Urgent changes are needed to ensure better care for these women and their babies. I believe there are clear opportunities for the Ministry of Justice, NHS England, and other stakeholders to provide prisons with legislation and resources to address these issues and work more effectively to improve outcomes for women and their children. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Few can fail to be shocked by the experiences of Layla, a women interviewed by Laura Abbott during doctoral research, reported in <\/span><em><a href=\"https:\/\/www.theguardian.com\/society\/2018\/nov\/13\/female-prisoners-in-england-left-to-give-birth-alone-in-their-cells-report-reveals\"><span style=\"font-weight: 400\">The Guardian<\/span><\/a><\/em><span style=\"font-weight: 400\"> this week. Layla was 24 weeks\u2019 pregnant with her second child when she was sent to prison, and had a footling breech birth at 36 weeks\u2019 gestation in her cell\u2014a birth with high risk of maternal and infant morbidity. Healthcare staff didn\u2019t believe her when she told them that she had lost her mucous plug, and again later when she told them she was in labour: \u201c\u2019<\/span><i><span style=\"font-weight: 400\">I\u2019m telling you I am in labour\u2019, \u2018No you\u2019re not. \u00a0Here\u2019s some paracetamol and a cup of tea.\u2019<\/span><\/i><span style=\"font-weight: 400\">\u201d Layla described the nurses being in <\/span><i><span style=\"font-weight: 400\">\u201cabsolute panic\u201d<\/span><\/i><span style=\"font-weight: 400\"> as she delivered her daughter in her cell and <\/span><i><span style=\"font-weight: 400\">\u201cnot even trained in that field.\u201d\u00a0<\/span><\/i><span style=\"font-weight: 400\">There were no provisions for Layla and her baby in the transfer to hospital, and she was not able to debrief her traumatic labour experience in the months that followed. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Aspects of Layla\u2019s experiences and other examples of inadequate care are reported by women across the prison estate. They represent significant risks not only to the women and babies involved, but also to prison and healthcare staff; none of whom should be put in the position of making decisions or supporting births for which they are unqualified (NMC Code 2015). To make matters worse, as <em>The Guardian<\/em>\u2019s report revealed, no data are kept on the numbers of pregnant women, births (in cells or otherwise), miscarriages, stillbirths, or adverse incidents in our prisons. How can proper care be planned, or accountability delivered, if no-one knows the numbers involved?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Birth Companions has been advocating for changes for over two years since the publication of our <\/span><a href=\"https:\/\/www.birthcompanions.org.uk\/media\/Public\/Resources\/Ourpublications\/Birth_Charter_Online_copy.pdf\"><span style=\"font-weight: 400\">Birth Charter for Women in Prison in England and Wales<\/span><\/a><span style=\"font-weight: 400\">, which contains comprehensive recommendations for perinatal women in custody. These include giving all pregnant women 24-hour phone access to midwives; allowing pregnant women the opportunity to live together on a designated wing; access to antenatal education; and recognition that unqualified staff should not make decisions about whether a woman is in labour or in need of midwifery care. <\/span><\/p>\n<p><span style=\"font-weight: 400\">There are positive signs that this advice is being listened to: some individual prisons are implementing some recommendations; Public Health England\u2019s <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/687146\/Gender_specific_standards_for_women_in_prison_to_improve_health_and_wellbeing.pdf\"><span style=\"font-weight: 400\">Standards to Improve Health and Wellbeing of Women in Prison in England<\/span><\/a><span style=\"font-weight: 400\"> contains a Pregnancy and Families section with strong aspirations for better care; Her Majesty\u2019s Prisons and Probation Service (HMPPS) will soon publish guidance on perinatal women in prison. However, the challenges of providing care in the custodial setting (as recently highlighted in the Parliamentary Health and Social Care Committee\u2019s <\/span><a href=\"https:\/\/publications.parliament.uk\/pa\/cm201719\/cmselect\/cmhealth\/963\/96302.htm\"><span style=\"font-weight: 400\">damning report into healthcare in prison<\/span><\/a><span style=\"font-weight: 400\">) means that good practice is extremely hard to maintain without more significant systemic change. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Thus, in response to Laura Abbott\u2019s research, Birth Companions recommends:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Data should be gathered and shared on the number of pregnant women in custody, and the number and nature of births, including those happening in prisons or during transfer to hospital. These data should include the miscarriages, stillbirths and adverse events experienced by perinatal women in custody.<\/span><\/li>\n<\/ul>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">These issues are addressed through a comprehensive policy framework that sets out clearly for prison staff their responsibilities to perinatal women and babies. The forthcoming MOJ framework for women offenders presents the perfect opportunity to do this.<\/span><\/li>\n<\/ul>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">NHS England should consider funding established models of good practice more widely.
