European Union (EU) guidelines are broader: they require all trials of medicinal products registered on their\u00a0<a href=\"https:\/\/www.clinicaltrialsregister.eu\/\">EU Clinical Trials Registry<\/a>\u00a0(EUCTR) since 2004 to report results within one year of completion. Our\u00a0<a href=\"https:\/\/fdaaa.trialstracker.net\/\">FDAAA TrialsTracker<\/a>\u00a0and\u00a0<a href=\"https:\/\/eu.trialstracker.net\/\">EU TrialsTracker<\/a>\u00a0show all individual trials that breach these legal requirements. Each week we write about one unreported clinical trial: you can read more background\u00a0<a href=\"http:\/\/blogs.bmj.com\/bmj\/2018\/03\/29\/it-is-time-to-fix-the-problem-of-unreported-clinical-trials\/\">here<\/a>, and past entries are\u00a0<a href=\"https:\/\/blogs.bmj.com\/bmj\/category\/unreported-trial-of-the-week\/\">here<\/a>.<\/p>\n<p><strong>Trials NCT02341430<\/strong><\/p>\n<p>This week\u2019s unreported trial is titled \u201cA Longitudinal Prospective Outcomes Study of Laparoscopic Abdominal Wall Hernia Repair Using Symbotex\u2122 Composite Mesh\u201d (<a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02341430\">NCT02341430<\/a>). This phase 4 trial had an estimated enrollment of 50 adult participants with a Grade I or II ventral hernia. The trial was not randomised with single group assignment for treatment with the Symbotex\u2122 Composite Mesh. The primary outcomes were hernia recurrence, quality of life, hernia associated pain and movement limitations, reoperation rate, percutaneous intervention rate, and adverse events.<\/p>\n<p><strong>Clinical discussion<\/strong><\/p>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5618132\/\">Surgical mesh<\/a>\u00a0is often used in hernia repair. Symbotex\u2122, one brand of mesh,\u00a0incorporates a bioabsorbable film into a nonabsorbable polyester mesh and is used to strengthen abdominal hernia repair and reduce rates of reoccurrence. Approved by the FDA in 2013, using the substantial equivalence route (<a href=\"https:\/\/www.accessdata.fda.gov\/scripts\/cdrh\/cfdocs\/cfpmn\/pmn.cfm?ID=K131969\">K131969<\/a>), Symbotex\u2122\u00a0<a href=\"https:\/\/www.medtronic.com\/covidien\/en-gb\/products\/hernia-repair\/symbotex-composite-mesh.html\">claims<\/a>\u00a0to be easier to deploy laparoscopically for abdominal hernias and reduce adhesions by minimising tissue attachment. However, there are\u00a0<a href=\"https:\/\/www.bmj.com\/content\/362\/bmj.k4104\">concerns that hernia meshes can cause severe complications<\/a>\u00a0including pain, infection, adhesions, obstruction and in some cases perforation of organs.\u00a0<a href=\"https:\/\/www.bmj.com\/content\/359\/bmj.j5515\">Trials of surgical mesh<\/a>\u00a0have been poorly designed, poorly conducted and often lacking in long-term follow-up to aid decision making. It is important that studies, like the one described above, are reported and\u00a0should inform recurrence and\u00a0complication rates in the first year after surgery to provide essential safety information.\u00a0 Results from another observational cohort study\u00a0(<a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02206828?term=Symbotex%E2%84%A2&amp;rank=1\">NCT02206828<\/a>) of 100 participants treated with Symbotex\u2122 and followed up for one year is not covered under FDAAA but\u00a0has recently had results submitted to\u00a0<a href=\"http:\/\/clinicaltrials.gov\/\">ClinicalTrials.gov<\/a>\u00a0and is currently undergoing quality control review.<\/p>\n<p><strong>Legislative discussion: when registry information doesn\u2019t add up<\/strong><\/p>\n<p>Sponsors are responsible for ensuring the information provided on their registries is accurate and up to date\u00a0<a href=\"https:\/\/www.gpo.gov\/fdsys\/pkg\/FR-2016-09-21\/pdf\/2016-22129.pdf\">per the FDAAA 2007<\/a>. While information submitted to\u00a0<a href=\"http:\/\/clinicaltrials.gov\/\">ClinicalTrials.gov<\/a>\u00a0does undergo quality control review, inaccurate or contradictory data can still be posted to the registry. For instance, the above trials is identified as a \u201cSingle Group Assignment.\u201d This is defined in the\u00a0<a href=\"https:\/\/clinicaltrials.gov\/ct2\/about-studies\/glossary\">ClinicalTrials.gov glossary<\/a>\u00a0as \u201ca type of intervention model describing a clinical trial in which all participants receive the same intervention\/treatment.\u201d However, the sponsor has also stated in registry entry that the trial was triple masked, meaning that the participant, care provider, and outcomes assessor are blinded to who receives which treatment. Masking makes little sense in the context of a trial with no comparator arm in which every patient recieved the same intervention\u2014and an invasive surgical intervention at that.<\/p>\n<p>The specified outcomes and completion dates are also at odds. As noted above, the sponsor identified six primary outcome measures, each meant to be assessed at \u201cone year.\u201d It is not clear from the entry whether these were meant to be assessed once, at one-year post-surgery, or continuously over the course of a year: best practice around prespecifying outcomes requires a description of what will be\u00a0<a href=\"https:\/\/www.equator-network.org\/reporting-guidelines\/spirit-2013-statement-defining-standard-protocol-items-for-clinical-trials\/\">measured, how, and when<\/a>. In any case, once each participant has had a year of follow-up, the study will have completed all data collection: however, the completion dates provided by the sponsor do not match this timeline. \u201cPrimary Completion Date\u201d and \u201cStudy Completion Date\u201d have very specific meanings on\u00a0<a href=\"http:\/\/clinicaltrials.gov\/\">ClinicalTrials.gov<\/a>. The primary completion date, used to assess when results are due to report under the FDAAA 2007, is defined as \u201cthe date on which the last participant in a clinical study was examined or received an intervention to collect final data for the primary outcome measure.\u201d The \u201cStudy Completion Date\u201d is defined as \u201cthe date on which the last participant in a clinical study was examined or received an intervention\/treatment to collect final data.\u201d In this trial, all the provided outcomes are \u201cprimary outcomes\u201d with one year follow-up. It is therefore impossible for the \u201cPrimary Completion Date\u201d and \u201cStudy Completion Date\u201d to differ and yet, according to the registry entry, they are separated by almost 2 years (11 Sept 2017 and 1 Sept 2019). This means that currently either the outcomes are incorrectly described or the completion dates for this trial are inaccurate, either of which is problematic for a third-party assessing this record.<\/p>\n<p><strong>Conclusion<\/strong><\/p>\n<p>This unreported trial was sponsored by\u00a0Anne Arundel Health System Research Institute in Maryland, USA\u00a0in collaboration with Medtronic Minimally Invasive Therapies Group (MITG). The PI is Dr. Adrian Park. As of 31 October 2018, this trial is 50 days overdue to report.
