{"id":46770,"date":"2020-03-04T17:58:29","date_gmt":"2020-03-04T16:58:29","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=46770"},"modified":"2020-03-06T17:31:02","modified_gmt":"2020-03-06T16:31:02","slug":"david-oliver-troubled-care-home-market-should-concern-us-all","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/03\/04\/david-oliver-troubled-care-home-market-should-concern-us-all\/","title":{"rendered":"David Oliver:\u00a0A troubled care home market should concern us all"},"content":{"rendered":"<p class=\"standfirst\">The success or failure of our care home market has serious implications for all health and social care services, warns David Oliver<\/p>\n<p><!--more--><span style=\"font-weight: 400\"><b><span style=\"font-weight: 400\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-46610\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/02\/david_oliver2019.jpg\" alt=\"\" width=\"179\" height=\"168\" \/><span style=\"font-weight: 400\">In December, the healthcare consultancy LaingBuisson published its <\/span><a href=\"https:\/\/www.laingbuisson.com\/laingbuisson-release\/care-homes-for-older-people-a-tale-of-two-markets\/\"><span style=\"font-weight: 400\">30<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> annual report on the state of the care home market<\/span><\/a><span style=\"font-weight: 400\">. Most NHS workers won\u2019t have seen it. It won\u2019t make the media outside niche professional outlets. Yet the picture it paints has major implications for the wider healthcare system, future social policy, and wider society.<\/span><\/span><\/b><\/span><\/p>\n<p><span style=\"font-weight: 400\">For international readers, care homes in this context encompasses both residential homes (where registered nurses are rarely or sparsely employed) and nursing homes (which employ nurses alongside care assistants). Care homes are also sometimes referred to by other names such as \u201clong term care facilities\u201d or \u201chostels\u201d in other nations.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Although some care homes specialise in the care of younger adults with physical, mental, or learning disabilities, most residents enter towards the end of the ageing process. By this stage they generally have severe frailty, age related disability, multiple long term conditions, and often dementia. The threshold for entry has become progressively higher, with the aging demographic of our population not matched by an expansion in care home places. Many older people who would have entered long term care in years past are now supported in their own home or have housing designed for their needs.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">So why should we care what market reports show?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Around <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/care-homes-market-study-summary-of-final-report\/care-homes-market-study-summary-of-final-report\"><span style=\"font-weight: 400\">410 000 people are in care homes in the UK<\/span><\/a><span style=\"font-weight: 400\"> in around 11 300 homes, with a total market of around \u00a316bn. This is almost three times the <\/span><a href=\"https:\/\/www.kingsfund.org.uk\/publications\/nhs-hospital-bed-numbers\"><span style=\"font-weight: 400\">number of people in hospital beds<\/span><\/a><span style=\"font-weight: 400\">. Many of us <\/span><a href=\"https:\/\/www.bgs.org.uk\/blog\/are-care-homes-the-hospices-of-the-future\"><span style=\"font-weight: 400\">will die in care homes<\/span><\/a><span style=\"font-weight: 400\">. Admissions <\/span><a href=\"https:\/\/www.bgs.org.uk\/blog\/are-care-homes-the-hospices-of-the-future\"><span style=\"font-weight: 400\">to acute hospitals<\/span><\/a><span style=\"font-weight: 400\"> from them are far higher than they are from among people of the same age living outside of them.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There is a growing policy focus on the equal rights of residents to access primary and community healthcare services, how best to provide those services to residents, and how much is about the skills of the employed staff doing it for themselves, rather than being done in reach of and with support from the local NHS. Witness, for example, recent disputes over <\/span><a href=\"https:\/\/www.hsj.co.uk\/policy-and-regulation\/two-services-face-axe-from-embattled-gp-contract\/7026697.article\"><span style=\"font-weight: 400\">proposed GP contract requirements<\/span><\/a><span style=\"font-weight: 400\"> for enhanced support for care homes in the NHS Ten Year Plan, the\u00a0 very promising experiences from <\/span><a href=\"https:\/\/www.health.org.uk\/press-release\/new-analysis-finds-encouraging-results-in-reducing-emergency-admissions-from-care\"><span style=\"font-weight: 400\">NHS England\u2019s care home vanguard sites<\/span><\/a><span style=\"font-weight: 400\">, or <\/span><a href=\"https:\/\/www.bgs.org.uk\/resources\/effective-healthcare-for-older-people-living-in-care-homes\"><span style=\"font-weight: 400\">resources from the British Geriatrics Society<\/span><\/a><span style=\"font-weight: 400\"> on better healthcare for residents.<\/span><\/p>\n<p><span style=\"font-weight: 400\">With growing pressure on maintaining flow through hospital beds\u2014often resulting from patients who are stable enough to leave but waiting for the funding, choice, and capacity to do so\u2014the care home market also has crucial implications for acute bed capacity. Delayed transfers of care<\/span><a href=\"https:\/\/www.bmj.com\/content\/bmj\/362\/bmj.k4006.full.pdf\"><span style=\"font-weight: 400\"> hit a record high in 2017<\/span><\/a><span style=\"font-weight: 400\"> and among the \u201csuperstranded\u201d patients who stay in hospital for longer than three weeks, those waiting for space to open up in a care home feature heavily.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Unfortunately, this <a href=\"https:\/\/www.laingbuisson.com\/laingbuisson-release\/care-homes-for-older-people-a-tale-of-two-markets\/\">latest report<\/a> does not offer much cause for optimism. Crucially, the overall number of places in nursing, as opposed to residential, homes has flattened and, according to LaingBuisson, fallen by around 5000 since 2015. This fall comes at a time when <\/span><a href=\"https:\/\/academic.oup.com\/ageing\/article\/47\/3\/374\/4815738\"><span style=\"font-weight: 400\">robust population modelling<\/span><\/a><span style=\"font-weight: 400\">\u00a0 predicts that over the next two decades an increasing number of people will be living longer, with the end of their lives marked by high levels of dependency and dementia.<\/span><\/p>\n<p><span style=\"font-weight: 400\">It also comes despite<\/span><a href=\"https:\/\/www.kingsfund.org.uk\/audio-video\/key-facts-figures-adult-social-care\"><span style=\"font-weight: 400\"> social care funding<\/span><\/a><span style=\"font-weight: 400\"> and provision and support for carers being cut since 2010, when the government\u2019s austerity agenda and cuts to local government support grants kicked in.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The financial numbers in the LaingBuisson report are also alarming. Crucially, 51% of care homes&#8217; income now comes from private payers (sometimes called \u201cself-funders\u201d), even though they only make up 45% of residents. In England, anyone with assets or savings worth more than \u00a322 250 must pay for their care (with thresholds set a little higher in the devolved UK nations).\u00a0 The <\/span><a href=\"https:\/\/www.laingbuisson.com\/laingbuisson-release\/care-homes-for-older-people-a-tale-of-two-markets\/\"><span style=\"font-weight: 400\">market research showed<\/span><\/a><span style=\"font-weight: 400\"> that the fees paid by local authorities for people who qualify for state funding are often less than the cost of providing the place. In effect, self-funding residents, who are paying far higher rates (typically \u00a33000 to \u00a34000 per month), are cross-subsidising the care of those funded by the local council.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Unless there is a major expansion in places to meet growing need, LaingBuisson predicts that we may soon be looking at a \u201csellers market,\u201d where care homes can increase fees to access increasingly scarce beds. Having largely outsourced long term care to the private and third sectors, who are reported to run <a href=\"https:\/\/www.gov.uk\/government\/publications\/care-homes-market-study-summary-of-final-report\/care-homes-market-study-summary-of-final-report\">around nine homes in 10<\/a>, some councils are now considering entering the market by running their own care homes again.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">If the care home market collapses or contracts, this will have serious knock-on implications for the rest of our health and social care services. If the fees increase, it will hit other areas of local government spending at a time when councils\u2019 funding has been serially cut and other local services hit in order to preserve social care capacity. Martin Green, chief executive of Care England, has highlighted <\/span><a href=\"https:\/\/thecareruk.com\/400-care-home-operators-collapse-in-five-years-as-cuts-bite\/\"><span style=\"font-weight: 400\">the collapse of hundreds of care home providers<\/span><\/a><span style=\"font-weight: 400\"> over the past five years, but also the threat that care homes risk becoming unviable in poorer areas with fewer self-funding residents. The way that local authorities raise money\u2014from tax on property values and business rates\u2014compounds <\/span><a href=\"https:\/\/www.bmj.com\/content\/361\/bmj.k2745\"><span style=\"font-weight: 400\">this inequality<\/span><\/a><span style=\"font-weight: 400\">. Socioeconomic deprivation drives demand, yet lowers the number of people who can pay for their own care, and reduces councils\u2019 ability to raise money for care.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Prime Minister Boris Johnson has bullishly <\/span><a href=\"https:\/\/www.eveningexpress.co.uk\/news\/uk\/boris-johnson-pledges-plan-for-social-care-overhaul-within-12-months\/\"><span style=\"font-weight: 400\">promised to \u201cprovide a solution to social care\u201d<\/span><\/a><span style=\"font-weight: 400\"> within 12 months, although there is currently no detail to back this statement.
