{"id":46757,"date":"2020-03-03T15:45:20","date_gmt":"2020-03-03T14:45:20","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=46757"},"modified":"2020-03-22T22:58:32","modified_gmt":"2020-03-22T21:58:32","slug":"behavioural-strategies-for-reducing-covid-19-transmission-in-the-general-population","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/03\/03\/behavioural-strategies-for-reducing-covid-19-transmission-in-the-general-population\/","title":{"rendered":"Behavioural strategies for reducing covid-19 transmission in the general population"},"content":{"rendered":"<p class=\"standfirst\">Human behaviour will determine how quickly covid-19 spreads, say Susan Michie and colleagues<\/p>\n<p><!--more--><\/p>\n<p><span style=\"font-weight: 400\">The UK government\u2019s action plan on coronavirus issued today (3 March 2020) briefly mentions the role the public can play in supporting this response, including following public health authorities\u2019 advice, for example on hand washing. [<\/span><span style=\"font-weight: 400\">1]<\/span><span style=\"font-weight: 400\"> A consensus is emerging about the behaviours that members of the general population should be enacting to reduce their risk of contracting or spreading covid-19. [2]<\/span><span style=\"font-weight: 400\"> From experience in the 2009 H1N1 epidemic, the proportion of people routinely engaging in these behaviours is likely to be low. [<\/span><span style=\"font-weight: 400\">3] <\/span><span style=\"font-weight: 400\">A larger percentage claim to know about the required behaviours, but as has been well demonstrated, neither knowledge nor intention on their own lead to action. [<\/span><span style=\"font-weight: 400\">4]<\/span><span style=\"font-weight: 400\">\u00a0Here we propose ways in which this \u201cknowledge-action gap\u201d can be reduced.<\/span><\/p>\n<p><span style=\"font-weight: 400\">For any behaviour to occur, people need all three of: 1) capability, 2) opportunity and 3) motivation. [<\/span><span style=\"font-weight: 400\">5,6]<\/span><span style=\"font-weight: 400\">\u00a0Capability involves psychological (e.g. the knowledge and skill to perform an action) as well as physical (strength and stamina) capability; opportunity involves both social (e.g. norms) and physical (e.g. resources) facilitators; and motivation involves both \u201creflective\u201d (e.g. conscious decision-making) and \u201cautomatic\u201d (e.g. emotion and habit) processes. These behavioural influences interact as shown below:<\/span><\/p>\n<p><strong>The COM-B Model (Capability-Opportunity-Motivation-Behaviour)<\/strong><\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/COM-B_model.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-46758\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/COM-B_model.jpg\" alt=\"\" width=\"335\" height=\"183\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/COM-B_model.jpg 335w, https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/COM-B_model-300x164.jpg 300w\" sizes=\"auto, (max-width: 335px) 100vw, 335px\" \/><\/a><\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400\">Given differences in behaviours and their contexts, the factors maintaining them differ as do the ways of changing them. A novel infectious disease outbreak presents a unique set of circumstances and challenges, and each recommended protective behaviour will vary according to changes in capability and\/or opportunity and\/or motivation that are required for them to be widely enacted. Understanding these influences is key to developing effective strategies to enable change.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The starting point for a successful behaviour change strategy is to state as clearly and specifically as possible what changes are required. We have started to do this for the current covid-19 outbreak. [<\/span><span style=\"font-weight: 400\">2]<\/span><span style=\"font-weight: 400\">\u00a0 However, it is also important to focus on a small number of key behaviours rather than long lists that people will struggle to remember and build into their everyday lives.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The COM-B model can help guide strategies to ensure that the capability, opportunity, and motivation are all in place when required. This involves the following broad strategies:<\/span><\/p>\n<p><b>Capability:<\/b><span style=\"font-weight: 400\"> Learn how to enact the behaviours and practise them so that you perform them effectively.\u00a0<\/span><\/p>\n<p><b>Opportunity: <\/b><span style=\"font-weight: 400\">Plan ahead so that you have everything you need when the time comes to enact the behaviour, giving yourself the opportunity to enact it.<\/span><\/p>\n<p><b>Motivation:<\/b><span style=\"font-weight: 400\"> Make the required behaviours more attractive or less aversive. Build them into habits and routines so that they occur without having to think about them. Minimise social awkwardness by giving explanations as to what you are doing. Take advantage of social contacts to get feedback on the behaviours to help build habits.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In Table 1, left-hand column, classifies into three groups the currently recommended behaviours with regards to the covid-19 outbreak for the general population to adopt in their everyday behavioural practices: maintaining hygiene, avoiding touching and social distancing (including isolation if advised).\u00a0 Within each category are three behaviours, making nine key behaviours in total. In the-right-hand column we list a set of enabling behaviours that we have developed using the COM-B model to help ensure that the key behaviours are enacted effectively when required.\u00a0<\/span><\/p>\n<p><b>Table 1. Categories of recommended and enabling behaviours to reduce COVID-19 transmission<\/b><\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/behaviour_table.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-46759\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/behaviour_table.png\" alt=\"\" width=\"788\" height=\"946\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/behaviour_table.png 788w, https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/behaviour_table-250x300.png 250w, https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/behaviour_table-768x922.png 768w, https:\/\/blogs.bmj.com\/bmj\/files\/2020\/03\/behaviour_table-640x768.png 640w\" sizes=\"auto, (max-width: 788px) 100vw, 788px\" \/><\/a><\/p>\n<p><span style=\"font-weight: 400\">To meet the challenges of the new behavioural and social changes that will help the population reduce the spread and impact of the COVID-19 outbreak, it is important ensure that people have the capability, opportunity and motivation to enact key behaviours. We are still learning how best to do this in relation to COVID-19, but some key principles are:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><b>practice<\/b><span style=\"font-weight: 400\"> developing the skills required,\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">establish routines and <\/span><b>habits<\/b><span style=\"font-weight: 400\">,\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">make the changes as <\/span><b>easy <\/b><span style=\"font-weight: 400\">as possible,\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">make<\/span><b> plans <\/b><span style=\"font-weight: 400\">including how to overcome possible barriers to the behaviours,\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">ask for <\/span><b>support<\/b><span style=\"font-weight: 400\"> and <\/span><b>feedback <\/b><span style=\"font-weight: 400\">from family, friends and colleagues,\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">if you are doing less of things you enjoy, try to find <\/span><b>enjoyable alternatives<\/b><span style=\"font-weight: 400\">.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">And finally it is important that people feel positive about, and reward themselves for, their successes in making the necessary changes rather than dwelling on times when they slip up.
