{"id":47161,"date":"2020-04-14T11:10:55","date_gmt":"2020-04-14T10:10:55","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=47161"},"modified":"2020-04-21T09:30:35","modified_gmt":"2020-04-21T08:30:35","slug":"elizabeth-rider-keeping-children-emotionally-healthy-covid-19-pandemic","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/04\/14\/elizabeth-rider-keeping-children-emotionally-healthy-covid-19-pandemic\/","title":{"rendered":"Elizabeth A Rider: Keeping children emotionally healthy during the covid-19 pandemic\u00a0"},"content":{"rendered":"<p class=\"standfirst\">We must not lose sight of children and adolescents during and after the covid-19 pandemic, says Elizabeth A Rider<\/p>\n<p><!--more--><span style=\"font-weight: 400\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-47163\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/04\/elizabeth_rider-scaled.jpg\" alt=\"\" width=\"140\" height=\"147\" \/>The rapidly evolving covid-19 pandemic, now with <\/span><a href=\"https:\/\/www.who.int\/emergencies\/diseases\/novel-coronavirus-2019\"><span style=\"font-weight: 400\">confirmed cases in 213 countries<\/span><\/a><span style=\"font-weight: 400\">, has caused worldwide fear, anxiety, and confusion in adults, and major disruptions in almost all aspects of daily life. The primary focus of healthcare leaders is necessarily containment of the epidemic. What about the pandemic\u2019s effects on children?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Although covid-19 currently appears to be <\/span><a href=\"https:\/\/pediatrics.aappublications.org\/content\/pediatrics\/early\/2020\/03\/16\/peds.2020-0702.full.pdf\"><span style=\"font-weight: 400\">less frequent and less severe among children and adolescents<\/span><\/a><span style=\"font-weight: 400\">, they remain significantly impacted by concomitant disruptions, including separation, social isolation, illness of caregivers and family members, uncertainty, and increasing stresses on families and living situations. Quarantines, school closings, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/32145186\"><span style=\"font-weight: 400\">home confinement<\/span><\/a><span style=\"font-weight: 400\">, and other containment approaches mandated in a rapidly growing number of countries pose additional challenges.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Children are especially vulnerable to the emotional and psychological impact of covid-19 given their need for security and comfort in the world, and their difficulty conceptualizing the real impact of a health or other disaster. <\/span><a href=\"https:\/\/www.cambridge.org\/core\/journals\/disaster-medicine-and-public-health-preparedness\/article\/posttraumatic-stress-disorder-in-parents-and-youth-after-healthrelated-disasters\/4F3E4300F74CEEAFA8EE95E490944888\"><span style=\"font-weight: 400\">Sprang and Silman studied<\/span><\/a><span style=\"font-weight: 400\"> the reactions of children and their parents to pandemic disasters and found that 30% of isolated or quarantined children and 25% of their parents met criteria for post-traumatic stress disorder (PTSD). PTSD was found to be four times higher in quarantined children compared to those who were not quarantined. Quarantined individuals often experience post-traumatic stress, detachment, anger, and insomnia, <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(20)30460-8\/fulltext\"><span style=\"font-weight: 400\">even years later<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Healthcare systems and clinicians worldwide are stretched thin looking after people who require acute care due to covid-19 in numbers that are unprecedented for non-wartime events. Children and parents are sheltering at home, with the primary burden of childcare and teaching now resting on at-home parents and other caregivers. Many parents do not have training in understanding child development, nor knowledge of age appropriate responses and interventions when their children articulate or exhibit behavioral, and sometimes physical, reactions to the covid-19 pandemic.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Increasing the capacity of parents, families, teachers, and other frontline child caregivers to recognize and respond to children\u2019s and adolescents\u2019 emotional and behavioral reactions to the covid-19 pandemic may help to prevent some of the negative sequelae of childhood emotional trauma from the pandemic. Consequently, understanding the impact of the covid-19 pandemic on children and adolescents, and <\/span><a href=\"https:\/\/accesspediatrics.mhmedical.com\/content.aspx?bookid=2126&amp;sectionid=181401137\"><span style=\"font-weight: 400\">its relationship to their developmental stage<\/span><\/a><span style=\"font-weight: 400\">, is vital.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Children and adolescents are acutely aware of their parents\u2019 and caregivers\u2019 stress and anxiety. Those also experiencing the life threatening illness or loss of family members or friends will be especially impacted. Practical interventions include giving parents and children permission to talk about their feelings and reactions. Avoidance makes the pandemic scarier in children\u2019s minds\u2013\u2013seemingly so dangerous that it cannot be discussed. Healthcare workers can help parents to understand how children may communicate through their reactions, behaviors, and symptoms, and <\/span><a href=\"https:\/\/accesspediatrics.mhmedical.com\/content.aspx?bookid=2126&amp;sectionid=181401137\"><span style=\"font-weight: 400\">guide parents to label and validate feelings and offer constructive ways to manage emotions<\/span><\/a><span style=\"font-weight: 400\">. Common reactions and supportive interventions by developmental stage include:\u00a0<\/span><\/p>\n<p><b>Infants and Toddlers <\/b><span style=\"font-weight: 400\">(0 \u2013 3 years):\u00a0<\/span><br \/>\n<i><span style=\"font-weight: 400\">Reactions:\u00a0<\/span><\/i><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Infants and toddlers are aware of parents\u2019\/caregivers\u2019 worry.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> They are distressed by parents\u2019 distress.\u00a0<\/span><\/li>\n<\/ul>\n<p><i><span style=\"font-weight: 400\">Interventions:\u00a0<\/span><\/i><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Remain calm, reassuring, patient.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Maintain normal routines.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Shield them from media coverage.\u00a0<\/span><\/li>\n<\/ul>\n<p><b>Preschool Children<\/b><span style=\"font-weight: 400\"> (3 \u2013 5 years):\u00a0<\/span><br \/>\n<i><span style=\"font-weight: 400\">Reactions:\u00a0<\/span><\/i><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Preschool children often view illness as resulting from wrongdoing or as punishment.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Child\u2019s primary concerns are separation from family members and safety.<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Children often work through issues via play and may replay images they have seen.<\/span><\/li>\n<li><span style=\"font-weight: 400\"> They may regress\u2014i.e., whining, clinging, sleep problems, physical symptoms.\u00a0<\/span><\/li>\n<\/ul>\n<p><i><span style=\"font-weight: 400\">Interventions:\u00a0<\/span><\/i><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Provide basic reassurance about child\u2019s and family\u2019s safety and what is being done\u2014e.g., handwashing, staying home, etc.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Answer questions simply.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Label and validate child\u2019s feelings.<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Let child know parents\u2019 and family members\u2019 whereabouts.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Maintain normal routines.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Limit television \/ media exposure. Young children do not understand the frightening images they may see.\u00a0<\/span><\/li>\n<\/ul>\n<p><b>School Aged Children <\/b><span style=\"font-weight: 400\">(6 \u2013 12 years):\u00a0<\/span><br \/>\n<i><span style=\"font-weight: 400\">Reactions:\u00a0<\/span><\/i><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Understand the cause of illness as external to themselves.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> May ask many questions.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Concerned about their safety.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> May be frightened and confused by television images of protective gear, masks, bodies.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> May be more irritable than usual.\u00a0<\/span><\/li>\n<\/ul>\n<p><em>Interventions:<\/em><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Answer children\u2019s questions directly and honestly.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Be open to questions: <\/span><i><span style=\"font-weight: 400\">\u201cTell me more about that.\u201d\u00a0<\/span><\/i><\/li>\n<li><span style=\"font-weight: 400\"> Label and validate child\u2019s feelings: <\/span><i><span style=\"font-weight: 400\">\u201cYes, it is scary, and this is what we are doing to stay healthy.\u201d<\/span><\/i><\/li>\n<li><span style=\"font-weight: 400\"> Ask their opinions.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Keep usual routines where possible.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Encourage participation in family or other efforts to help as appropriate.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Minimize television\/ media viewing.\u00a0<\/span><\/li>\n<\/ul>\n<p><b>Adolescents <\/b><span style=\"font-weight: 400\">(13 \u2013 17 years):\u00a0<\/span><br \/>\n<i><span style=\"font-weight: 400\">Reactions:\u00a0<\/span><\/i><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Adolescents can integrate multiple factors in understanding illness and imagine alternative possibilities.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Able to understand the implications of the events.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> May have legitimate fears about the future\u2014e.g., economic repercussions, long term health issues.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> May express concerns about justice, power, and control, and want to discuss the policy issues involved.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> May have varied responses such as worry, fear, sadness, anger.\u00a0<\/span><\/li>\n<\/ul>\n<p><i><span style=\"font-weight: 400\">Interventions:\u00a0<\/span><\/i><\/p>\n<ul>\n<li><span style=\"font-weight: 400\"> Show respect for the adolescent\u2019s feelings.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Discuss their opinions, feelings, and different ways to cope.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Discuss causes and effects of the covid-19 pandemic.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Talk about what adolescents see on social media and hear from their friends.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> When possible, be present if they watch television; discuss and debrief what is seen.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Share your own coping mechanisms.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Encourage participation in family\/community efforts to help.\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">Children and adolescents with particularly intense reactions, such as frequent, aggressive emotional outbursts, preoccupation with the traumatic event, serious problems at school, or emotional numbness or withdrawal, will need further evaluation and referral.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We must not lose sight of children and adolescents during and after the covid-19 pandemic. Psychological risks are high during health and other disasters and may ultimately negatively influence their adult lives. We can work together with existing frontline home support\u2014parents, caregivers, teachers, and others\u2014to recognize and protect children\u2019s emotional health, as well as physical health, and to minimize potential negative sequelae including PTSD, anxiety, depression, and other issues that would require more substantial care and resources in the future.\u00a0<\/span><\/p>\n<p><em><b>Elizabeth A Rider<\/b><span style=\"font-weight: 400\"> is a pediatrician and former child and family therapist.
