From ruminations on the <\/span><a href=\"https:\/\/whyy.org\/segments\/why-do-so-few-doctors-vote\/\"><span style=\"font-weight: 400\">poor physician turnout<\/span><\/a><span style=\"font-weight: 400\"> in elections to <\/span><a href=\"https:\/\/www.statnews.com\/2016\/10\/19\/politics-doctor-patient-relationship\/\"><span style=\"font-weight: 400\">fears about clinicians\u2019 ideologies alienating patients<\/span><\/a><span style=\"font-weight: 400\">, this question has spurred <a href=\"https:\/\/journalofethics.ama-assn.org\/article\/physician-involvement-politics-obligation-or-avocation-commentary-1\/2011-11\">disagreement<\/a> for a long time. Most recently, an <\/span><a href=\"https:\/\/www.wsj.com\/articles\/take-two-aspirin-and-call-me-by-my-pronouns-11568325291\"><span style=\"font-weight: 400\">editorial in the <\/span><i><span style=\"font-weight: 400\">Wall Street Journal<\/span><\/i><\/a><span style=\"font-weight: 400\"> argued that the inclusion of subjects like gun control and climate change in the medical curriculum is tangential and hinders physician training. <\/span><\/span><\/p>\n<p><span style=\"font-weight: 400\"><span style=\"font-weight: 400\">As a medical student, I strongly believe that being educated in social justice will immeasurably improve the quality of care we provide in centring the needs of the communities and people we will serve.\u00a0<\/span><\/span><\/p>\n<p><span style=\"font-weight: 400\">During my undergraduate studies, I spent several years collecting evidence to inform an opiate prescribing guideline. The experience gave me the opportunity to shadow an anaesthesiologist who worked with patients with chronic pain, many of whom had come to him on high dose opiates and were also struggling with dependency or addiction. The escalating opioid epidemic kept the pain clinic busy, with patients commuting several hours each way simply because this was the only location where they could receive the specialised care they needed.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">During each appointment, the anaesthesiologist made a point of having the patients narrate the life experiences that had led them to where they were. Through listening to these stories, I began to understand how their social context, such as occupation and socioeconomic status, shaped their exposure to risk, exacerbated their conditions, and limited their access to care.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">It led me to realise that it is not only impossible but irresponsible to separate medicine from politics. The political landscape informs the care that we are able to provide to our patients and the resources they have access to. Research has repeatedly shown that the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3863696\/\"><span style=\"font-weight: 400\">social determinants of health have a substantial impact on health outcomes<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Some of the most pressing issues that America faces, such as climate change, gun control, and access to health insurance, are often framed in terms of their relationship to the political left and right. Yet the fact is that all of these issues are objectively tied to the health of our patients. Climate change is the greatest<\/span><a href=\"https:\/\/www.healthaffairs.org\/do\/10.1377\/hblog20181218.278288\/full\/\"><span style=\"font-weight: 400\"> threat to human health<\/span><\/a><span style=\"font-weight: 400\"> and survival<\/span><span style=\"font-weight: 400\">. As healthcare providers, we have an obligation to counsel our patients when environmental changes will impact their health and to explain how to mitigate those effects. Gun violence brings <\/span><a href=\"https:\/\/www.apha.org\/-\/media\/files\/pdf\/factsheets\/160317_gunviolence_factsheet.ashx?la=en&amp;hash=3FD94E7FA4353EAB9798815A483BEC73EB69C236\"><span style=\"font-weight: 400\">tens of thousands of people<\/span><\/a><span style=\"font-weight: 400\"> to the emergency room every year and <\/span><a href=\"https:\/\/www.amnesty.org\/en\/documents\/AMR51\/0566\/2019\/en\/\"><span style=\"font-weight: 400\">affects the health needs of survivors<\/span><\/a><span style=\"font-weight: 400\"> for the rest of their lives. Troubles with health insurance restrict the options that patients have within the healthcare system or exclude them from it entirely\u2014even when they desperately need care\u2014which is associated with poorer<\/span><a href=\"https:\/\/annals.org\/aim\/fullarticle\/2635326\/relationship-health-insurance-mortality-lack-insurance-deadly\"><span style=\"font-weight: 400\"> health outcomes<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Physicians have a rich history of social activism directed toward improving public health. Prominent historical examples include advocacy for tobacco control and <\/span><a href=\"https:\/\/www.democratandchronicle.com\/story\/news\/2015\/03\/31\/john-states-dr-seatbelt-dies-university-rochester\/70718726\/\"><span style=\"font-weight: 400\">seatbelt usage<\/span><\/a><span style=\"font-weight: 400\">; more modern examples include <\/span><a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2018\/06\/25\/623126968\/pediatrician-who-exposed-flint-water-crisis-shares-her-story-of-resistance\"><span style=\"font-weight: 400\">Dr Mona Hanna-Attisha calling attention to the water crisis in Flint, Michigan<\/span><\/a><span style=\"font-weight: 400\"> and breakthroughs in patients\u2019 access to <\/span><a href=\"https:\/\/www.ama-assn.org\/advocacy\/leadership-viewpoints\/landmark-deal-medication-assisted-treatment-model-nation\"><span style=\"font-weight: 400\">medication assisted treatment<\/span><\/a><span style=\"font-weight: 400\">. These initiatives have improved the health of millions of Americans in a way that transcends the reach of each physician\u2019s individual practice. Leveraging <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp1407373\"><span style=\"font-weight: 400\">our social capital<\/span><\/a><span style=\"font-weight: 400\"> to advocate for vulnerable communities and evidence based changes to health and social policies may very well be the most effective way that we can practice preventive care.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The <\/span><i><span style=\"font-weight: 400\">Wall Street Journal<\/span><\/i><span style=\"font-weight: 400\"> Editorial Board followed the original article with <a href=\"https:\/\/www.wsj.com\/articles\/corrupting-medical-education-11568578153\">an editorial<\/a> that claimed that, \u201c<\/span><span style=\"font-weight: 400\">patients want an accurate diagnosis, not a lecture on social justice or climate change.\u201d My time spent shadowing leads me to doubt that clinicians spend their time giving unwanted lectures on activism to patients. Equally, I\u2019ve yet to see evidence that educating students on the intersection between social justice and healthcare interferes with clinical reasoning and giving a patient the correct diagnosis. If anything, equipping future doctors with this kind of knowledge is valuable because it helps physicians to better understand the issues faced by their patient populations and, by extension, how best to counsel and support the people they care for.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">It is also important to note that a substantial portion of physicians <\/span><a href=\"https:\/\/www.aamc.org\/download\/493046\/data\/october2018anupdatedlookattheeconomicdiversityofu.s.medicalstud.pdf\"><span style=\"font-weight: 400\">come from privileged backgrounds<\/span><\/a><span style=\"font-weight: 400\">. This does not prevent a physician from delivering empathic care, but it does mean that it\u2019s important our medical education fills in any gaps that a student has not been made privy to by virtue of their lived experiences. Understanding the complex, multifactorial causes of the illnesses and barriers to treatment that our patients face grounds us in our limitations. This is paramount for people who have power over the lives of others, as we do.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">A thoughtful medical curriculum understands that medicine is rooted in science but profoundly influenced by socioeconomic, cultural, and political factors. Over time, it has become standard practice to ask patients about aspects of their lives that were not previously viewed as relevant to health, such as smoking and domestic violence. Medical schools that choose to include social justice and advocacy in their curricula are not diluting the curriculum; they are adapting to the evolving needs of the patients our generation of physicians will be serving.
