These stories highlight how pressing and critical the issue of achieving wellbeing at work for doctors has become, and how important it is to undertake research in this area. Current interventions seem to have limited effect given that the issue of mental ill-health in NHS staff is not improving, but, rather, <\/span><a href=\"https:\/\/www.bmj.com\/content\/360\/bmj.k1074.full\"><span style=\"font-weight: 400\">getting worse. <\/span><\/a><span style=\"font-weight: 400\">Something needs to change. &#8220;Care Under Pressure,&#8221; a research project synthesising <\/span><a href=\"https:\/\/bmjopen.bmj.com\/content\/8\/2\/e021273\"><span style=\"font-weight: 400\">existing interventions and resources to tackle and prevent mental ill health in doctors,<\/span><\/a><span style=\"font-weight: 400\"> aims to help with this need. <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">Most existing interventions are based on the idea of building up individual resilience to cope with stress. This one dimensional approach is not the best way to deal with a complex issue such as mental ill-health in doctors. Moreover, this approach is not only <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0003497517310974\"><span style=\"font-weight: 400\">ineffective<\/span><\/a><span style=\"font-weight: 400\">, but is also potentially harmful. It ends up assigning <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2018\/08\/07\/agnes-arnold-forster-doctors-wellbeing-learning-past-help-improve-future\/\"><span style=\"font-weight: 400\">all the responsibility<\/span><\/a><span style=\"font-weight: 400\"> to (and potentially isolating and harming) the individual doctor, while the issue needs to be understood and addressed also from <\/span><a href=\"https:\/\/www.bmj.com\/content\/351\/bmj.h6719\/rr-5\"><span style=\"font-weight: 400\">organisational, historical, social and cultural perspectives<\/span><\/a><span style=\"font-weight: 400\">. <\/span><\/p>\n<p><span style=\"font-weight: 400\">A common objection to the idea of developing new and more sophisticated interventions is the lack of funds within an already overstretched NHS. However, there <\/span><i><span style=\"font-weight: 400\">already<\/span><\/i><span style=\"font-weight: 400\"> are initiatives and interventions, sometimes at national level or in specific trusts<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">but they are either not well publicised, or they are under-utilised. <\/span><\/p>\n<p><span style=\"font-weight: 400\">When I present the \u201cCare Under Pressure\u201d project to a clinical audience, I am often told about a support scheme for doctors (e.g. one offered by occupational health), or about tools to increase staff wellbeing. On questioning, it becomes clear that these initiatives are virtually unknown outside of the group I am speaking to, yet this group complains about the low level of engagement with, or awareness of, such initiatives. More surprisingly, through our discussions, we discover that even in their own trusts, those who are meant to be in the know are not aware of other initiatives in the same organisation (e.g. schemes to encourage staff to recognise positive outcomes in the workplace, to counterbalance negativity and blame culture). Within this hidden world, there are probably \u201cgood ideas\u201d which could save lives and\/or inform the improvement of effective interventions. This \u201chiddenness\u201d potentially also means unnecessary replication of efforts or even \u201creinventing the wheel\u201d<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\"> which is counterproductive in a system that is already under pressure. <\/span><b><br \/>\n<\/b><b><br \/>\n<\/b><span style=\"font-weight: 400\">Developing interventions alone is not enough for them to be used successfully; how they are designed and conveyed is equally important. There are interventions that currently \u201cdo not work\u201d because they are not properly communicated to doctors who could benefit from them. In some cases they are not communicated <\/span><i><span style=\"font-weight: 400\">at all<\/span><\/i><span style=\"font-weight: 400\">, so it is hardly surprising that they are not accessed. Sometimes these interventions are effectively communicated, but they are not <\/span><i><span style=\"font-weight: 400\">accessible<\/span><\/i> <i><span style=\"font-weight: 400\">to <\/span><\/i><span style=\"font-weight: 400\">doctors, due for example to rigid shifts and lack of time. If available, they may not be <\/span><i><span style=\"font-weight: 400\">accessed<\/span><\/i> <i><span style=\"font-weight: 400\">by <\/span><\/i><span style=\"font-weight: 400\">doctors, due to stigma or fear of career repercussions. \u00a0<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\"><br \/>\n<\/span><span style=\"font-weight: 400\">We need to think about the wider context, because of the <\/span><a href=\"https:\/\/www.dc.nihr.ac.uk\/blog\/care-under-pressure-going-beyond-individual-level-interventions-to-improve-doctors-mental-health\/8656\"><span style=\"font-weight: 400\">complexity of the problem of mental ill-health in doctors<\/span><\/a><span style=\"font-weight: 400\"> and because <\/span><a href=\"http:\/\/bjgp.org\/content\/67\/663\/e709\"><span style=\"font-weight: 400\">doctors are diverse and have different needs<\/span><\/a><span style=\"font-weight: 400\">. <\/span><span style=\"font-weight: 400\">We need to take a systems-wide perspective which incorporates individual, organisational, professional and broader sociocultural and historical dimensions. \u00a0This also implies that a \u201cone-size is not fits all\u201d approach is unlikely to be successful, but rather certain interventions and approaches may work well in certain contexts.
