{"id":43012,"date":"2018-09-11T18:12:47","date_gmt":"2018-09-11T17:12:47","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=43012"},"modified":"2018-09-18T16:17:47","modified_gmt":"2018-09-18T15:17:47","slug":"volatile-substance-abuse-overlooked-and-under-serviced","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2018\/09\/11\/volatile-substance-abuse-overlooked-and-under-serviced\/","title":{"rendered":"Volatile substance abuse\u2014overlooked and under-serviced"},"content":{"rendered":"<p>The recent release of new UK <a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/deaths\/bulletins\/deathsrelatedtodrugpoisoninginenglandandwales\/previousReleases\"><span style=\"font-weight: 400\">data<\/span><\/a><span style=\"font-weight: 400\"> on the number of deaths caused by <\/span><a href=\"https:\/\/www.nrscotland.gov.uk\/news\/2018\/934-drug-related-deaths-in-scotland-in-2017\"><span style=\"font-weight: 400\">drug misuse<\/span><\/a> <span style=\"font-weight: 400\">highlighted the continued high levels of harm associated with substance use. Of <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-45083167\"><span style=\"font-weight: 400\">particular concern<\/span><\/a><span style=\"font-weight: 400\"> in the ensuing media reports was the increase in fentanyl related deaths registered in England and Wales, which reached 75 in 2017\u2014an increase of 30% from 2016. <\/span><span style=\"font-weight: 400\">Yet absent from the ONS release, and failing to receive as much publicity on publication, was the number of <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/deaths\/datasets\/deathsrelatedtovolatilesubstanceabuseandheliumgreatbritain\"><span style=\"font-weight: 400\">deaths associated with volatile substances.<\/span><\/a><span style=\"font-weight: 400\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There have been 834 deaths since 2001 related to VSA in Great Britain, and 64 recorded in 2016 alone (including 25 suicides, latest data). Despite this, people who use volatile substances are not a research, policy, or practice priority, and we know little about who uses these substances, nor the best ways to support them with their health and social care needs. <\/span><\/p>\n<p><a href=\"http:\/\/www.emcdda.europa.eu\/publications\/drug-profiles\/volatile\"><span style=\"font-weight: 400\">Volatile substance abuse<\/span><\/a><span style=\"font-weight: 400\"> (VSA) refers to the intentional inhalation of volatile substances for psychoactive effects. Substances include aromatic and chlorinated hydrocarbons (e.g. toluene containing glues, cleaning fluids, paint); lighter fuels (butane, propane); alkyl, butyl, and isobutyl nitrites; petrol; fluorocarbons (aerosol propellants); and acetone (nail polish remover). As volatile substances are marketed for household or industrial purposes, they are readily available.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The effects of VSA are <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23018545\"><span style=\"font-weight: 400\">rapid in onset and of brief duration<\/span><\/a><span style=\"font-weight: 400\">, but they can be serious. Unfortunately, the UK has no publicly available data on hospital admissions and poisonings related to VSA. Our data collection systems for acute drug toxicity need to be <\/span><a href=\"http:\/\/www.emcdda.europa.eu\/activities\/emergencies\"><span style=\"font-weight: 400\">improved in general<\/span><\/a><span style=\"font-weight: 400\">, but sharing information on VSA would be a step towards a more coordinated response. We\u2019ve already seen how the publication of <\/span><a href=\"https:\/\/digital.nhs.uk\/news-and-events\/latest-news\/hospital-admissions-for-drug-related-mental-and-behavioural-disorders-fall-by-12-per-cent-in-a-year\"><span style=\"font-weight: 400\">data on other illicit drugs<\/span><\/a><span style=\"font-weight: 400\"> has provided an important <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/cocaine-powder-review-of-its-prevalence-patterns-of-use-and-harm\"><span style=\"font-weight: 400\">focus for action<\/span><\/a><span style=\"font-weight: 400\">, and improved <\/span><a href=\"https:\/\/www.theguardian.com\/society\/2018\/mar\/03\/admissions-to-hospital-for-drug-related-mental-health-problems-soar\"><span style=\"font-weight: 400\">public awareness<\/span><\/a><span style=\"font-weight: 400\"> of the potential negative impact of substance use. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Furthermore, while the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23018545\"><span style=\"font-weight: 400\">clinical presentations<\/span><\/a><span style=\"font-weight: 400\"> of VSA can be serious, including delirium (which can be prolonged), seizures, respiratory depression, and coma, the number of international <\/span><a href=\"https:\/\/www.nhmrc.gov.au\/about\/history-nhmrc\/national-institute-clinical-studies\/nics-programs\/volatile-substance-use\/volatil\"><span style=\"font-weight: 400\">clinical<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"http:\/\/www.inhalants.org\/guidelines.htm\"><span style=\"font-weight: 400\">practice<\/span><\/a><span style=\"font-weight: 400\"> guidelines are limited, with VSA failing to appear in the <\/span><a href=\"http:\/\/neptune-clinical-guidance.co.uk\/\"><span style=\"font-weight: 400\">NEPTUNE<\/span><\/a><span style=\"font-weight: 400\"> guidance. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Early detection is important as the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23649409\"><span style=\"font-weight: 400\">chronic effects<\/span><\/a><span style=\"font-weight: 400\"> of VSA are serious and include headaches, sleep disturbances, respiratory tract symptoms, renal damage, and neuropsychiatric impairments that may not always be reversible. Yet healthcare professionals\u2019 awareness of VSA and their associated competency is relatively low compared to other substances. <\/span><\/p>\n<p><span style=\"font-weight: 400\">At the root of this unfamiliarity may be a lack of media attention on the harms of VSA, the rapid rise and focus on <\/span><a href=\"https:\/\/www.bmj.com\/content\/356\/bmj.i6814\"><span style=\"font-weight: 400\">new psychoactive substances<\/span><\/a><span style=\"font-weight: 400\">, or perhaps even because of outdated associations with <\/span><a href=\"https:\/\/www.independent.co.uk\/arts-entertainment\/music\/sniffin-glue-a-fanzine-that-epitomized-punk-a7023131.html\"><span style=\"font-weight: 400\">1970s punk music<\/span><\/a><span style=\"font-weight: 400\">. There may also be the lingering perception that VSA is largely restricted to periods of adolescent experimentation; if so, that perception needs to be reframed. While <\/span><a href=\"https:\/\/digital.nhs.uk\/data-and-information\/publications\/statistical\/smoking-drinking-and-drug-use-among-young-people-in-england\/2016\"><span style=\"font-weight: 400\">school surveys in England<\/span><\/a><span style=\"font-weight: 400\">, have estimated that 4.4% of pupils aged 11-15 report use in the previous year\u2014making it the second most popular drug after cannabis\u2014most adolescent use tends to be time limited. <\/span><\/p>\n<p><span style=\"font-weight: 400\">The number of VSA deaths in young people in the UK has actually declined. While in contrast, almost three quarters of all VSA deaths are now in <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/birthsdeathsandmarriages\/deaths\/articles\/deathsrelatedtovolatilesubstancesandheliumingreatbritain\/2001to2016registrations\"><span style=\"font-weight: 400\">adults aged over 30<\/span><\/a><span style=\"font-weight: 400\">, and the mean age of death is increasing each year. This shift in the profile of people who die after VSA suggests that while policies, such as purchasing and <\/span><a href=\"https:\/\/echa.europa.eu\/substances-restricted-under-reach\/-\/dislist\/details\/0b0236e1807e2c14\"><span style=\"font-weight: 400\">manufacturing<\/span><\/a><span style=\"font-weight: 400\"> restrictions, may have helped to protect younger people from the most serious harm, it has not helped adults. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Acute presentation to health services might provide a good opportunity for assessment and care planning, but the common perception of VSA as something affecting young people may mean we are missing the signs in adults. Unlike <\/span><a href=\"https:\/\/www.gov.uk\/government\/collections\/drug-misuse-declared\"><span style=\"font-weight: 400\">other substances<\/span><\/a><span style=\"font-weight: 400\">, we no longer routinely collect data on adult VSA prevalence, so assessing the required health service response is made even more difficult. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Public Health England reported that 359 individuals presenting to drug treatment services in 2016\/17 declared use of \u201csolvents.\u201d Yet this may be an underestimate of need as VSA is often stigmatised or regarded by some as not a \u201creal\u201d form of substance use, and there is a lack of knowledge of effective service responses. <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/j.1360-0443.2011.03650.x\"><span style=\"font-weight: 400\">Systematic reviews<\/span><\/a><span style=\"font-weight: 400\"> have <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26969125\"><span style=\"font-weight: 400\">highlighted <\/span><\/a><span style=\"font-weight: 400\">the lack of high quality intervention research in this area, which makes recommending responses difficult. Public education on VSA,<\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/j.1465-3362.2010.00232.x\"> <span style=\"font-weight: 400\">information about harm reduction strategies for those unlikely to desist<\/span><\/a><span style=\"font-weight: 400\">, building the treatment evidence base (including updated healthcare guidance), and further<\/span><a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/09687630600762202\"> <span style=\"font-weight: 400\">development of supply reduction approaches<\/span><\/a><span style=\"font-weight: 400\"> are needed to prevent VSA and its associated harms.<\/span><\/p>\n<p><span style=\"font-weight: 400\">There should never be a competition for resources between vulnerable groups with health and social care needs; and no group is more \u201cdeserving\u201d than another. Nevertheless, with the drug and alcohol treatment sector in England already <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2017\/05\/25\/colin-drummond-cuts-to-addiction-services-in-england-are-a-false-economy\/\"><span style=\"font-weight: 400\">experiencing budget cuts<\/span><\/a><span style=\"font-weight: 400\"> of up to 30%, people who use volatile substances are unlikely to become a priority group, and it is telling that these substances are not mentioned anywhere in the <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/drug-strategy-2017\"><span style=\"font-weight: 400\">2017 Drugs Strategy<\/span><\/a><span style=\"font-weight: 400\">. Healthcare professionals and providers should review their understanding of and responses to VSA, to ensure that services provide high quality care for these often underserved and ignored members of our communities.<\/span><\/p>\n<p style=\"font-weight: 400\"><em><strong><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-43014 alignleft\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/09\/harry_sumnall.jpg\" alt=\"\" width=\"150\" height=\"142\" \/>Harry Sumnall<\/strong> is a professor in substance use at the Public Health Institute, Liverpool John Moores University.
