{"id":45751,"date":"2019-09-26T02:00:00","date_gmt":"2019-09-26T01:00:00","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=45751"},"modified":"2020-03-19T12:03:32","modified_gmt":"2020-03-19T11:03:32","slug":"julie-silver-medical-journals-must-address-gender-bias","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2019\/09\/26\/julie-silver-medical-journals-must-address-gender-bias\/","title":{"rendered":"Julie Silver: Medical journals must address gender bias"},"content":{"rendered":"<p class=\"standfirst\">When women physicians and scientists face barriers to publication and journal leadership, the pace of discovery slows and critical perspectives are lost<\/p>\n<p><!--more--><span style=\"font-weight: 400\">Reports demonstrate gender bias in academic medicine, and an important subset of this evidence focuses on journal level disparities, for example the numbers of women as authors, reviewers, and editors. Notably, gender is not binary, but studies on publishing related disparities typically compare proportions of men and women, and there is a paucity of data for many people at risk of underrepresentation (e.g. individuals<\/span><span style=\"font-weight: 400\"> identifying as LGBTQ+<\/span><span style=\"font-weight: 400\"> or ethnic or racial minorities). However, an abundance of research shows women in academia, inclusive of medicine, face more barriers to publishing than men.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Barriers are wide ranging and multifactorial. Bias harms scientific discovery, and when it occurs at the journal level <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(19)31042-6\/fulltext\"><span style=\"font-weight: 400\">there are profound financial implications<\/span><\/a><span style=\"font-weight: 400\">. For example, if a researcher faces barriers to publication this affects organisations that financially support her work (i.e. employer and grant funders), and their return on investment is not fully realised.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Journal owners, some of which are medical societies, have an obligation to address both conscious (explicit) and unconscious (implicit) bias. <\/span><span style=\"font-weight: 400\">Firewalls exist between medical society owners and their journals. These are in place to avoid conflicts of interest and protect content selection. Firewalls do not prevent owners from ensuring their journals are fair and that they hold senior editors accountable for proactively uncovering and addressing gender bias in a scientific manner using established metrics.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Perpetuating gender bias is <\/span><a href=\"http:\/\/sheleadshealthcare.com\/wp-content\/uploads\/2018\/10\/Be_Ethical_Campaign_101418.pdf\"><span style=\"font-weight: 400\">unethical<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><span style=\"font-weight: 400\">incompatible with a position of trust. T<\/span><span style=\"font-weight: 400\">he <\/span><span style=\"font-weight: 400\">International Committee of Medical Journal Editors (ICMJE) has <\/span><a href=\"http:\/\/www.icmje.org\/recommendations\/browse\/roles-and-responsibilities\/journal-owners-and-editorial-freedom.html#one\"><span style=\"font-weight: 400\">codified owner obligations<\/span><\/a><span style=\"font-weight: 400\">, which include <\/span><span style=\"font-weight: 400\">ensuring <\/span><span style=\"font-weight: 400\">editors act in a manner that is compatible \u201cwith a position of trust.\u201d <\/span><span style=\"font-weight: 400\">For example, <\/span><span style=\"font-weight: 400\">in the United States, g<\/span><span style=\"font-weight: 400\">ender equity among senior editors and on editorial boards is a <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/187760\"><span style=\"font-weight: 400\">long-standing<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1550857911002282?via%3Dihub\"><span style=\"font-weight: 400\">well-documented disparity<\/span><\/a><span style=\"font-weight: 400\">, yet it is a straightforward and inexpensive issue to fix. A lack of equitable inclusion (i.e., a lower proportion of women editors as compared to their presence in the specialty or members of the society) is itself a disparity. Although one cannot assume equitable representation of women among editors or on editorial boards positively impacts other journal-level gender disparities; nevertheless, there is no reasonable justification for perpetuating an easily solvable inequity.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Fortunately, there is good news to report. For example, recently the <\/span><a href=\"https:\/\/twitter.com\/BJSM_BMJ\/status\/1122834366890037250\"><i><span style=\"font-weight: 400\">British Journal of Sports Medicine<\/span><\/i><span style=\"font-weight: 400\"> announced on Twitter<\/span><\/a><span style=\"font-weight: 400\"> it will \u201ccommit to at least half of the BJSM Ed board being women\u201d\u2014giving this reason: \u201cBecause the talent is there.\u201d Even in a male-dominated specialty, parity on editorial boards is easy to achieve. <\/span><a href=\"https:\/\/www.journalofhospitalmedicine.com\/jhospmed\/article\/202723\/hospital-medicine\/leading-example-how-medical-journals-can-improve\"><span style=\"font-weight: 400\">Editors at the <\/span><i><span style=\"font-weight: 400\">Journal of Hospital Medicine<\/span><\/i><span style=\"font-weight: 400\"> acknowledged<\/span><\/a><span style=\"font-weight: 400\"> \u201cmedical journals have an obligation to address unequal opportunities\u201d<\/span><span style=\"font-weight: 400\"> and vowed <\/span><span style=\"font-weight: 400\">as an initial step that they will \u201c<\/span><span style=\"font-weight: 400\">assess the current representation of women and racial and ethnic minorities in our journal community, including first and senior authors, invited expert contributors, reviewers, and editorial team members.\u201d\u00a0<\/span><\/p>\n<p><i><span style=\"font-weight: 400\">The Lancet\u2019s <\/span><\/i><span style=\"font-weight: 400\">editors recently published <\/span><a href=\"https:\/\/www.thelancet.com\/lancet-women\"><span style=\"font-weight: 400\">a special issue<\/span><\/a><span style=\"font-weight: 400\"> focused on women in medicine and discussed <\/span><a href=\"https:\/\/www.thelancet.com\/action\/showPdf?pii=S0140-6736%2819%2930289-2\"><span style=\"font-weight: 400\">what they are doing about gender and diversity<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">They noted gaps are not due to a lack of qualified women, but rather the \u201csupposed meritocracy is rigged against them, resulting in an unjustified and unacceptable masking of women\u2019s contributions.\u201d Their data also showed gaps in the representation of women authors and reviewers. They disclosed gaps in the representation of women as commentary authors, consistent with existing research in <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2688347\"><span style=\"font-weight: 400\">pediatrics<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamadermatology\/article-abstract\/2723281\"><span style=\"font-weight: 400\">dermatology<\/span><\/a> <span style=\"font-weight: 400\">which showed that women in these specialties were not equitably represented as authors of opinion and perspective articles. This is a very important finding, because it essentially diminishes the voices of women in medicine, and a lack of diverse insights may negatively impact research and clinical care.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Journals depend on the privilege of publishing the best scientific work, and they need women\u2019s research to be successful. As such, physicians and scientists (regardless of gender) can consider preferentially submitting to journals with a good track record on equity and inclusion. Before hitting \u201csubmit\u201d women and allies might ask themselves, <\/span><i><span style=\"font-weight: 400\">\u201cIs there evidence that this journal is focused on treating women fairly?\u201d\u00a0<\/span><\/i><\/p>\n<p><span style=\"font-weight: 400\">Now is the time for all journals and their owners to address gender bias. When women physicians and scientists face barriers to publication and journal leadership, the pace of discovery slows and critical perspectives are lost. <\/span><a href=\"https:\/\/www.liebertpub.com\/doi\/pdf\/10.1089\/heq.2019.0040\"><span style=\"font-weight: 400\">Patients may also be affected<\/span><\/a><span style=\"font-weight: 400\"> as they are relying on researchers with the brightest minds<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">many of whom are women<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">to advance the science that will help them overcome disease and disability. There is no doubt we need her science, and when science wins, everyone wins.\u00a0<\/span><\/p>\n<p><b><i>Julie K.
