Sch\u00fcnemann: Getting trustworthy guideline recommendations into practice"},"content":{"rendered":"<p><span style=\"font-weight: 400\">Practising clinicians, healthcare professionals<\/span><span style=\"font-weight: 400\">,<\/span> <span style=\"font-weight: 400\">researchers<\/span><span style=\"font-weight: 400\">, policymakers and<\/span> <span style=\"font-weight: 400\">patients<\/span> <span style=\"font-weight: 400\">rely on guidelines to help inform their practice, so it is vital that practice guidelines are trustworthy and quick. However, users, creators, and funders of guidelines often<\/span><span style=\"font-weight: 400\"> criticize the development of recommendations because they take too long to become public<\/span><span style=\"font-weight: 400\">. By the time a guideline has undergone various approval processes and is published, its recommendations may be outdated, leaving developers frustrated and funders disappointed. Therefore, we need a new approach to speed up the development of guidelines.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Firstly, we should modify traditional ways of publishing guidelines. When recommendations have been completed and approved by a guideline development group, they should be made available in small informative recommendation units that cover a topic without causing confusion or important gaps. Confusion and gaps can arise if single recommendations remain uninformative because they depend on other recommendations, or do not provide key information that is relevant to address a topic.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The European Commission Initiative on Breast Cancer (ECIBC) made its<\/span><a href=\"https:\/\/ecibc.jrc.ec.europa.eu\/recommendations\/\"> <span style=\"font-weight: 400\">breast cancer screening recommendations describing the age ranges and intervals available immediately after approval on a dedicated website with interfaces for different target audiences<\/span><\/a><span style=\"font-weight: 400\">. These were published, even though the guidelines of over 50 other recommendations are still being developed. This approach avoided the long delays that occur when guideline groups wait for approval of an entire guideline document. It allows for rapid feedback by patients, health professionals, and policy makers. The approach also supports maintaining small informative recommendation units in a living or updated format. [1] Academic credit<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">one of the incentives for guideline panel members who give up time to develop guidelines<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">can be provided by publishing derivation products, short peer reviewed articles that describe these recommendations, and acknowledgement on the relevant websites.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Secondly, in order to be efficient, the evidence and rationale for recommendations should not require extensive work or modification by others, such as medical editors, before they are made public. Guideline Development Groups should work in ways that allow for more transparency to explain the rationale for recommendations to users and to understand any potential conflicts of interest. The<\/span><a href=\"https:\/\/www.bmj.com\/content\/353\/bmj.i2089.long\"> <span style=\"font-weight: 400\">GRADE Evidence to Decision (EtD) frameworks<\/span><\/a><span style=\"font-weight: 400\"> provide a structured and transparent approach <\/span><span style=\"font-weight: 400\">to help panels use evidence to inform their decisions and make guidelines clear. <\/span><span style=\"font-weight: 400\">The EtD frameworks lay out the importance of a health problem; the balance between health benefits and harms; values that people attach to outcomes; resource use, equity, acceptability, and feasibility; and the interests of panel members. They create a record of the discussion and judgments made in a guideline development group. <\/span><span style=\"font-weight: 400\">Recommendations created using these<\/span><span style=\"font-weight: 400\">\u00a0frameworks can also be used to<\/span><a href=\"https:\/\/ecibc.jrc.ec.europa.eu\/recommendations\/list\/1\"> <span style=\"font-weight: 400\">produce patient-friendly material<\/span><\/a><span style=\"font-weight: 400\">. Making protocols or recordings of meetings available for users could further increase transparency, although this is currently does not happen.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Thirdly, guideline developers need to make better use of existing work. In order for this to happen, guideline developers need to make the information that determines the direction and strength of a recommendation transparent and available. Sharing online evidence to decision frameworks supports this. This allows guideline development groups to focus on reviewing<\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0895435616304826\"> <span style=\"font-weight: 400\">evidence syntheses and making judgements<\/span><\/a><span style=\"font-weight: 400\"> as opposed to compiling evidence. Guideline developers can<\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0895435616304826\"> <span style=\"font-weight: 400\">adopt, adapt, or develop de novo recommendations<\/span><\/a><span style=\"font-weight: 400\"> for the relevant context. It avoids duplication of effort and wasting resources. This approach has been used for<\/span><a href=\"https:\/\/www.bmj.com\/content\/360\/bmj.j5158\"> <span style=\"font-weight: 400\">Chinese guidelines<\/span><\/a><span style=\"font-weight: 400\">, in the<\/span><a href=\"https:\/\/hqlo.biomedcentral.com\/articles\/10.1186\/s12955-017-0754-1\"> <span style=\"font-weight: 400\">middle East<\/span><\/a><span style=\"font-weight: 400\">,<\/span><a href=\"https:\/\/bmcpublichealth.biomedcentral.com\/articles\/10.1186\/s12889-017-4867-6\"> <span style=\"font-weight: 400\">Australia<\/span><\/a><span style=\"font-weight: 400\">,<\/span><a href=\"http:\/\/www.claht.org\/web\/images\/pdf\/ADOLOPMENT%20project%20%E2%80%93%20PARA%20CLAHT.docx\"> <span style=\"font-weight: 400\">South America<\/span><\/a><span style=\"font-weight: 400\"> and in the<\/span><a href=\"http:\/\/www.euro.who.int\/en\/countries\/estonia\/publications\/developing-and-implementing-guidelines-for-health-policy-and-clinical-practice-in-estonia-interim-appraisal-of-progress-2015\"> <span style=\"font-weight: 400\">Estonian guideline program<\/span><\/a><span style=\"font-weight: 400\">me. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Finally, guideline developers should focus on their accomplishments. Over a decade ago, <\/span><a href=\"https:\/\/journals.plos.org\/plosmedicine\/article?id=10.1371\/journal.pmed.0040119\"><span style=\"font-weight: 400\">colleagues and I published a <\/span><span style=\"font-weight: 400\">framework for developing rapid recommendations<\/span><\/a><span style=\"font-weight: 400\">. Over a period of 3 months, an expert committee at the World Health Organization (WHO)\u00a0<\/span><a href=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(06)70684-3\/fulltext\"><span style=\"font-weight: 400\">developed 23 separate recommendations and a number of research recommendations<\/span><\/a><span style=\"font-weight: 400\"> for the treatment of avian influenza, a serious public health threat at that time. It included a review of mechanistic and animal research evidence. From inception to final agreement on the recommendations, the guideline development group produced, on average, one recommendation every four days. They were published on the WHO website and later in peer reviewed journals as a model for rapid guideline development. Since then, several guideline groups have<\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0895435616303869\"> <span style=\"font-weight: 400\">emulated these approaches<\/span><\/a><span style=\"font-weight: 400\"> and produced<\/span><a href=\"https:\/\/health-policy-systems.biomedcentral.com\/articles\/10.1186\/s12961-018-0330-0\"> <span style=\"font-weight: 400\">related guidance<\/span><\/a><span style=\"font-weight: 400\">. The American Society of Hematology (ASH) just completed over 100 recommendations in six guidelines on venous thromboembolism (VTE) management in less than 36 months by applying efficient methodology and working in well-coordinated, large teams that provide full EtDs for each recommendation. This represents a formidable effort producing a recommendation every 10 days on average. Yet, to allow for traditional guideline publications, the recommendations were not published online in small informative recommendation units or as living recommendations once ready. Thus, although rapid in their development, traditional approaches based on completing an entire guideline rather than informative units causes delays and, in turn, can lead to concerns by panel members and other stakeholders.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Adopting this approach could speed up guideline development without compromising their credibility and enhance stakeholder satisfaction.<\/span><\/p>\n<p><em><span style=\"font-weight: 400\"><strong><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/holger.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-43793\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/holger.jpg\" alt=\"\" width=\"160\" height=\"160\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/holger.jpg 160w, https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/holger-150x150.jpg 150w\" sizes=\"auto, (max-width: 160px) 100vw, 160px\" \/><\/a>Holger J.
