n<p><span style=\"font-weight: 400\">We talk. She gives me a blow by blow account of her recent experience and the shock of an unanticipated diagnosis. I ask how she is feeling and what the next steps are? &#8220;Not bad,&#8221; she says. &#8220;I&#8217;ve been told a doctor will be in touch about a possible biopsy which may have to be on the liver, not the lung.&#8221; She is being characteristically stoical. The family are worrying about diagnostic delay and uncertainty around treatment options.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">With services stretched responding to high and predicted demand for care of patients with covid-19 there is increasing concern about the wider health impact of the pandemic. Disruption of &#8220;normal&#8221; services is affecting many, <\/span><a href=\"https:\/\/www.bmj.com\/content\/368\/bmj.m1174\"><span style=\"font-weight: 400\">particularly those with chronic conditions and cancer<\/span><\/a><span style=\"font-weight: 400\">, patients waiting for planned surgical operations including organ transplantation, and others in need of prompt assessment, investigation, and treatment. A recent feature in the Financial Times\u00a0 was entitled <\/span><i><span style=\"font-weight: 400\">&#8220;<\/span><\/i><a href=\"https:\/\/www.ft.com\/content\/d5ac0a79-6647-4f49-bb64-d1cc66362043\"><i><span style=\"font-weight: 400\">Empty beds prompt fears for non-Covid cases<\/span><\/i><\/a><i><span style=\"font-weight: 400\">&#8220;<\/span><\/i><span style=\"font-weight: 400\"> includes reference to the 30% fall in A&amp;E attendance rates in NHS hospitals due in part, it is suggested by <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1401?utm_source=etoc&amp;utm_medium=email&amp;utm_campaign=tbmj&amp;utm_content=weekly&amp;utm_term=20200410\"><span style=\"font-weight: 400\">people who need urgent assessment delaying seeking help<\/span><\/a><span style=\"font-weight: 400\">. Shortage of medicines are reported and not only on intensive care units. Steroid inhalers are one example, hydroxychloroquine another; although the merits of hydroxychloroquine as <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1432\"><span style=\"font-weight: 400\">treatment for covid-19 are unproven<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.clinicaltrialsarena.com\/news\/dcri-hydroxychloroquine-covid-19\/\"><span style=\"font-weight: 400\">trials only recently launched<\/span><\/a><span style=\"font-weight: 400\">, alongside hundreds of other trials exploring new possible treatments.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">As uncertainties and gaps in the evidence to inform the political decisions continue to be aired, debate on <\/span><a href=\"https:\/\/www.reuters.com\/investigates\/special-report\/health-coronavirus-usa-cost\/)%20%20https:\/\/www.weforum.org\/agenda\/2020\/03\/take-five-quarter-life-crisis\/\"><span style=\"font-weight: 400\">&#8220;lockdown&#8221; policies has escalated<\/span><\/a><span style=\"font-weight: 400\">. The sudden loss and falls in people\u2019s income are having a\u00a0<\/span><a href=\"https:\/\/www.cityam.com\/coronavirus-downturn-will-exacerbate-uk-health-inequality\/\"><span style=\"font-weight: 400\">disproportionate impact on the poor<\/span><\/a><span style=\"font-weight: 400\">. The UK Food Foundation reports <\/span><a href=\"https:\/\/foodfoundation.org.uk\/covid-19-latest-impact-on-food-2\/\"><span style=\"font-weight: 400\">rising levels of food insecurity<\/span><\/a><span style=\"font-weight: 400\"> and the <\/span><a href=\"https:\/\/www.economist.com\/international\/2020\/04\/04\/how-will-humans-by-nature-social-animals-fare-when-isolated?fsrc=newsletter&amp;utm_campaign=the-economist-today&amp;utm_medium=newsletter&amp;utm_source=salesforce-marketing-cloud&amp;utm_term=2020-04-03&amp;utm_content=article-link-1\"><span style=\"font-weight: 400\">impact on mental health<\/span><\/a><span style=\"font-weight: 400\">, and <\/span><a href=\"https:\/\/www.reuters.com\/investigates\/special-report\/health-coronavirus-usa-cost\/\"><span style=\"font-weight: 400\">domestic violence<\/span><\/a><span style=\"font-weight: 400\"> is provoking much concern. Looming high for all is the mind boggling scale of the damage to <\/span><a href=\"https:\/\/www.reuters.com\/investigates\/special-report\/health-coronavirus-usa-cost\/\"><span style=\"font-weight: 400\">countries\u2019 economie<\/span><\/a><span style=\"font-weight: 400\">s. Damage, which is closely intertwined with health and social impact, and where the cost of recession will be felt most by the young.<\/span><\/p>\n<p><b>Solidarity among uncertain trade offs\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">As countries struggle with implementing their policies to combat covid-19 it&#8217;s still not entirely clear which are proving most successful, how best to mitigate the adverse impacts,\u00a0 and what &#8220;exit&#8221; strategies to pursue and when. While attempts are being made to learn from different countries approaches, comparisons are hard and trade-offs contested. Some believe that <\/span><a href=\"https:\/\/www.ecnmy.org\/engage\/lockdown-cause-deaths-coronavirus\/\"><span style=\"font-weight: 400\">lockdown may risk killing more people than it saves<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">As citizens in most countries follow the emotionally charged covid 19 media coverage, which may itself be <\/span><a href=\"https:\/\/psycnet.apa.org\/fulltext\/2020-20168-001.html\"><span style=\"font-weight: 400\">having an adverse effect on public health<\/span><\/a><span style=\"font-weight: 400\">, the case for solidarity on shielding, social distancing, lockdown, and increased surveillance has largely been accepted. And few have pushed the solidarity message <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/apr\/05\/queen-urges-britons-stay-strong-coronavirus-covid-lockdown\"><span style=\"font-weight: 400\">better than the Queen<\/span><\/a><span style=\"font-weight: 400\">. The call for &#8220;<\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1336\"><span style=\"font-weight: 400\">international solidarity (not isolationism)<\/span><\/a><span style=\"font-weight: 400\">&#8221; is also being heeded in some quarters.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Meanwhile, action being taken by patient advocates, patient organisations, voluntary, and civil society groups has been swift and equally passionate, if less eye catching. They have provided information and <\/span><a href=\"https:\/\/www.eu-patient.eu\/COVID-19\/covid-resource-point\/\"><span style=\"font-weight: 400\">authoritative guidance for their communities and other forms of\u00a0 support<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">\u00a0<\/span><span style=\"font-weight: 400\">The European Lung Foundation, for example, <\/span><a href=\"https:\/\/europeanlung.org\/en\/QA-covid-19\"><span style=\"font-weight: 400\">is running Q and A sessions<\/span><\/a><span style=\"font-weight: 400\">. UK based charities have also been quick to point to flaws and inconsistencies in the government\u2019s definition of who counts as a &#8220;vulnerable patient&#8221;\u00a0 The umbrella UK organisation National Voices has issued <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/09\/charlotte-augst-we-must-not-let-our-shared-values-fall-victim-to-covid-19\/\"><span style=\"font-weight: 400\">a strong call for continued commitment<\/span><\/a><span style=\"font-weight: 400\"> to human rights and equality. It\u2019s also drawn politicians\u2019 attention to the fact that independent charities, including hospices, have suffered sudden drops in staffing, revenue (and non covid 19 related research budgets resulting in <\/span><a href=\"https:\/\/www.theguardian.com\/science\/2020\/apr\/07\/cancer-research-uk-to-cut-funding-for-research-by-44m\"><span style=\"font-weight: 400\">services being scaled back at a time when demand is high<\/span><\/a><span style=\"font-weight: 400\">. Clarion calls for patients and the public to be involved in decision making on what policies are &#8220;<\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/03\/30\/the-views-of-patients-and-the-public-should-be-included-in-policy-responses-to-covid-19\/\"><span style=\"font-weight: 400\">in the public\u2019s best interest<\/span><\/a><span style=\"font-weight: 400\">&#8221; have been issued. The initial NICE guidelines on admission to critical care, omitted their input, but were subsequently modified as a result of patient and public feedback. As <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/09\/it-takes-a-community-to-establish-core-outcomes-for-research-in-covid-19\/?utm_source=feedburner&amp;utm_medium=feed&amp;utm_campaign=Feed%3A+bmj%2Fblogs+%28Latest+BMJ+blogs%29&amp;g=w_bmj-com\"><span style=\"font-weight: 400\">research related to covid-19 escalates<\/span><\/a><span style=\"font-weight: 400\"> the importance of including their voice in this agenda is underlined too.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Arguably, the voices who most need to be heard are the frail and elderly. Chronological age might not be the best\u00a0 indicator of survival, <\/span><a href=\"https:\/\/www.bmj.com\/content\/368\/bmj.m1198\"><span style=\"font-weight: 400\">but frailty and pre-existing multiple conditions seems to be<\/span><\/a><span style=\"font-weight: 400\">. Hospitals are not where most wish to die. It seems likely that given the option<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">although it seems that at present this option is open to few<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">most would prefer supportive, and where needed palliative care at home or other community settings. The accounts of patients dying alone, in hospital and care homes without family at the bedside are harrowing, and <\/span><a href=\"https:\/\/www.politico.eu\/article\/coronavirus-death-by-video-link-inhumanity\/?utm_source=POLITICO.EU&amp;utm_campaign=456d38e663-EMAIL_CAMPAIGN_2020_04_14_05_05&amp;utm_medium=email&amp;utm_term=0_10959edeb5-456d38e663-189189021\"><span style=\"font-weight: 400\">video links surely not the answer<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Despite the unprecedented challenges, person centred care and shared decision making, outside as well as inside hospitals remains as vital as ever. Trust matters and clarity about low survival rates and the risks as well as <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1387\"><span style=\"font-weight: 400\">benefits of intensive care for covid-19<\/span><\/a><span style=\"font-weight: 400\"> is needed, and the scale of excess deaths it&#8217;s causing over expected deaths, based on previous annual data. The ethical dilemmas <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/07\/daniel-sokol-where-are-the-icu-triage-protocols\/?utm_source=feedburner&amp;utm_medium=feed&amp;utm_campaign=Feed%3A+bmj%2Fblogs+%28Latest+BMJ+blogs%29&amp;g=w_bmj-com\"><span style=\"font-weight: 400\">must be discussed openly too<\/span><\/a><span style=\"font-weight: 400\">. Confronting mortality is not easy, and society has got out of the habit of doing so, but sensitive discussion and advanced care planning for those whose life expectancy is known to be short makes sense. And it also makes sense for many more of us who are currently well, but over 70, or not far off, <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/06\/catherine-white-advance-care-planning-and-why-it-matters-now\/\"><span style=\"font-weight: 400\">to make these plans<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><b>Assessing the impact of the pandemic<\/b><\/p>\n<p><span style=\"font-weight: 400\">Full assessment of the wide health impact of the pandemic won&#8217;t be easy and will only become apparent over time. Hospital deaths dominate current headlines, but primary care doctors are reporting &#8220;fallout&#8221; too and beginning to share information about it. The European Forum for Primary Care has set up weekly webinars to air what it terms\u00a0 &#8220;<\/span><a href=\"http:\/\/euprimarycare.org\/2020\/03\/31\/wednesday-8-april-two-online-focus-group-on-collateral-damage-caused-by-health-care-systems-in-covid-19-mode\/\"><span style=\"font-weight: 400\">collateral damage due to covid-19<\/span><\/a><span style=\"font-weight: 400\">.&#8221; At one I joined last week, a GP asked, &#8220;what data should be we collecting?&#8221;\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">It&#8217;s a good question. The quantitative data that health systems are collecting will provide a lot of information but they won&#8217;t capture the full impact of the pandemic.\u00a0 And as ever, data will be least well captured from poor and marginalised communities and low income countries, which are set to bear a disproportionate burden of the pandemic. The <\/span><a href=\"https:\/\/www.france24.com\/en\/20200402-troops-gather-150-bodies-of-virus-victims-in-ecuador-port-city\"><span style=\"font-weight: 400\">bodies on the streets of Ecuador<\/span><\/a><span style=\"font-weight: 400\">, may be counted, but what about the impact on families and the lives of other dependents? And on countless others who were already struggling to afford or get access to the care they needed before the pandemic swamped their health systems? <\/span><a href=\"https:\/\/www.vbb.com\/media\/Insights_Articles\/Global_operation_sees_a_rise_in_fake_medical_products_related_to_COVID_19.pdf\"><span style=\"font-weight: 400\">Those who fall prey to opportunists pedalling fake products and cures<\/span><\/a><span style=\"font-weight: 400\">? And what of those who have borne the brunt of punitive lockdown policies?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Qualitative data, narratives, surveys, and online feedback will also provide valuable insights. Vivid accounts from junior medical staff working at the front line of healthcare <\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2020\/apr\/08\/icu-doctor-covid-crisis-hospital?CMP=share_btn_tw\"><span style=\"font-weight: 400\">have already caught attention<\/span><\/a><span style=\"font-weight: 400\">, as have <a href=\"https:\/\/www.ft.com\/content\/00312c48-6e87-11ea-9bca-bf503995cd6f\">reflections from senior doctors<\/a><\/span><i><span style=\"font-weight: 400\">. <\/span><\/i><span style=\"font-weight: 400\">\u00a0Patient organisations are encouraging people to post their experience on their web sites, mainstream media are doing the same, and some dedicated platforms are being set up to collect feedback over time.\u00a0<\/span><\/p>\n<p><i><span style=\"font-weight: 400\">The BMJ<\/span><\/i><span style=\"font-weight: 400\">\u2019s policy to co produce its content with <\/span><a href=\"https:\/\/www.bmj.com\/campaign\/patient-partnership\"><span style=\"font-weight: 400\">patients and the public<\/span><\/a><span style=\"font-weight: 400\"> has already spurred patient advocates to share <\/span><a href=\"http:\/\/blogs.bmj.com\/bmj\/category\/patient-perspectives\/\"><span style=\"font-weight: 400\">illuminating early perspectives of the impact of the pandemic<\/span><\/a><span style=\"font-weight: 400\">. We invite others and comments to this article. We also warmly invite wide participation in a twitter chat we are holding on 16 April 2020 at 5.00 pm (BST) to extend the accumulating feedback into the impact of the covid 19 crisis. Follow the chat using the hashtag #BMJdebate.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Although much is adverse and worrying, not all is negative. Many have noted positive environmental change and the rapid move to use online, phone, and video interaction to replace face to face exchange, although it&#8217;s highlighted the digital divide. Reports of innovation and <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/07\/covid-19-a-pivotal-moment-in-community-care\/\"><span style=\"font-weight: 400\">improved collaborative team working are also heartening<\/span><\/a><span style=\"font-weight: 400\">. As health professionals and others at the frontline of care are drawing on their courage and resourcefulness, patients and the public are too.<\/span><\/p>\n<p><span style=\"font-weight: 400\">But courage and resourcefulness can only go so far.
