Less visibly, many are also in need of mental health support. Globally, t<\/span><span style=\"font-weight: 400\">here is an extremely <\/span><a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/mental-health-in-emergencies\"><span style=\"font-weight: 400\">high burden of mental health disorders among conflict affected populations.<\/span><\/a> <span style=\"font-weight: 400\">A recent systematic review shows <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(19)30934-1\/fulltext#seccestitle120\"><span style=\"font-weight: 400\">one in five<\/span><\/a> people have a <span style=\"font-weight: 400\">mental illness.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In Syria, t<\/span><span style=\"font-weight: 400\">he population has been exposed to conflict and terrorism for almost a decade, and many have experienced multiple traumatic events. <\/span><span style=\"font-weight: 400\">The protracted conflict has led to recurrent displacement, depleted socio-economic resources, destruction of infrastructure, and major gaps in the provision of health services. This is having a re-traumatising effect on those in northeast Syria and prevents psychological healing.<\/span><\/p>\n<p><b>Recurrent displacement diminishes resilience and disrupts healing after trauma\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">The Syrian conflict has resulted in the <\/span><a href=\"https:\/\/www.cambridge.org\/core\/services\/aop-cambridge-core\/content\/view\/80C6F6E59EF24566CB98B3C19E29BB75\/S2045796016000044a.pdf\/mental_health_and_psychosocial_wellbeing_of_syrians_affected_by_armed_conflict.pdf\"><span style=\"font-weight: 400\">largest refugee displacement crisis of our time. <\/span><\/a><span style=\"font-weight: 400\">The latest escalation<\/span><span style=\"font-weight: 400\"> in violence in northeast Syria <\/span><span style=\"font-weight: 400\">forced a<\/span><span style=\"font-weight: 400\">n estimated <\/span><a href=\"https:\/\/www.unocha.org\/story\/situation-update-north-east-syria-0\"><span style=\"font-weight: 400\">180,000<\/span><\/a><span style=\"font-weight: 400\"> people to <\/span><span style=\"font-weight: 400\">flee their homes. Before M\u00e9decins Sans Fronti\u00e8res\/Doctors Without Borders (MSF) were <\/span><a href=\"https:\/\/www.msf.org.uk\/article\/northeast-syria-msf-forced-evacuate-staff-due-extreme-volatility-region\"><span style=\"font-weight: 400\">forced to evacuate our international staff<\/span><\/a><span style=\"font-weight: 400\"> from the country, our team was working with people who had fled their homes and were living in overcrowded settlements in the town of Tal Tamer. MSF teams witnessed desperate<\/span><span style=\"font-weight: 400\">, confused, fearful families, many of whom had lost relatives and had left everything behind\u2014not for the first time<\/span><span style=\"font-weight: 400\">.<\/span><span style=\"font-weight: 400\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The p<\/span><span style=\"font-weight: 400\">sychological distress experienced by those in northeast Syria has been caused by severe trauma and exacerbated by recurrent mass displacement. <\/span><span style=\"font-weight: 400\">Our MSF colleagues were also present in Ein Issa camp in northeast Syria, which has been home to displaced people since 2017. Between 2016 and 2017, more <\/span><span style=\"font-weight: 400\">than <\/span><a href=\"https:\/\/conflictandhealth.biomedcentral.com\/track\/pdf\/10.1186\/s13031-019-0216-y\"><span style=\"font-weight: 400\">507,000<\/span><\/a><span style=\"font-weight: 400\"> people fled<\/span><span style=\"font-weight: 400\"> besieged Deir Ez-Zor and Raqqa governorates, resulting in an influx of internally displaced people to Ein Issa camp. <\/span><a href=\"https:\/\/conflictandhealth.biomedcentral.com\/track\/pdf\/10.1186\/s13031-019-0216-y\"><span style=\"font-weight: 400\">A study<\/span><span style=\"font-weight: 400\"> in the camp<\/span><\/a><span style=\"font-weight: 400\"> revealed high levels of exposure to violence and psychological distress. Atrocities, such as public floggings, executions, and public displays of dead bodies had been witnessed by 19% of those surveyed. Among men, 16% had been detained or kidnapped and 11% had been tortured, beaten, or attacked. Over a<\/span><span style=\"font-weight: 400\"> third<\/span><span style=\"font-weight: 400\"> of respondents were so distressed, disturbed, or upset that they had become almost or entirely inactive. A t<\/span><span style=\"font-weight: 400\">hird of people<\/span><span style=\"font-weight: 400\"> showed obvious depressive symptoms and 14% felt so hopeless that they did not want to carry on living.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In such settings, psychological recovery is hindered as people often lack the social and financial capital to improve their situations, arriving in fragile host communities that have little capacity to support newcomers. For many, this leaves them with limited access to food, water, sanitation, housing, adequate health services and education.<\/span> <span style=\"font-weight: 400\">Certain groups, such as older people, those with special needs, unaccompanied children and single mother headed households, are particularly at risk of mental distress.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Another vulnerable group are patients living with chronic conditions. <\/span><a href=\"https:\/\/www.msf.org\/syria-treating-patients-chronic-conditions-war-context\"><span style=\"font-weight: 400\">Thalassemia<\/span><\/a><span style=\"font-weight: 400\"> is prevalent in Syria and requires long-term medical care. <\/span><a href=\"https:\/\/www.msf.org\/syria-treating-patients-chronic-conditions-war-context\"><span style=\"font-weight: 400\">MSF<\/span><\/a><span style=\"font-weight: 400\"> was treating nearly 300 thalassemia patients\u2014mostly children\u2014at the Tal Abyad national hospital until the<\/span><span style=\"font-weight: 400\"> recent escalation in violence forced <\/span><a href=\"https:\/\/www.msf.org\/northeast-syria-msf-forced-evacuate-staff-due-extreme-volatility-region\"><span style=\"font-weight: 400\">MSF to suspend activities<\/span><\/a><span style=\"font-weight: 400\">. For these patients, no longer being able to access treatment puts their lives in danger, causes physical complications, and <\/span><span style=\"font-weight: 400\">severely affects their psychological wellbeing. <\/span><span style=\"font-weight: 400\">Those <\/span><span style=\"font-weight: 400\">with chronic medical conditions, like thalassemia, are at <\/span><a href=\"https:\/\/psycnet.apa.org\/record\/1999-10032-005\"><span style=\"font-weight: 400\">increased risk of depression<\/span><\/a><span style=\"font-weight: 400\">,<\/span><span style=\"font-weight: 400\"> as a<\/span> <a href=\"https:\/\/psycnet.apa.org\/record\/1999-10032-005\"><span style=\"font-weight: 400\">loss of control<\/span><\/a><span style=\"font-weight: 400\"> becomes a source of psychological stress for patients and their families, increasing the risk of psychological harm.<\/span><\/p>\n<p><b>Mental health problems from recurrent trauma and displacement require a multifaceted response\u00a0\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">W<\/span><span style=\"font-weight: 400\">hile<\/span><span style=\"font-weight: 400\"> the conflict in northeast Syria continues with no resolution in sight, many live in a state of \u201cfight or flight\u201d, expecting future adversity while the mechanisms required to process their trauma remain absent.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Treatment for mental health problems from recurrent trauma and displacement require a multifaceted response. <\/span><a href=\"https:\/\/www.cambridge.org\/core\/services\/aop-cambridge-core\/content\/view\/80C6F6E59EF24566CB98B3C19E29BB75\/S2045796016000044a.pdf\/mental_health_and_psychosocial_wellbeing_of_syrians_affected_by_armed_conflict.pdf\"><span style=\"font-weight: 400\">Clinical mental health interventions need to go hand in hand with efforts to mitigate difficult living conditions and to strengthen community based protection mechanisms.<\/span><\/a><span style=\"font-weight: 400\"> A focus on addressing their exposure to violence is not enough. <\/span><span style=\"font-weight: 400\">The t<\/span><span style=\"font-weight: 400\">reatment of psychological problems must be prioritised alongside other needs, such as nutrition, treatment of chronic illnesses and personal safety.<\/span><\/p>\n<p><a href=\"https:\/\/books.google.rs\/books?id=zUxfAgAAQBAJ&amp;pg=PA33&amp;lpg=PA33&amp;dq=Contrary+to+Maslow%E2%80%99s+hierarchy+of+needs,+suggesting+that+treatment+for+psychological+problems+cannot+be+addressed+as+long+as+the+basic+needs+of+nutrition+and+safety+are+pressing,+our+investigations+shows+that+survivors+see+their+mental+health+as+having+highest+priority+and+that+mental+health+is+prerequisite+for+self-efficacy+and+meeting+one%E2%80%99s+basic+needs&amp;source=bl&amp;ots=4qBEEc1lWU&amp;sig=ACfU3U23bFp75rABElvWwv4s8ZnJogSe9Q&amp;hl=en&amp;sa=X&amp;ved=2ahUKEwjam7vE-8XlAhWdwsQBHUHfDIUQ6AEwAHoECAkQAQ#v=onepage&amp;q=Contrary%20to%20Maslow%E2%80%99s%20hierarchy%20of%20needs%2C%20suggesting%20that%20treatment%20for%20psychological%20problems%20cannot%20be%20addressed%20as%20long%20as%20the%20basic%20needs%20of%20nutrition%20and%20safety%20are%20pressing%2C%\"><span style=\"font-weight: 400\">Research shows that survivors of trauma view their mental health as a high priority and a prerequisite for self-efficacy<\/span><\/a><span style=\"font-weight: 400\">;<\/span><span style=\"font-weight: 400\"> however, the latest military operations in northeast Syria have caused mass displacement and retraumatised an already vulnerable population. They have also forced MSF and others to suspend crucial medical services, including the provision of mental healthcare, to people in need.
