{"id":43470,"date":"2018-11-19T16:48:51","date_gmt":"2018-11-19T15:48:51","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=43470"},"modified":"2018-11-26T18:46:02","modified_gmt":"2018-11-26T17:46:02","slug":"linda-bauld-why-we-need-to-keep-talking-about-obesity-and-cancer","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2018\/11\/19\/linda-bauld-why-we-need-to-keep-talking-about-obesity-and-cancer\/","title":{"rendered":"Linda Bauld: Why we need to keep talking about obesity and cancer"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/11\/Linda_Bauld.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-43471\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/11\/Linda_Bauld.jpg\" alt=\"\" width=\"160\" height=\"160\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/11\/Linda_Bauld.jpg 160w, https:\/\/blogs.bmj.com\/bmj\/files\/2018\/11\/Linda_Bauld-150x150.jpg 150w\" sizes=\"auto, (max-width: 160px) 100vw, 160px\" \/><\/a><a href=\"https:\/\/www.cancerresearchuk.org\/about-us\/cancer-news\/press-release\/2018-09-24-obesity-could-overtake-smoking-as-biggest-preventable-cause-of-cancer-in-women-0\">Obesity is predicted to become the biggest preventable cause of cancer <\/a><span style=\"font-weight: 400\">among UK women in the next 25 years, overtaking smoking.<\/span><\/p>\n<p><span style=\"font-weight: 400\">This is mostly down to falling smoking rates, but it\u2019s also in part due to rising levels of obesity. <\/span><\/p>\n<p><span style=\"font-weight: 400\">During the past four decades we\u2019ve seen substantial declines in smoking rates<\/span><span style=\"font-weight: 400\">\u2014<\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/healthandlifeexpectancies\/bulletins\/adultsmokinghabitsingreatbritain\/2017\"><span style=\"font-weight: 400\">smoking rates in Great Britain<\/span><\/a><span style=\"font-weight: 400\"> have fallen by 57% since the mid 1970\u2019s. Alongside this, we\u2019ve seen a rise in the number of people who are overweight or obese. Today <\/span><a href=\"https:\/\/www.gov.scot\/Publications\/2017\/10\/2970\/0\"><span style=\"font-weight: 400\">most<\/span><\/a> <a href=\"https:\/\/digital.nhs.uk\/data-and-information\/publications\/statistical\/health-survey-for-england\/health-survey-for-england-2016\"><span style=\"font-weight: 400\">adults<\/span><\/a> <a href=\"https:\/\/gov.wales\/statistics-and-research\/national-survey\/?lang=en\"><span style=\"font-weight: 400\">in<\/span><\/a> <a href=\"https:\/\/www.health-ni.gov.uk\/publications\/health-survey-northern-ireland-first-results-201617\"><span style=\"font-weight: 400\">the<\/span><\/a><span style=\"font-weight: 400\"> UK are overweight or obese. <\/span><\/p>\n<p><span style=\"font-weight: 400\">And while the fight continues to achieve a tobacco-free country, obesity can\u2019t be ignored.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Only 15% of people spontaneously listed obesity as a risk factor for cancer, when asked in a recent Cancer Research UK <\/span><a href=\"https:\/\/www.cancerresearchuk.org\/sites\/default\/files\/cam_2017_key_findings_201808.pdf\"><span style=\"font-weight: 400\">survey<\/span><\/a><span style=\"font-weight: 400\">. This is despite there being a large body of <\/span><a href=\"https:\/\/www.wcrf.org\/sites\/default\/files\/Body-fatness-and-weight-gain.pdf\"><span style=\"font-weight: 400\">evidence<\/span><\/a><span style=\"font-weight: 400\">, to support the association between obesity and <\/span><span style=\"font-weight: 400\">many common types of the disease, including breast, bowel and kidney cancer. <\/span><\/p>\n<p><span style=\"font-weight: 400\">GPs and other health professionals are uniquely placed to raise awareness of the health risks associated with obesity as well as support weight management. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Talking about weight isn\u2019t always easy, while one person might be aware of being overweight and happy to discuss changes that will help them lose weight, another may be offended by the topic. <\/span><span style=\"font-weight: 400\">Doctors might also have concerns about whether this discussion could change the dynamic of the consultation or be detrimental to the doctor patient relationship.<\/span><\/p>\n<p><span style=\"font-weight: 400\">But it is important to tackle this issue. Raising awareness is about helping people make informed choices about their own health, not about stigmatising people or telling them how they should live.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Cancer Research UK\u2019s obesity awareness campaign earlier this year received <\/span><a href=\"http:\/\/www.pulsetoday.co.uk\/clinical\/clinical-specialties\/cancer\/is-cancer-research-uks-obesity-campaign-fat-shaming\/20036430.article\"><span style=\"font-weight: 400\">some<\/span><\/a> <a href=\"https:\/\/www.bmj.com\/content\/360\/bmj.k1139\"><span style=\"font-weight: 400\">criticism<\/span><\/a><span style=\"font-weight: 400\"> from the public and health professionals, highlighting that this is an issue for debate. <\/span><\/p>\n<p><span style=\"font-weight: 400\">But this was a minority view. The evaluation of the campaign launch found that most people (84%) agreed it was an important message, while 8% said they found it offensive. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Some of the criticism suggested that Cancer Research UK was blaming individuals for their weight or trying to shame them into losing weight. But the charity has explained that its ultimate aim is to help change policies so it\u2019s easier for everyone to keep a healthy weight. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Smoking rates didn\u2019t come down on their own. The decline followed decades of effort to raise awareness about the health risks, coupled with strong policy action such as tobacco taxation, a ban on smoking in indoor public places, removing tobacco marketing and more recently introducing standardised packaging. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Cancer Research UK aims to help tackle obesity in a similar way, by pushing the government and food and drink industry to act in the interests of public health. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Making individual choices is important, and health professionals should support their patients to do so. But we also need to change the environment so it\u2019s easier for everyone to make those healthy choices. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Obesity is a complex issue with <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/reducing-obesity-future-choices\"><span style=\"font-weight: 400\">many causes,<\/span><\/a><span style=\"font-weight: 400\"> but one of the most important influences is the obesogenic environment around us. We\u2019re bombarded with junk food marketing, we have far greater and easier access to cheap and energy-dense food and drinks, and fewer opportunities to move in our everyday lives than ever before. <\/span><\/p>\n<p><span style=\"font-weight: 400\">That\u2019s why we need strong political will to introduce policies that help make healthy choices easier. Obesity isn\u2019t the same as smoking, and we can\u2019t address it in exactly the same way, but there\u2019s a lot to learn from the action that brought down smoking rates in the UK. <\/span><\/p>\n<p><span style=\"font-weight: 400\">We\u2019re campaigning for a 9pm watershed on junk food adverts on TV and similar protection for children viewing adverts on-demand and online as well as restrictions on price promotions in supermarkets. <\/span><\/p>\n<p><span style=\"font-weight: 400\">And the good news is that the tide might be starting to turn, with the release of <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/childhood-obesity-a-plan-for-action-chapter-2\"><span style=\"font-weight: 400\">chapter 2 of the Childhood Obesity Plan<\/span><\/a> <span style=\"font-weight: 400\">\u00a0for England earlier this year which proposes many important actions from our campaign.<\/span><\/p>\n<p><span style=\"font-weight: 400\">But there\u2019s a lot more work to be done\u2014not least to ensure the intentions set out in the <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/childhood-obesity-a-plan-for-action-chapter-2\"><span style=\"font-weight: 400\">plan<\/span><\/a><span style=\"font-weight: 400\"> become a reality. \u00a0\u00a0<\/span><\/p>\n<p><em><b>Linda Bauld <\/b>is professor of Public Health at the University of Edinburgh and holds the CRUK\/BUPA Chair in Cancer Prevention at Cancer Research UK.\u00a0<\/em><\/p>\n<p><em><strong>Competing interests<\/strong>: None declared.<\/em><b><\/b><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Obesity is predicted to become the biggest preventable cause of cancer among UK women in the next 25 years, overtaking smoking.
