{"id":46889,"date":"2020-03-17T16:58:03","date_gmt":"2020-03-17T15:58:03","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=46889"},"modified":"2020-03-26T12:54:22","modified_gmt":"2020-03-26T11:54:22","slug":"uks-coronavirus-policy-places-too-much-responsibility-in-the-hands-of-the-public","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/03\/17\/uks-coronavirus-policy-places-too-much-responsibility-in-the-hands-of-the-public\/","title":{"rendered":"UK&#8217;s coronavirus policy places too much responsibility in the hands of the public"},"content":{"rendered":"<p><span style=\"font-weight: 400\">This week, the UK has <\/span><a href=\"https:\/\/www.gov.uk\/government\/speeches\/pm-statement-on-coronavirus-16-march-2020\"><span style=\"font-weight: 400\">stepped up its efforts<\/span><\/a><span style=\"font-weight: 400\"> to mitigate the impact of the coronavirus pandemic by recommending that the public avoid all \u201cnon-essential\u201d contact and \u201cunnecessary travel.&#8221; In addition to advising household isolation for suspected cases, the government has asked people to work from home where possible and suggested that mass gatherings will not be supported by emergency services. These measures follow Boris Johnson\u2019s admission <\/span><a href=\"https:\/\/www.gov.uk\/government\/speeches\/pm-statement-on-coronavirus-12-march-2020\"><span style=\"font-weight: 400\">last Thursday<\/span><\/a><span style=\"font-weight: 400\"> (13<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> March) that coronavirus presents \u201cthe worst public health crisis for a generation\u201d and \u201cmany more families are going to lose loved ones before their time.\u201d The measures will be welcome to many who have criticised the lack of intervention to this point. However, these advisory measures still fall short of some of the more extensive \u201csocial distancing measures\u201d being implemented in many countries in Europe and around the world. Indeed, at time of writing, a total of 22 European countries had enforced either partial or full <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2020\/mar\/12\/how-do-coronavirus-containment-measures-vary-across-europe\"><span style=\"font-weight: 400\">school closures<\/span><\/a><span style=\"font-weight: 400\">, <\/span><span style=\"font-weight: 400\">and many have implemented <\/span><a href=\"https:\/\/www.wsj.com\/articles\/european-nations-impose-stricter-novel-coronavirus-measures-11584312410\"><span style=\"font-weight: 400\">mandatory closures and lockdowns<\/span><\/a><span style=\"font-weight: 400\">.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">As to the question of whether the UK should have followed suit earlier and implemented such, in Johnson\u2019s words, \u201cDraconian\u201d measures, only time will tell. The lack of scientific certainty that accompanies novel infectious diseases and emergent pandemics necessarily means that there is no \u201cright\u201d course of action (hindsight notwithstanding). Pandemic policy is at best, the result of an educated, albeit highly-educated, guess of scientific advisors.\u00a0<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">In my research on <\/span><a href=\"https:\/\/www.tandfonline.com\/doi\/abs\/10.1080\/09581590701877241\"><span style=\"font-weight: 400\">pandemic influenza policy<\/span><\/a><span style=\"font-weight: 400\">, I found that an important role of scientific advisers was to communicate the scientific uncertainty surrounding the virus to policymakers, who would in turn communicate it to the public in \u201cplausible\u201d ways.<\/span> <span style=\"font-weight: 400\">Although pandemic viruses are, by nature, unpredictable, it meant that pandemic policy decisions were as much based on politics as they were on science. What has concerned me most about the UK\u2019s response to the coronavirus pandemic so far is how eager the government have been to emphasise that their policy is based on \u201cthe science.\u201d\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">One problem with <\/span><a href=\"https:\/\/www.gov.uk\/government\/speeches\/pm-statement-on-coronavirus-12-march-2020\"><span style=\"font-weight: 400\">Johnson\u2019s rhetoric<\/span><\/a><span style=\"font-weight: 400\"> is that it seems to use the terms \u201cscience\u201d and \u201cscientific advice\u201d interchangeably.\u00a0 For instance, in his March 13<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> speech, he argued at one point that \u201cat all stages, we have been guided by the science, and we will do the right thing at the right time\u201d.\u00a0 Yet, in the next sentence he states that \u201cthe scientific advice is that this [closing schools] could do more harm than good at this time.\u201d Science, under normal circumstances, prides itself on its ability to provide objective answers to well defined problems, informed by well established, and often time consuming techniques of scientific research and peer-review.\u00a0 Scientific advice, as the social scientist <\/span><a href=\"https:\/\/www.hup.harvard.edu\/catalog.php?isbn=9780674300620\"><span style=\"font-weight: 400\">Sheila Jasanoff<\/span><\/a><span style=\"font-weight: 400\"> has argued, \u201cfrequently fails to produce consensus over the use of science.\u201d Scientific advisers have the unenviable task of translating and negotiating the technical knowledge of scientists and the political and economic needs of policymakers.\u00a0 And science is always much slower than policymaking, particularly when in a pandemic, as the need for a rapid response is greater than usual.<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">That banning public events will have \u201clittle effect\u201d on the spread of coronavirus, and that closing schools \u201ccould do more harm than good\u201d<\/span> <span style=\"font-weight: 400\">are bold statements that belie the inherent uncertainty of the available evidence. Despite their virological differences, pandemic flu is perhaps the best comparison we have for coronavirus, certainly from a policy perspective. Research on social distancing is far from conclusive, but does tend to contradict the government\u2019s claim, so long as it is implemented in certain ways.\u00a0 For example, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2762764\/\"><span style=\"font-weight: 400\">one review<\/span><\/a><span style=\"font-weight: 400\"> of literature on past influenza and SARS outbreaks found that when measures such as closing schools and banning public gatherings were implemented systematically, for long durations and, crucially, early in the outbreak, epidemic peaks and overall mortality were substantially reduced.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The government\u2019s rationale for holding fire on more extensive and mandatory social distancing policies seems to be their belief that people will not be able to sustain compliance over the long-haul. The problem is that, as yet, they have not published the evidence upon which such assertions are made. I assume it partly derives from social science research on public beliefs and behaviours associated with past flu pandemics, where <\/span><a href=\"https:\/\/academic.oup.com\/cid\/article\/52\/suppl_1\/S138\/501065\"><span style=\"font-weight: 400\">some studies<\/span><\/a><span style=\"font-weight: 400\"> have found that public compliance with preventative measures is far from guaranteed. There are however important caveats, including that <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21199571\"><span style=\"font-weight: 400\">intention to comply<\/span><\/a><span style=\"font-weight: 400\"> is linked to the public\u2019s perception of the severity of the illness and of the reliability of government information.<\/span> <span style=\"font-weight: 400\">At present, it is hard to predict how severe the coronavirus pandemic will be and therefore to what extent the public will be willing to comply with preventative measures and for how long. <a href=\"https:\/\/www.ipsos.com\/ipsos-mori\/en-uk\/three-quarters-britons-support-quarantining-cities-and-towns-prevent-spread-coronavirus\" target=\"_blank\" rel=\"noopener noreferrer\" data-saferedirecturl=\"https:\/\/www.google.com\/url?q=https:\/\/www.ipsos.com\/ipsos-mori\/en-uk\/three-quarters-britons-support-quarantining-cities-and-towns-prevent-spread-coronavirus&amp;source=gmail&amp;ust=1584541388456000&amp;usg=AFQjCNE6Z8wrSDHTkyltVxpPJRLM-wi0Kg\">\u00a0A recent poll<\/a>\u00a0suggests that three-quarters of the UK public would be in favour of quarantining cities.\u00a0 What is clearer is that by not adequately communicating the scientific evidence base for policy, trust in the reliability of the government&#8217;s policy is likely to wane.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Ultimately the issue is one of responsibility. Handwashing, avoiding handshaking, sneezing or coughing into our hands, along with voluntary self- and household- isolation, are all critical components of a pandemic response.
