{"id":45804,"date":"2019-10-01T11:31:24","date_gmt":"2019-10-01T10:31:24","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=45804"},"modified":"2019-10-14T15:24:46","modified_gmt":"2019-10-14T14:24:46","slug":"life-expectancy-in-england-whats-going-on","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2019\/10\/01\/life-expectancy-in-england-whats-going-on\/","title":{"rendered":"Life expectancy in England: what\u2019s going on?\u00a0\u00a0"},"content":{"rendered":"<p><span style=\"font-weight: 400\">The latest <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/lifeexpectancies\/bulletins\/nationallifetablesunitedkingdom\/2016to2018\"><span style=\"font-weight: 400\">life expectancy data from the Office for National S<\/span><span style=\"font-weight: 400\">tatistics<\/span><\/a> doesn\u2019t provide much to celebrate. The UK\u2019s life expectancy continues to stall and its poor standing in international life expectancy league tables calls for urgent action, especially as mortality is the tip of a much bigger disease iceberg. <a href=\"https:\/\/www.bmj.com\/content\/362\/bmj.k4050\"><span style=\"font-weight: 400\">Action needs to be informed by an accurate assessment of mortality changes<\/span><\/a><span style=\"font-weight: 400\"> and how the root causes\u00a0vary in their impact on different population groups and areas, so that government policy and services can respond with measures that are evidence-led.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Recent trends in life expectancy have attracted much debate about both the direction and causes of these changes. Reports even differ about whether life expectancy is stalling, falling, or improving but <\/span><span style=\"font-weight: 400\">more slowly than before, depending on how official data are interpreted.<\/span><span style=\"font-weight: 400\"> Here we use <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/lifeexpectancies\/bulletins\/nationallifetablesunitedkingdom\/2016to2018\"><span style=\"font-weight: 400\">the latest ONS data<\/span><\/a><span style=\"font-weight: 400\"> to clarify how life expectancy is changing.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Interpretations of mortality trends vary depending on the measure used, and this has contributed to inconsistencies in reporting. For example, the number of deaths is an easily understood figure, but it predominantly measures the population\u2019s size and age structure. Even if death rates are unchanged or fall, the number of deaths can rise <\/span><span style=\"font-weight: 400\">when the population grows or ages &#8211; the UK population is doing both currently. For example: <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/deaths\/bulletins\/deathsregistrationsummarytables\/2018\"><span style=\"font-weight: 400\">57,000 more deaths were registered in England and Wales in 2018 compared with 2011 (a 12 percent rise), but the age-standardised death rate fell by 13.2 per 100,000 (a one percent decrease).<\/span><\/a><span style=\"font-weight: 400\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Measures such as age-standardised death rates and period life expectancy at birth are more reliable for interpreting trends because they adjust for these demographic changes. Both are summary measures of the same underlying data, providing alternative, but mutually consistent and widely used ways of comparing mortality reliably over time and between different populations. Life expectancy can be measured on a period (point in time) or cohort (for a specific group) basis.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Different measures can present different views of mortality. For example, there have been <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2019\/03\/08\/lucinda-hiam-and-martin-mckee-the-deepening-health-crisis-in-the-uk-requires-society-wide-political-intervention\/\"><span style=\"font-weight: 400\">recent reports<\/span><\/a><span style=\"font-weight: 400\"> that <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2019\/03\/19\/danny-dorling\/\"><span style=\"font-weight: 400\">life expectancy is falling<\/span><\/a><span style=\"font-weight: 400\">. These were references to work by the Institute and Faculty of Actuaries, who recently revised down their <\/span><span style=\"font-weight: 400\">Continuous Mortality Investigations (CMI) model <\/span><a href=\"https:\/\/www.actuaries.org.uk\/system\/files\/field\/document\/CMI%20WP119%20v01%202019-03-07%20-%20CMI%20Mortality%20Projections%20Model%20CMI_2018%20Briefing%20Note.pdf\"><span style=\"font-weight: 400\">projections of long-term improvement in cohort life expectancy at age 65<\/span><\/a><span style=\"font-weight: 400\"> (a quite different measure to current, period life expectancy at birth)<\/span><span style=\"font-weight: 400\">. However, even on this basis, the Institute did not say life expectancy overall is falling, nor is it projecting a fall in life expectancy for any group \u2013 it is instead projecting a slower rate of improvement among people aged 65+ than previously assumed: \u201ca<\/span><i><span style=\"font-weight: 400\">lmost all users of the CMI Model expect that mortality will continue to improve, even if this is at a slower rate than in the first decade of this century<\/span><\/i><span style=\"font-weight: 400\">.\u201d <\/span><a href=\"https:\/\/ourworldindata.org\/whats-happening-to-life-expectancy-in-britain\"><span style=\"font-weight: 400\">Similar reports of falling life expectancy followed when ONS\u2019 2016 projections of life expectancy improvements were revised downward from previous projections<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Even the <\/span><a href=\"https:\/\/www.theguardian.com\/society\/2019\/jun\/23\/why-is-life-expectancy-falling?CMP=Share_iOSApp_Other\"><span style=\"font-weight: 400\">national<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.nytimes.com\/2019\/08\/30\/world\/europe\/uk-life-expectancy.html\"><span style=\"font-weight: 400\">foreign<\/span><\/a><span style=\"font-weight: 400\"> media are now reporting that life expectancy in Britain is falling<\/span><span style=\"font-weight: 400\">. <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/lifeexpectancies\/bulletins\/nationallifetablesunitedkingdom\/2016to2018\"><span style=\"font-weight: 400\">ONS data released this week<\/span><\/a><span style=\"font-weight: 400\"> shows that life expectancy at birth in UK overall (and in England) isn\u2019t falling, the average for 2016-18 being slightly higher than previous years.<\/span><span style=\"font-weight: 400\">**<\/span><span style=\"font-weight: 400\">\u00a0 However, after many decades of steady improvement, the current decade has seen a dramatic reduction in the pace of improvement. <\/span><a href=\"https:\/\/www.ons.gov.uk\/releases\/changingtrendsinmortalityinenglandandwales1990to2017experimentalstatistics\"><span style=\"font-weight: 400\">This slowdown started around 2011<\/span><\/a><span style=\"font-weight: 400\">: <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/lifeexpectancies\/bulletins\/nationallifetablesunitedkingdom\/2016to2018\"><span style=\"font-weight: 400\">between 2011 and 2018 life expectancy in England increased by just 0.5 years in males and 0.2 years in females, compared with 2.2 and 1.7 years respectively in the preceding seven-year period 2004 to 2011<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">And there are some other adverse trends. Although male life expectancy has shown a steady if small rise since 2011, the virtual flat-lining of female life expectancy since then is particularly worrying given that <\/span><a href=\"https:\/\/doi.org\/10.1787\/223159ab-en\"><span style=\"font-weight: 400\">UK&#8217;s\u00a0life expectancy compares\u00a0poorly with similar European countries<\/span><\/a><span style=\"font-weight: 400\">, especially for females. <\/span><span style=\"font-weight: 400\">Moreover, although improvements in life expectancy have slowed across much of Europe, the slowdown has <\/span><span style=\"font-weight: 400\">been greatest in UK &#8211; hence <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/lifeexpectancies\/bulletins\/nationallifetablesunitedkingdom\/2016to2018\"><span style=\"font-weight: 400\">UK\u2019s life expectancy disadvantage relative to European counterparts has widened<\/span><\/a><span style=\"font-weight: 400\">, especially for females.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The bad news doesn\u2019t stop there. <\/span><span style=\"font-weight: 400\">Although the slowdown in mortality improvements is affecting all types of neighbourhoods, the impact is greatest in more deprived areas, causing inequalities to widen. <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/healthinequalities\/bulletins\/healthstatelifeexpectanciesbyindexofmultipledeprivationimd\/2015to2017\"><span style=\"font-weight: 400\">In the poorest ten percent of areas in England, female life expectancy actually fell by 100 days of life between 2012-14 and 2015-17, compared with an increase of 84 days in the least deprived areas<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Commenting on life expectancy trends in England, Public Health England noted \u201c<\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/786515\/Recent_trends_in_mortality_in_England.pdf\"><i><span style=\"font-weight: 400\">the overall slowdown in improvement is due to factors operating across a wide range of age groups, geographies and causes of death<\/span><\/i><\/a><span style=\"font-weight: 400\">\u201d, with decelerating mortality improvements from cardiovascular disease being one of the significant contributors.<\/span> <span style=\"font-weight: 400\">Public Health England\u2019s analysis and other evidence to date suggests several factors are driving <\/span><span style=\"font-weight: 400\">the slowdown, making longer-term trends less predictable.<\/span><span style=\"font-weight: 400\">\u00a0\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In summary, life expectancy at birth among males continues to improve, but much more slowly since 2011; female life expectancy has been fluctuating annually and, overall, has changed little since 2011. But there are no signs yet of a trend of falling life expectancy, or that there will not be a return to improvement in the longer term.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">That said, there is no room for complacency. <\/span><a href=\"https:\/\/www.bmj.com\/content\/365\/bmj.l2278?utm_source=etoc&amp;utm_medium=email&amp;utm_campaign=tbmj&amp;utm_content=weekly&amp;utm_term=20190607\"><span style=\"font-weight: 400\">Urgent action is needed to reinvigorate mortality improvements and reduce inequalities<\/span><\/a><span style=\"font-weight: 400\">. The <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2018\/12\/12\/veena-raleigh-englands-stalling-life-expectancy-pointers-action\/\"><span style=\"font-weight: 400\">analyses to date provide clear pointers for action<\/span><\/a><span style=\"font-weight: 400\">, but further investigations of the underlying drivers are needed.
