Lesbian women have a higher asthma rate than straight women. We know that women have a longer life expectancy than men, and that heterosexual marriage seems to narrow that gap (married men live longer, married women less)\u2014but what about two women who are married, or two men?<\/span><\/p>\n<p><span style=\"font-weight: 400\">It turns out that there are a lot of things we don\u2019t know, or don\u2019t understand, about the health of LGBT+ people\u2014but that\u2019s not surprising given the dearth of attention that we have received. It\u2019s probably worth pointing out here that HIV\/AIDS is not equal to LGBT health.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Over my years at <\/span><i><span style=\"font-weight: 400\">The BMJ<\/span><\/i><span style=\"font-weight: 400\">, I\u2019ve been invited to an inordinate number of conferences. In the past month alone, I\u2019ve received 27 conference invitations for topics as broad as discussing how to ban the tools used in torture, to primary care in Manchester. But in all that time I\u2019ve not seen a single one focused on the wider aspects of LGBT+ health. At least, that was until the Royal Society of Medicine stepped into the breach with <a href=\"https:\/\/www.bmj.com\/content\/366\/bmj.l4470\">Pride in Medicine<\/a>\u2014the first conference of its kind in the UK, where <\/span><a href=\"https:\/\/aru.ac.uk\/people\/catherine-meads\"><span style=\"font-weight: 400\">Catherine Meads<\/span><\/a><span style=\"font-weight: 400\"> spoke of the unexpected findings I opened this piece with.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Why has so little attention been paid to this group of people? Is it a lack of need? We\u2019re don\u2019t have basic epidemiological information, but what we do know points to the need for care. A recent systematic review and meta-analysis published in <\/span><i><span style=\"font-weight: 400\">JAMA Pediatrics<\/span><\/i><span style=\"font-weight: 400\"> found that <\/span><span style=\"font-weight: 400\">LGBT+ youths<\/span><span style=\"font-weight: 400\"> were <a href=\"https:\/\/jamanetwork.com\/journals\/jamapediatrics\/article-abstract\/2704490\">3.5 times more likely to attempt suicide<\/a> than their straight counterparts. LGBT+ people are almost <a href=\"https:\/\/bmcpsychiatry.biomedcentral.com\/articles\/10.1186\/s12888-016-0767-z\">twice as likely to experience mental health problems<\/a><\/span><span style=\"font-weight: 400\">. Smoking and drinking rates (and presumably the concomitant health problems) are <\/span><a href=\"https:\/\/bmjopen.bmj.com\/content\/7\/10\/e015058\"><span style=\"font-weight: 400\">higher in LGBT+ people<\/span><\/a><span style=\"font-weight: 400\">. Researchers who<\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3818511\/\"><span style=\"font-weight: 400\"> modelled the effect of living surrounded by anti LGBT prejudice <\/span><\/a><span style=\"font-weight: 400\">found that living in a high prejudice areas leads to a lower life expectancy of around 12 years for LGBT+ people. This all points clearly to an urgent and specific need, so why hasn\u2019t it been addressed?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Is it because healthcare in the UK is institutionally homophobic? Certainly, there seem to be many individual staff who are: <\/span><a href=\"https:\/\/www.stonewall.org.uk\/about-us\/media-releases\/stonewall-report-reveals-impact-discrimination-health-lgbt-people\"><span style=\"font-weight: 400\">a recent Stonewall survey<\/span><\/a><span style=\"font-weight: 400\"> found that 23% of respondents had witnessed \u201cnegative remarks about LGBT people from healthcare staff while accessing services.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">One speaker at the conference, Mike Farquhar from the Evelina Children\u2019s hospital, showed the results of some <\/span><a href=\"https:\/\/www.stonewall.org.uk\/system\/files\/unhealthy_attitudes.pdf\"><span style=\"font-weight: 400\">research carried out on doctors\u2019 attitudes<\/span><\/a><span style=\"font-weight: 400\">, including this doctor\u2019s response:\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">\u201cThe needs of others that are not the norm in my opinion should not be forced upon others that choose to be what we have up to now considered mainstream \u2018normal\u2019. As human beings, we are biologically programmed to function in a certain manner, and deviations are not to be considered mainstream society.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400\">Is it any wonder then that <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/721367\/GEO-LGBT-Action-Plan.pdf\"><span style=\"font-weight: 400\">16% of people accessing health services have had a negative experience because of their sexuality, and 38% because of their gender identity<\/span><\/a><span style=\"font-weight: 400\">? Or that <\/span><a href=\"https:\/\/www.stonewall.org.uk\/about-us\/media-releases\/stonewall-report-reveals-impact-discrimination-health-lgbt-people\"><span style=\"font-weight: 400\">14% of LGBT+ people have avoided treatment altogether<\/span><\/a><span style=\"font-weight: 400\">, for fear of being faced with that discrimination?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Another explanation is that LGBT+ people are reticent about disclosing their gender or sexuality to any authority. It\u2019s true that we now have equal ages of consent, and equal marriage in mainland Britain, but homosexuality wasn\u2019t decriminalised in Scotland until 1981, or 1982 in Northern Ireland\u2014which still outlaws same sex marriage.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The final piece of UK legislation allowing discrimination on the basis of sexuality was finally repealed in a bill in 2017. You might think that this was just an anachronism, but it sends a strong signal: don\u2019t be complacent about your place in society.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">(<\/span><a href=\"https:\/\/www.youtube.com\/watch?v=WXayhUzWnl0\"><span style=\"font-weight: 400\">If you\u2019re still unconvinced, I beg you to take 10 minutes to watch this<\/span><\/a><span style=\"font-weight: 400\">).<\/span><\/p>\n<p><span style=\"font-weight: 400\">I know that I\u2019m reluctant to disclose my sexuality to my doctor; I\u2019ve barely mentioned it in my work at <\/span><i><span style=\"font-weight: 400\">The BMJ<\/span><\/i><span style=\"font-weight: 400\">\u2014this will be the first time I\u2019ve put it in such an easily googleable form. That wasn\u2019t a conscious decision, but an unconscious inhibition, one which I and the rest of the LGBT+ community need to be helped to overcome if we are to start collecting good data, and targeting proper interventions.<\/span><\/p>\n<p><span style=\"font-weight: 400\">That help is now arriving. Michael Farquhar has set up the NHS rainbow badges, which have been <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2019\/02\/04\/michael-farquhar-nhs-staff-are-in-a-perfect-position-to-be-advocates-for-lgbt-people-when-they-most-need-it\/\"><span style=\"font-weight: 400\">mentioned elsewhere<\/span><\/a><span style=\"font-weight: 400\"> on <\/span><a href=\"https:\/\/www.bmj.com\/content\/363\/bmj.k4988\"><span style=\"font-weight: 400\">our pages<\/span><\/a><span style=\"font-weight: 400\">. They\u2019re not there to liven up a dull uniform, but to signal to a reluctant patient that it\u2019s OK to talk about their sexuality or gender identity. NHS trusts around the country sign up to a set of tenets that set out standards of conduct for badge wearers. I\u2019m a cis white man, who\u2019s in a pretty powerful position in the healthcare system, but I\u2019d still choose the consultant who has one.<\/span><\/p>\n<p><span style=\"font-weight: 400\">There\u2019s also the fantastic work of the LGBT Foundation\u2019s Pride in Practice programme, which is working in greater Manchester and London, but also branching out into Liverpool, Leeds, Essex, and West Yorkshire. Pride in Practice work with community services (GP practices, pharmacies, optometrists, and dentists) to make sure that those services are prepared for working with LGBT+people. They too have a visible signal of allyship, a poster which tells patients that they\u2019re safe here.<\/span><\/p>\n<p><span style=\"font-weight: 400\">There are 226 acute NHS trusts in England and 6993 GP practices.
