{"id":44188,"date":"2019-03-08T12:18:06","date_gmt":"2019-03-08T11:18:06","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=44188"},"modified":"2019-03-15T12:14:44","modified_gmt":"2019-03-15T11:14:44","slug":"clare-bostock-bring-your-daughter-to-cervical-screening","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2019\/03\/08\/clare-bostock-bring-your-daughter-to-cervical-screening\/","title":{"rendered":"Clare Bostock: Bring your daughter to cervical screening"},"content":{"rendered":"<p class=\"standfirst\">We can all help to demystify this test by talking more openly about it, says Clare Bostock<\/p>\n<p><!--more--><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-44191\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2019\/03\/Clare_Bostock.jpg\" alt=\"\" width=\"149\" height=\"105\" \/><span style=\"font-weight: 400\">At a time when cervical screening in England is at its <\/span><a href=\"https:\/\/digital.nhs.uk\/data-and-information\/publications\/statistical\/cervical-screening-programme\/england---2017-18\"><span style=\"font-weight: 400\">lowest rate<\/span><\/a><span style=\"font-weight: 400\"> for two decades, with one in four eligible women not attending the test, something needs to be done. Public Health England (PHE) stepped forward this week with a <\/span><a href=\"https:\/\/youtu.be\/JkutlKFqqec\"><span style=\"font-weight: 400\">newly launched advertising campaign<\/span><\/a><span style=\"font-weight: 400\">, but breaking down the barriers and taboos that prevent women from attending screening will be a multifaceted job. <\/span><\/p>\n<p><a href=\"https:\/\/www.cancerresearchuk.org\/health-professional\/cancer-statistics\/statistics-by-cancer-type\/cervical-cancer#heading-Two\"><span style=\"font-weight: 400\">In the UK<\/span><\/a><span style=\"font-weight: 400\">, there are more than 3000 new cases and around 850 deaths from cervical cancer each year. It is estimated that if women attended screening regularly, 83% of cervical cancer cases could be <\/span><a href=\"https:\/\/www.gov.uk\/government\/news\/phe-launches-cervical-screening-saves-lives-campaign\"><span style=\"font-weight: 400\">prevented<\/span><\/a><span style=\"font-weight: 400\">. So why do I feel embarrassed writing about my experience of attending for cervical screening last week? <\/span><\/p>\n<p><span style=\"font-weight: 400\">Even the prime minister Theresa May spoke recently in the House of Commons about <\/span><a href=\"https:\/\/www.theguardian.com\/politics\/video\/2019\/jan\/23\/those-few-minutes-can-save-lives-theresa-may-encourages-women-to-take-smear-test-video\"><span style=\"font-weight: 400\">her experiences of the smear test<\/span><\/a><span style=\"font-weight: 400\">. And here the problem starts with that horrible little word, <\/span><a href=\"https:\/\/en.oxforddictionaries.com\/definition\/smear\"><i><span style=\"font-weight: 400\">smear<\/span><\/i><\/a><span style=\"font-weight: 400\">: \u201cto coat or mark something messily or carelessly with a greasy or sticky substance.\u201d I certainly didn&#8217;t want anyone doing anything messily or carelessly with my cervix. From the <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/health-47445785\"><span style=\"font-weight: 400\">media coverage<\/span><\/a><span style=\"font-weight: 400\">, it appears that PHE made a conscious decision to avoid the word \u201csmear\u201d in their campaign. <\/span><\/p>\n<p><span style=\"font-weight: 400\">If, as a healthcare professional, I feel nervous about sharing my experiences with you, then it is no wonder that many women cite fear and embarrassment as reasons for not attending cervical screening. The charity <\/span><a href=\"https:\/\/www.jostrust.org.uk\/node\/1076499\"><span style=\"font-weight: 400\">Jo&#8217;s Cervical Cancer Trust<\/span><\/a><span style=\"font-weight: 400\"> found that young women who delay or don\u2019t go for cervical screening are scared (71%) or embarrassed (81%). \u00a0Even those who attend regular tests still feel body conscious (67%), scared (43%), vulnerable (46%), and not in control (36%) before their test. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">But last week I did something new at my smear test: I brought my 8 year old daughter with me. I thought that this would be an excellent way to have a chat about some anatomical terms and the importance of screening\u2014which it was. I think that it&#8217;s better for my children if we are relaxed about using the word vagina, and they hear the word from me before they hear it in the playground. <\/span><\/p>\n<p><span style=\"font-weight: 400\">What I hadn&#8217;t banked on was the fact that the curtain could separate me from the door but not from my daughter. And so she sat in a chair next to me, watching everything (except my vagina, which only the nurse could see). My daughter could see what the nurse was doing and could see my (hopefully) relaxed and pain-free facial expression. I felt that it was an important experience to share, and the only person my daughter took pity on was the nurse for needing to see my bottom\u2014we spoke about that too. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Society often relies on celebrities for health promotion. Actress <\/span><a href=\"https:\/\/www.instagram.com\/p\/BqCYsRIlPYq\/?utm_source=ig_embed\"><span style=\"font-weight: 400\">Michelle Keegan took to Instagram<\/span><\/a><span style=\"font-weight: 400\"> recently to share her experiences of cervical screening. And about half a million <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3385661\/\"><span style=\"font-weight: 400\">extra cervical screening attendances<\/span><\/a><span style=\"font-weight: 400\">\u00a0took place in England between mid-2008 and mid-2009, the period during which British reality TV star Jade Goody was diagnosed with and died from cervical cancer. Yet we can all help to demystify this test by talking more openly about it and not treating the subject as something to be spoken of in hushed tones. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Allowing our daughters to see our smear tests firsthand as a routine, non-painful, dignified event would be a step towards banishing some of the shame and embarrassment that causes young women to shy away from screening. We could even start a public health campaign to bring our daughters to our smear tests.
