{"id":46341,"date":"2019-12-30T16:13:42","date_gmt":"2019-12-30T15:13:42","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=46341"},"modified":"2020-01-14T12:24:26","modified_gmt":"2020-01-14T11:24:26","slug":"innovation-support-societal-responsibility-health","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2019\/12\/30\/innovation-support-societal-responsibility-health\/","title":{"rendered":"Innovation should support societal responsibility for health"},"content":{"rendered":"<p class=\"standfirst\"><span style=\"font-weight: 400\">The popular emphasis on technology being harnessed by individuals fails to consider how it can be used to improve social determinants of health, say Richard Milne, Edo Richard, Eric B Larson, and Carol Brayne<\/span><\/p>\n<p><!--more--><span style=\"font-weight: 400\">In <a href=\"https:\/\/www.gov.uk\/government\/speeches\/how-we-make-public-health-fit-for-the-future\">a speech to Public Health England<\/a> earlier this year, health secretary Matt Hancock set out his vision for the future of public health\u2014a vision based around \u201cproactive, predictive, personalised prevention,\u201d the \u201cpredictive power of genomics, and the data-crunching power of AI.\u201d<\/span><span style=\"font-weight: 400\"> Hancock\u2019s vision echoes the \u201cfutures thinking\u201d of the <a href=\"https:\/\/www.gov.uk\/government\/publications\/chief-medical-officer-annual-report-2018-better-health-within-reach\">chief medical officer\u2019s (CMO) 2018 report<\/a>, which scoped out the potential landscape of healthcare in 2040.<\/span><span style=\"font-weight: 400\">\u00a0Together, they represent the vision of technological innovation for public health that is being developed in UK health policy.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We think that there are tensions within this vision, which should push us to broaden our understanding of how technology can help us to deal with health challenges\u2014not only as responsible individuals, but as groups, communities, and societies.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The CMO\u2019s report sets out how the future offers the potential for better and more equitable health. The first half of the report focuses on social determinants of health and the role of social, political, and community action in securing the future health of the population. The second half discusses the potential of technology to bring about individualised health change. The futures imagined in these two halves of the report contrast in a way that usefully casts light on the problems of Hancock\u2019s vision of personalised health and the wider impoverishment of our imagination when it comes to conceiving the future of public health. This is captured in microcosm within the CMO report in a scenario of how health may be experienced in 2040.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The scenario presents the case of a couple, Sara and Michael, with their baby Allie. In line with Hancock\u2019s emphasis on the implementation of genomics and AI from before birth, the family opt into a national genome sequencing project, and take part in various \u201cprecision medicine\u201d activities. Sara orders a home gut biome test, which shows that Michael has a predisposition for type 2 diabetes, and he changes his lifestyle to counter this risk. The couple consent to donate their daughter\u2019s genetic data to a research project, and in return they are provided with access to the latest research findings. Throughout the scenario, the NHS provides wearables, chatbots, home genome testing kits, and health coaching\u2014all of which produce data that are collected in a single, user controlled platform.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Hancock\u2019s vision of a public health \u201cfit for the future\u201d makes a series of assumptions about that future, which are captured in this scenario. Both forecasts fail to acknowledge the complexity of biological systems and, in our view, downplay the risks of oversimplification that come with new ways of measuring biological systems, such as microbiome sequencing. Furthermore, his vision demarcates the anticipated role of detection and prediction; how and where we see responsibility for action in future healthcare; and the future relationship between citizens, health services, and the private sector.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The data generated by and about Michael, Sara, and Allie in this scenario maps out both their current and predicted health, and are used to inform lifestyle changes and discussions with doctors. However, <a href=\"https:\/\/www.bmj.com\/content\/366\/bmj.l5270\">as commentators have pointed out<\/a>, the growing volume of data generated through the continuous monitoring of the body will inevitably detect many abnormalities without necessarily having the ability to discern which will have clinical consequences.<\/span><span style=\"font-weight: 400\"> Such <a href=\"https:\/\/www.bmj.com\/content\/349\/bmj.g4485\">technologies thus come with a high likelihood<\/a> of overdiagnosis and overtreatment.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Michael is clearly responsible for acting on the basis of his gut biome result; as an empowered individual, he is expected to \u201coptimise\u201d his health and reduce his innate biological risks. Yet to date, the <a href=\"https:\/\/www.bmj.com\/content\/352\/bmj.i1102\">accumulated evidence suggests<\/a> that knowledge of genetic profiles alone is insufficient to change people\u2019s behaviour.<\/span><\/p>\n<p><span style=\"font-weight: 400\">For Hancock, emphasising people\u2019s role as \u201cactive participants in their own health\u201d reflects the wishes of a public who have been found to \u201cbelieve the responsibility for their health lies with them\u2014the individual, not the state.\u201d If we want to improve public health, however, there are compelling reasons to move beyond \u201cthe wisdom of the crowd\u201d and this increasing emphasis on individual responsibility. The\u00a0<\/span><a href=\"https:\/\/www.bloomsbury.com\/uk\/the-health-gap-9781408857991\/\"><span style=\"font-size: 1rem\">evidence we have on the importance of social determinants<\/span><\/a><span style=\"font-size: 1rem\">\u2014inc<\/span><span style=\"font-weight: 400\">luding poverty, education, and geography\u2014in shaping health is overwhelming.<\/span><span style=\"font-weight: 400\"> Yet in the push for personalised prevention, these social determinants are being sidelined.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The popular emphasis on technology being harnessed by individuals is unlikely to affect social determinants of health, and risks increasing the divide between the haves and have nots. Yet technological and social change over the next few decades could also open up new possibilities for public health that shift attention away from the individual. We\u2019d like to see more focus on how a future health service can use technology to affect and improve the social factors which support or impede Michael, Sara, and Allie in \u201coptimising\u201d their health. Examples include supporting innovative \u201ccitizen science\u201d efforts to <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/29608871\">monitor local air and noise pollution or to improve mobility<\/a>.<\/span><span style=\"font-weight: 400\"> Other ideas might involve using technologies to support access to education across the lifecourse or building social networks for the chronically ill or socially excluded.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Genomic medicine and the potential of AI driven healthcare have captured the attention and imagination of clinicians and politicians. Yet we need to consider how we shape this vision of the future, what financial and scientific commitments it drives, and <a href=\"https:\/\/www.bloomsbury.com\/uk\/the-health-gap-9781408857991\/\">how these limit our ability to change direction<\/a>.<\/span><span style=\"font-weight: 400\">\u00a0<\/span><span style=\"font-weight: 400\">The current visions being proffered for health technology neglect innovation\u2019s potential to impact on social determinants of health; redistribute responsibility; and <a href=\"https:\/\/acmedsci.ac.uk\/file-download\/41399-5807581429f81.pdf\">affect social, economic, and cultural inequalities<\/a>.<\/span><span style=\"font-weight: 400\"> As we think about the future of public health, we should remember that there is an alternative.\u00a0<\/span><\/p>\n<p><em><strong><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-46343\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2019\/12\/richard_milne.jpg\" alt=\"\" width=\"118\" height=\"141\" \/>Richard Milne<\/strong> is a sociologist based at the Wellcome Genome Campus and University of Cambridge.
