Histories of &#8220;need&#8221; in the National Health Service"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/04\/jenny_crane.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-41877\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/04\/jenny_crane.jpg\" alt=\"\" width=\"160\" height=\"160\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/04\/jenny_crane.jpg 160w, https:\/\/blogs.bmj.com\/bmj\/files\/2018\/04\/jenny_crane-150x150.jpg 150w\" sizes=\"auto, (max-width: 160px) 100vw, 160px\" \/><\/a><span style=\"font-weight: 400\">After 5,500 votes, 23% of BMJ readers have stated that the &#8220;greatest achievement&#8221; of the NHS is that it is &#8220;based on need.&#8221; This concept is a historic one, embedded in the very inception of the NHS, which replaced an <\/span><a href=\"https:\/\/peopleshistorynhs.org\/encyclopaedia\/pre-nhs-healthcare-reforms\/\"><span style=\"font-weight: 400\">ad hoc system of local and voluntary health providers<\/span><\/a><span style=\"font-weight: 400\">, access to which was determined by ability to pay, area of residence, and workplace insurance systems.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In this context, the NHS had to be explained to\u2014or <\/span><a href=\"https:\/\/www.kingsfund.org.uk\/blog\/2017\/10\/nhs-and-public-historical-perspective\"><span style=\"font-weight: 400\">&#8220;learnt by&#8221;<\/span><\/a>\u2014<span style=\"font-weight: 400\">the British public. In 1948, <\/span><a href=\"https:\/\/wdc.contentdm.oclc.org\/digital\/collection\/health\/id\/1400\/rec\/1\"><span style=\"font-weight: 400\">a Central Office of Information<\/span><\/a><span style=\"font-weight: 400\"> leaflet stated that:<\/span><\/p>\n<p><i><span style=\"font-weight: 400\">&#8220;It will provide you with all medical, dental, and nursing care. Everyone\u2014rich or poor, man, woman or child\u2014can use it or any part of it.&#8221;<\/span><\/i><\/p>\n<p><span style=\"font-weight: 400\">Continuing, the leaflet stated that the service was &#8220;not a &#8216;charity'&#8221; but, rather, funded by taxation. The &#8220;family doctor&#8221;\u2014or general practitioner\u2014would be &#8220;paid by the Government, <\/span><a href=\"https:\/\/wdc.contentdm.oclc.org\/digital\/collection\/health\/id\/1400\/rec\/1\"><span style=\"font-weight: 400\">out of funds provided by everybody.&#8221;<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400\">The key concerns in this short leaflet thus echoed the three most <a href=\"https:\/\/www.bmj.com\/content\/361\/bmj.k2770\">popular options in the BMJ poll<\/a>. \u00a0Both emphasised the significance of the NHS as &#8220;based on need&#8221; and &#8220;non-commercialised,&#8221; and paid tribute to the role of general practice.<\/span><\/p>\n<p><span style=\"font-weight: 400\">These three options were all key facets ingrained in the very structure of the early NHS. By contrast, the fourth- and fifth- most popular choices in the BMJ poll\u2014<\/span><a href=\"https:\/\/peopleshistorynhs.org\/encyclopaedia\/childhood-vaccination-and-the-nhs\/\"><span style=\"font-weight: 400\">vaccination<\/span><\/a><span style=\"font-weight: 400\"> and anaesthesia\u2014were achievements of modern medicine. Notably, however, the NHS became the <\/span><a href=\"https:\/\/peopleshistorynhs.org\/encyclopaedia\/childhood-vaccination-and-the-nhs\/\"><span style=\"font-weight: 400\">&#8220;central instrument&#8221;<\/span><\/a><span style=\"font-weight: 400\">\u00a0for their distribution and use.<\/span><\/p>\n<p><span style=\"font-weight: 400\">While a key ideal from 1948, the ability of the NHS to distribute resources based on need alone soon came under attack. In 1952, charges were introduced for prescriptions, glasses, and dental services. <\/span><a href=\"https:\/\/peopleshistorynhs.org\/galleries\/portraits-of-bevan\/\"><span style=\"font-weight: 400\">Aneurin Bevan<\/span><\/a><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">the Minister of Health who introduced the NHS\u2014resigned accordingly. Contemporary <\/span><a href=\"http:\/\/www.nationalarchives.gov.uk\/education\/resources\/fifties-britain\/aneurin-bevan-resigns\/\"><span style=\"font-weight: 400\">Cabinet notes recorded his objections<\/span><\/a><span style=\"font-weight: 400\"> as that this move was: &#8220;politically dangerous,&#8221; financially unnecessary and, importantly, against the very &#8220;principle&#8221; of a free health service.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Since, and particularly from the 1970s and 1980s, campaigners, researchers, and NHS staff have highlighted and challenged threats to the NHS as based on need.\u00a0<\/span><a href=\"http:\/\/history.lshtm.ac.uk\/wp-content\/uploads\/sites\/9\/2013\/07\/The-Black-Report-and-the-Health-Divide-VB.pdf\"><span style=\"font-weight: 400\">The <\/span><i><span style=\"font-weight: 400\">Black Report<\/span><\/i><span style=\"font-weight: 400\">, published in 1980<\/span><\/a><span style=\"font-weight: 400\">, argued that economic inequalities continued to cause significant health inequalities, even with a health service free at the point of use.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Second-wave feminist and health campaigners have likewise drawn attention to gender disparities in the NHS. In the 1980s, the activist group <\/span><a href=\"https:\/\/www.healthemergency.org.uk\/\"><span style=\"font-weight: 400\">London Health Emergency<\/span><\/a><span style=\"font-weight: 400\"> criticised cuts made to cervical screening, family planning clinics, and abortion facilities, and argued that women would bear the brunt of shifts towards &#8220;community care.&#8221; These groups questioned why women\u2019s services\u2014and needs\u2014were persistently at the forefront of NHS cuts and closures.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Campaign groups used the concept of &#8220;needs&#8221; as a mobilising tool, as well as to enact critique. London Health Emergency and indeed the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2925204\/\"><span style=\"font-weight: 400\">Community Health Councils, which ran from 1974 until 2003<\/span><\/a><span style=\"font-weight: 400\">, asked members of the public to document and record local health needs, and used this information to lobby for change. <\/span><\/p>\n<p><span style=\"font-weight: 400\">From the 1990s, pharmaceutical companies, charities, and interest groups argued that &#8220;lifestyle drugs&#8221;\u2014notably Viagra\u2014should be provided free by the Service. These arguments became increasingly terse amidst political and public visions of NHS scarcity and crisis. Nonetheless, they also represented an important argument: that the NHS should be a National <\/span><i><span style=\"font-weight: 400\">Health<\/span><\/i><span style=\"font-weight: 400\"> Service, and that &#8220;medical needs&#8221; were fulfilled by improving quality of life, as well as by treating life-threatening illnesses.<\/span><\/p>\n<p><span style=\"font-weight: 400\">These debates, ongoing since the 1940s, raised a series of crucial questions. In a system that is free at the point of use, but governed by limited expenditure, <\/span><i><span style=\"font-weight: 400\">whose needs<\/span><\/i><span style=\"font-weight: 400\"> have been prioritised? How are hierarchies of needs constructed within NHS policy, and how do they reflect broader systems of inequality and power? These key and historic questions continue to be central in the NHS\u2019s 70<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> Anniversary year.
