{"id":42280,"date":"2018-06-05T14:31:08","date_gmt":"2018-06-05T13:31:08","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=42280"},"modified":"2018-06-05T14:32:42","modified_gmt":"2018-06-05T13:32:42","slug":"matthew-christie-medics-and-mental-health-the-disclosure-delusion","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2018\/06\/05\/matthew-christie-medics-and-mental-health-the-disclosure-delusion\/","title":{"rendered":"Matthew Christie: Medics and mental health\u2014the disclosure delusion"},"content":{"rendered":"<p class=\"standfirst\">To begin solving the problem we first need to admit that there is one<\/p>\n<p><!--more--><br \/>\n<a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/06\/matthew_christie.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-42285\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/06\/matthew_christie.jpg\" alt=\"\" width=\"160\" height=\"160\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/06\/matthew_christie.jpg 160w, https:\/\/blogs.bmj.com\/bmj\/files\/2018\/06\/matthew_christie-150x150.jpg 150w\" sizes=\"auto, (max-width: 160px) 100vw, 160px\" \/><\/a><\/p>\n<p><span style=\"font-weight: 400\">Mental health issues in doctors are common, <\/span><a href=\"http:\/\/webarchive.nationalarchives.gov.uk\/20130124042251\/http:\/\/www.dh.gov.uk\/prod_consum_dh\/groups\/dh_digitalassets\/@dh\/@en\/documents\/digitalasset\/dh_083090.pdf\"><span style=\"font-weight: 400\">more so than in the general population<\/span><\/a><span style=\"font-weight: 400\"> and doctors have higher<\/span><a href=\"https:\/\/www.cambridge.org\/core\/journals\/advances-in-psychiatric-treatment\/article\/improving-the-health-of-psychiatrists\/511A513C1D4BD23F9BF0FB1D7BA4C494\"><span style=\"font-weight: 400\"> rates of suicide<\/span><\/a><span style=\"font-weight: 400\">. Though problems often manifest early in training, they can and do <\/span><a href=\"http:\/\/php.nhs.uk\/wp-content\/uploads\/sites\/26\/2013\/11\/Brooks-2011-review-of-Drs-mental-health.pdf\"><span style=\"font-weight: 400\">occur at any career stage<\/span><\/a><span style=\"font-weight: 400\">. This is not just detrimental for the individual, but also to patient care and has broader <\/span><a href=\"http:\/\/www.healthyregions.eu\/uk\/Boorman%20NHS%20HWB%20Final%20Report%20Nov%2009.pdf\"><span style=\"font-weight: 400\">economic impacts<\/span><\/a><span style=\"font-weight: 400\"> through sick days and dropout.<\/span><\/p>\n<p><span style=\"font-weight: 400\">There are many risk factors that can predispose doctors to mental ill health. Occupational aspects such as the nature of the work and work environment as well as individual personal factors which interact to make doctors vulnerable to mental health problems. Increasing workload and less predictable hours can exacerbate these risks. Despite efforts to improve mental health awareness in doctors and open a positive dialogue, <\/span><a href=\"https:\/\/www.cambridge.org\/core\/journals\/the-british-journal-of-psychiatry\/article\/workrelated-ill-health-in-doctors-working-in-great-britain-incidence-rates-and-trends\/CA0F4692FAE857DF3E3EB076F6B512D5\"><span style=\"font-weight: 400\">the problem appears to be growing.<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400\">For a doctor to properly address their mental health problem requires disclosure; a willingness to acknowledge that a problem exists, and to seek help and relay that information, as appropriate, in the workplace. Early disclosure leads to prompter diagnosis, treatment, and improved outcomes. In fact, doctors who disclose and engage with specialist services have a <\/span><a href=\"http:\/\/www.tandfonline.com\/doi\/abs\/10.3109\/09638237.2012.734647?journalCode=ijmh20\"><span style=\"font-weight: 400\">better prognosis than the general population<\/span><\/a><span style=\"font-weight: 400\">. However, disclosure can be a very difficult step to take. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The obstacles against disclosure are numerous and complex. \u00a0Some are specific to our profession, including concerns about career progression, GMC involvement, or difficulty in transitioning from doctor to patient roles. Others are more general, such as the ongoing stigma around mental health problems.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Crucially, however, there is a great disparity between how we as doctors believe we would behave if faced with mental ill health and the evidence of how doctors actually do behave when unwell.<\/span><\/p>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/?term=cohen+2016+Understanding+doctors%E2%80%99+attitudes+towards+self-disclosure+of+mental+ill%C2%A0health\"><span style=\"font-weight: 400\">Research<\/span><\/a><span style=\"font-weight: 400\">\u00a0shows that doctors are not only less likely to disclose, but do so at a later stage than they thought and the reasons for disclosure are different from what they imagined. Most believed professional responsibility would be their main reason for disclosure, but in reality, the driving force is the need for advice and treatment.<\/span><\/p>\n<p><span style=\"font-weight: 400\">A lack of understanding of support structures remains a significant hindrance, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/?term=cohen+2016+Understanding+doctors%E2%80%99+attitudes+towards+self-disclosure+of+mental+ill%C2%A0health\"><span style=\"font-weight: 400\">with only 6% of doctors stating they would go to occupational health and no trainee saying that they would go to their deanery support unit<\/span><\/a><span style=\"font-weight: 400\"> if they became unwell, despite these being the recognised support pathways. Lack of understanding of these pathways may explain why trainees and younger doctors felt less likely to disclose.<\/span><\/p>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/?term=cohen+2016+Understanding+doctors%E2%80%99+attitudes+towards+self-disclosure+of+mental+ill%C2%A0health\"><span style=\"font-weight: 400\">Many doctors cite concerns about being labelled as a key obstacle to disclosure<\/span><\/a><span style=\"font-weight: 400\"> indicating stigma remains a major factor.
