{"id":42768,"date":"2018-08-07T17:31:42","date_gmt":"2018-08-07T16:31:42","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=42768"},"modified":"2018-08-16T17:34:55","modified_gmt":"2018-08-16T16:34:55","slug":"technology-enabled-remote-consulting-save-time-add-value-primary-care","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2018\/08\/07\/technology-enabled-remote-consulting-save-time-add-value-primary-care\/","title":{"rendered":"Would technology enabled remote consulting save time and add value in primary care?"},"content":{"rendered":"<p class=\"standfirst\">Rising multimorbidity makes the evaluation and adoption of potentially time saving technologies ever more necessary, say Brian McKinstry and colleagues<\/p>\n<p><!--more--><span style=\"font-weight: 400\">Most face to face general practice appointments in the UK last at least eight to nine minutes almost regardless of the reason for the consultation.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn1\" name=\"_ftnref1\">[1]<\/a><a href=\"#_ftn2\" name=\"_ftnref2\">[2]<\/a><\/span><span style=\"font-weight: 400\"> Doctors and patients, respecting the effort involved in attending an appointment, expect a \u201creasonable\u201d consultation time.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn3\" name=\"_ftnref3\">[3]<\/a> <\/span><span style=\"font-weight: 400\">If someone has taken half a day off work to come and see the doctor for their results\u2014all of which are normal\u2014the doctor does not quickly say, \u201cgood news, it\u2019s all clear, goodbye.\u201d A consultation comes with an expectation of time to be spent on it, and this perception may lead the doctor or patient to raise another issue they may not otherwise have mentioned. <\/span><\/p>\n<p><span style=\"font-weight: 400\">This is not the case for a remote consultation, where the same effort has not been expended and it is perfectly acceptable for it to last less than a minute. Telephone consultations are generally shorter and often focused on a single problem.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn2\" name=\"_ftnref2\">[2]<\/a> <\/span><span style=\"font-weight: 400\">Although telephone triage of acute problems does not save time,<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn4\" name=\"_ftnref4\">[4]<\/a> <\/span><span style=\"font-weight: 400\">research suggests that follow-up appointments for established diagnoses, particularly long term conditions, are more efficiently carried out remotely and that patients like them.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn5\" name=\"_ftnref5\">[5]<\/a> <\/span><span style=\"font-weight: 400\">However, there are two main barriers to telephone consulting:\u00a0<\/span><span style=\"font-weight: 400\">the loss of non-verbal cues and the inability to physically examine a patient. <a href=\"#_ftn3\" name=\"_ftnref3\">[3]<\/a><\/span> <span style=\"font-weight: 400\">Technology may provide solutions for these. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Video consulting is little used in the UK, but may overcome the loss of non-verbal cues.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn6\" name=\"_ftnref6\">[6]<\/a><a href=\"#_ftn7\" name=\"_ftnref7\">[7]<\/a> <\/span><span style=\"font-weight: 400\">It could prove useful for following up patients who don\u2019t require a physical examination, particularly those with psychological problems where the assessment of affect is important. However, research is required to determine how easily it could be securely integrated into ageing NHS IT infrastructures, and whether it saves time or improves outcomes.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn8\" name=\"_ftnref8\">[8]<\/a><\/span><\/p>\n<p><span style=\"font-weight: 400\">Remote physical examination is a challenge. Although trials of remote monitoring of some long term conditions have been equivocal, there is strong evidence of clinical effectiveness for high blood pressure and type 2 diabetes.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn9\" name=\"_ftnref9\">[9]<\/a>[<a href=\"#_ftn10\" name=\"_ftnref10\">[10]<\/a> <\/span><span style=\"font-weight: 400\">Blood pressure management may provide an exemplar for clinical pathway changes that can save clinician and patient time. Based on Scottish figures, an estimated 12 million UK GP and practice nurse appointments annually are for blood pressure checks, even though patients&#8217; measurements of their blood pressure at home is more predictive of cardiovascular outcomes than clinic measures.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn11\" name=\"_ftnref11\">[11]<\/a><a href=\"#_ftn12\" name=\"_ftnref12\">[12]<\/a><\/span><span style=\"font-weight: 400\">\u00a0 \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Self-monitoring alone can leave patients feeling somewhat abandoned, however, clinician-supported telemonitoring enhances clinician-patient collaboration and improves blood pressure control.<\/span><span style=\"font-weight: 400\"> <a href=\"#_ftn9\" name=\"_ftnref9\">[9]<\/a><a href=\"#_ftn13\" name=\"_ftnref13\">[13]<\/a><a href=\"#_ftn14\" name=\"_ftnref14\">[14]<\/a><\/span><span style=\"font-weight: 400\"> Systems now exist that allow for telemonitored data to be integrated into monthly, three monthly, or six monthly reports within normal data management routines in practice. Clinicians can phone, text, or email advice, and patients appreciate avoiding surgery attendance.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn15\" name=\"_ftnref15\">[15]<\/a><\/span><span style=\"font-weight: 400\"> Recent price drops in sphygmomanometers and telemetry provide an opportunity to scale up this technology. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Remote consulting could free up time for patients with complex multimorbidity to have longer consultations.<\/span><span style=\"font-weight: 400\"><a href=\"#_ftn16\" name=\"_ftnref16\">[16]<\/a> <\/span><span style=\"font-weight: 400\">We should implement those technologies that have an evidence base, albeit in an evaluative framework, and explore the potential of others, such as video consulting. <\/span><span style=\"font-weight: 400\">The rapid growth in the prevalence of long term conditions makes the evaluation and adoption of potentially time saving technologies ever more necessary.<\/span><\/p>\n<p><em><span style=\"font-weight: 400\"><strong><img loading=\"lazy\" decoding=\"async\" class=\" wp-image-42778 alignleft\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/08\/Brian_McKinstry.jpg\" alt=\"\" width=\"114\" height=\"103\" \/>Brian McKinstry<\/strong> is a practising general practitioner and professor of primary care eHealth at the University of Edinburgh.
