Naming the digits\u2014the thumb"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/jeffrey_aronson.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-32935\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/jeffrey_aronson-223x300.jpg\" alt=\"jeffrey_aronson\" width=\"122\" height=\"146\" \/><\/a><span style=\"font-weight: 400\">In the last two weeks I have discussed various aspects of our digits, including <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2018\/12\/03\/jeffrey-aronson-when-i-use-a-word-digits\/\"><span style=\"font-weight: 400\">finger counting<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2018\/12\/07\/jeffrey-aronson-when-i-use-a-word-fingerprints\"><span style=\"font-weight: 400\">fingerprints<\/span><\/a><span style=\"font-weight: 400\">. This week I shall start looking at the names we give to the different fingers.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The <\/span><em><a href=\"http:\/\/www.unifr.ch\/ifaa\/\"><span style=\"font-weight: 400\">Terminologia Anatomica<\/span><\/a><\/em><span style=\"font-weight: 400\">, which in 1998 replaced the previous standard, the <\/span><em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Nomina_Anatomica\"><span style=\"font-weight: 400\">Nomina Anatomica<\/span><\/a><\/em><span style=\"font-weight: 400\">, gives the names. They are the thumb, which is not always regarded as a finger, the index finger, the middle finger, the ring finger, and the little finger.<\/span><\/p>\n<p><span style=\"font-weight: 400\">I start with the thumb. It comes from the IndoEuropean root TEU\u018f , meaning to swell or a lump, reminding us that the thumb, which a thimble will cover, swells out from the hand, as the thigh swells out from the hip. And the root itself swells out in many different directions.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Add a <\/span><i><span style=\"font-weight: 400\">b<\/span><\/i><span style=\"font-weight: 400\"> to the basic root, and you get a swell Latin word, tuber, an underground structure in which a stem or rhizome becomes thickened, like a potato. <\/span><a href=\"https:\/\/www.omim.org\/entry\/191100?search=bourneville&amp;highlight=bourneville\"><span style=\"font-weight: 400\">Tuberous sclerosis<\/span><\/a><span style=\"font-weight: 400\"> is a hereditary disease, with autosomal dominant transmission, associated with dysregulation of the mammalian target of rapamycin pathway (mTOR). [Rapamycin is now known as sirolimus.] The disease is characterized by hard swellings in the brain (e.g. subependymal giant cell astrocytomas), causing developmental delay and seizures, in the skin (facial angiofibromas and shagreen patches), and elsewhere (e.g. cardiac rhabdomyomas, renal angiomyolipomas, and retinal hamartomas). There are mutations in the <\/span><i><span style=\"font-weight: 400\">TSC<\/span><\/i><span style=\"font-weight: 400\"> (tuberous sclerosis complex) genes on chromosomes 9q34 and 16p13. Tuberous sclerosis can be treated with immunosuppressant drugs, such as <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/29925060\"><span style=\"font-weight: 400\">sirolimus<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/30455224\"><span style=\"font-weight: 400\">everolimus<\/span><\/a><span style=\"font-weight: 400\">. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/30406604\"><span style=\"font-weight: 400\">Anti-EGFR antibodies<\/span><\/a><span style=\"font-weight: 400\"> have also been used. PFO Rayer published a colour drawing of a patient with facial angiofibromas in 1835 and von Recklinghausen later reported cardiac myomas and cerebral sclerosis in a neonate who died minutes after being born. The name \u201ctuberous sclerosis\u201d was coined in 1880 by D\u00e9sir\u00e9-Magloire Bourneville, a French neurologist (1840\u20131909).<\/span><\/p>\n<p><span style=\"font-weight: 400\">With consonantal shift and then metathesis, a tuber becomes a tufer and then a truffle, which also swells underground. In French un truffe was originally an unlikely tale or a joke, from which we get trifle, which meant the same before it came to mean something trivial and then the trivial dessert of the same name, in a form that was originally also called a fool.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Add an <\/span><i><span style=\"font-weight: 400\">l<\/span><\/i><span style=\"font-weight: 400\"> and you get the Greek words <\/span><i><span style=\"font-weight: 400\">\u03c4\u03cd\u03bb\u03bf\u03c2,<\/span><\/i><span style=\"font-weight: 400\"> a knob or lump, and <\/span><i><span style=\"font-weight: 400\">\u03c4\u03cd\u03bb\u03b7<\/span><\/i><span style=\"font-weight: 400\">, a callus. Tylectomy is the surgical removal of a lump, often inelegantly called lumpectomy. The former was introduced in <em>The <\/em><\/span><em><a href=\"https:\/\/www.bmj.com\/content\/bmj\/2\/5811\/423.full.pdf\"><span style=\"font-weight: 400\">BMJ<\/span><\/a><\/em><span style=\"font-weight: 400\"> in 1972, but the latter, which is of at least the same vintage, has survived better. Tylenol, a proprietary brand of paracetamol (acetaminophen), is unrelated; it comes from the chemical name, N-ace<u>tyl<\/u>-para-aminoph<u>enol<\/u>.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Add an <\/span><i><span style=\"font-weight: 400\">m<\/span><\/i><span style=\"font-weight: 400\"> for the Latin derivative tumere, to swell, giving us tumour, tumescent, and detumescent\u2014down boy!. A tumulus is a pile of earth, a burial mound, or tomb.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Add an <\/span><i><span style=\"font-weight: 400\">r<\/span><\/i><span style=\"font-weight: 400\"> and you get the Greek word \u03c4\u1fe1\u03c1\u03cc\u03c2, cheese, which swells out of the milk from which it is produced by coagulation. And \u03b2\u03bf\u1fe6\u03c2<\/span><i><span style=\"font-weight: 400\">, <\/span><\/i><span style=\"font-weight: 400\">an ox or cow,<\/span><i><span style=\"font-weight: 400\">+ <\/span><\/i><span style=\"font-weight: 400\">\u03c4\u03c5\u03c1\u03cc\u03c2 gives the Greek \u03b2\u03bf\u03cd\u03c4\u03c5\u03c1\u03bf\u03bd and the Latin butyrum, or butter. Tyrosine was originally isolated by Liebig from dried cheese, and tyramine is derived from tyrosine. In turn, tyramine can be directly converted to the neurotransmitters dopamine, noradrenaline, and adrenaline, but the main pathway for their synthesis is from tyrosine via levodopa (Figure 1). <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2135154\/\"><span style=\"font-weight: 400\">Tyrothricin<\/span><\/a><span style=\"font-weight: 400\">, a mixture of the antibiotics tyrocidine and gramicidin, was named in 1940 after the genus of organisms called Tyrothrix, first isolated from cheese in 1878 by <\/span><a href=\"http:\/\/www.nature.com\/articles\/145965b0.pdf\"><span style=\"font-weight: 400\">Emile Duclaux<\/span><\/a><span style=\"font-weight: 400\"> (1840\u20131904).<\/span><\/p>\n<figure id=\"attachment_43721\" aria-describedby=\"caption-attachment-43721\" style=\"width: 451px\" class=\"wp-caption alignnone\"><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/aronson_thumb.png\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-43721 size-full\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/aronson_thumb.png\" alt=\"\" width=\"451\" height=\"650\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/aronson_thumb.png 451w, https:\/\/blogs.bmj.com\/bmj\/files\/2018\/12\/aronson_thumb-208x300.png 208w\" sizes=\"auto, (max-width: 451px) 100vw, 451px\" \/><\/a><figcaption id=\"caption-attachment-43721\" class=\"wp-caption-text\">Figure 1. A tingling in the thumb was once supposed to indicate evil: \u201cBy the pricking of my thumbs, something wicked this way comes\u201d (<\/span><i><span style=\"font-weight: 400\">Macbeth<\/span><\/i><span style=\"font-weight: 400\"> iv:1<\/span><i><span style=\"font-weight: 400\">)<\/span><\/i><span style=\"font-weight: 400\">. You could avert witches by turning the thumb into the palm, and holding thumbs was thought to be lucky. In Latin a thumb was pollex, from pollens, powerful, the power being experienced when one is under someone\u2019s thumb. In Roman amphitheatres the crowd would show the power of their pleasure or displeasure by turning their thumbs. As Juvenal put it \u201cverso pollice vulgus cum iubet occidunt populariter\u201d. But which way the thumb was turned to signify death is <\/span><a href=\"https:\/\/www.goodreads.com\/book\/show\/995213.Nature_Embodied\"><span style=\"font-weight: 400\">not known<\/span><\/a><span style=\"font-weight: 400\">. It may have been up, down, or even inward. Nowadays we give the thumbs down to things we dislike, like tuberous sclerosis, and the thumbs up to things we like, including tubers and truffles, especially of the chocolate variety.<\/span><\/p>\n<p><em><strong>Jeffrey Aronson<\/strong>\u00a0is a clinical pharmacologist, working in the Centre for Evidence Based Medicine in Oxford&#8217;s Nuffield Department of Primary Care Health Sciences.
