Our\u00a0<a href=\"https:\/\/fdaaa.trialstracker.net\/\">FDAAA TrialsTracker<\/a>\u00a0shows all individual trials that\u00a0<a href=\"https:\/\/www.biorxiv.org\/content\/early\/2018\/03\/12\/266452.full.pdf\">breach<\/a>\u00a0this legal requirement. Once a week, we write about one unreported clinical trial: you can read more background\u00a0<a href=\"https:\/\/blogs.bmj.com\/bmj\/2018\/03\/29\/it-is-time-to-fix-the-problem-of-unreported-clinical-trials\/\">here<\/a>.<\/p>\n<p style=\"font-weight: 400\"><strong>Trial NCT02097641<\/strong><\/p>\n<p style=\"font-weight: 400\">This week\u2019s unreported trial is titled: \u201cProspective, randomized, multi-center phase 2 clinical trial of allogeneic bone marrow-derived human mesenchymal stem cells for the treatment of acute respiratory distress syndrome\u201d (<a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT02097641\">NCT02097641<\/a>). The trial enrolled 60 adult participants with respiratory distress. The intervention group received a single infusion of human mesenchymal stem cells; the control group received a placebo infusion of Plasma-Lyte A. The study was blinded for participants, care providers, and investigators. The primary outcomes were incidence of pre-specified infusion events within six hours, any cardiac arrest or death within 24 hours, and any other unexpected severe adverse effects over 12 months. The secondary outcomes were a combination of respiratory, systemic, and biological efficacy endpoints. This study is an extension of a Phase 1 study (<a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT01775774\">NCT01775774<\/a>) which reported results onto ClinicalTrials.gov in August 2017.<\/p>\n<p style=\"font-weight: 400\"><strong>Clinical Discussion<\/strong><\/p>\n<p style=\"font-weight: 400\">Human mesenchymal stem cells are a\u00a0<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3712145\/\">promising area of clinical study<\/a>. The first mesenchymal stem cell therapy recently\u00a0<a href=\"https:\/\/www.takeda.com\/newsroom\/newsreleases\/2018\/tigenix-and-takeda-announce-alofisel-receives-approval-in-europe\/\">gained approval<\/a>\u00a0from European regulators and the US FDA has a\u00a0<a href=\"https:\/\/edition.cnn.com\/2018\/03\/08\/health\/stem-cell-therapy-fda-bn\/index.html\">stated interest in<\/a>\u00a0\u201cexpediting the development of stem cell therapies proved to be safe and effective.\u201d<a href=\"https:\/\/www.bmj.com\/content\/359\/bmj.j5055\">Acute respiratory distress syndrome<\/a>\u00a0(ARDS) is an \u201cacute inflammatory lung injury, often caused by infection&#8230;resulting in hypoxaemic respiratory failure.\u201d It is\u00a0<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3410685\/\">estimated that<\/a>\u00a0ARDS accounts for ~5% of all mechanically ventilated patients and 75% of all ARDS cases are categorized as \u201cmoderate or severe.\u201d\u00a0<a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2492877\">A study of 459 ICUs across 50 countries<\/a>\u00a0found that ARDS criteria was met in 10.4% of all ICU admissions with 23.4% requiring mechanical ventilation. The unadjusted mortality rate was 35.5% in ICUs and 40% in hospitals. Estimated average daily ICU cost for an ARDS patient is\u00a0<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4261619\/\">$2,278-$3,518 in the US<\/a>\u00a0and\u00a0<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4982209\/\">\u00a31738 in the UK<\/a>\u00a0with the mean societal cost of a UK ARDS patient at one year estimated at \u00a344,077. It is imperative that the researchers fully share the results of research into this cutting edge therapy for a costly, and often deadly, disease.<\/p>\n<p style=\"font-weight: 400\"><strong>Legislative Discussion<\/strong><\/p>\n<p style=\"font-weight: 400\">We intend that this series should occasionally shed light on interesting issues around transparency rules, and how registry data is used. You can read some general background about the FDA Amendments Act 2007\u2014and why a trial is considered \u201cdue\u201d\u2014<a href=\"https:\/\/www.biorxiv.org\/content\/early\/2018\/03\/12\/266452\">here<\/a>\u00a0and\u00a0<a href=\"https:\/\/ebmdatalab.net\/why-is-this-trial-due-to-report\/\">here<\/a>.<\/p>\n<p style=\"font-weight: 400\">This is the first trial in our Unreported Clinical Trial series that is\u00a0<em>not<\/em>\u00a0on a drug. It is important to note that the reporting requirements of the FDAAA 2007 extend beyond drug trials alone. When considering if trials that started prior to January 18, 2017 are covered by FDAAA 2007, the \u201c<a href=\"https:\/\/prsinfo.clinicaltrials.gov\/prs-users-guide.html#fdaaa801problems\">PRS User Guide for ClinicalTrials.gov<\/a>\u201d includes the following intervention types: Drug, Device, Biological\/Vaccine, Radiation, Genetic, Combination Product, or Diagnostic Test. This trial, on a stem cell based therapy, is a biological intervention: it is therefore covered under the FDAAA 2007 and required to report within one year of the primary completion date.<\/p>\n<p style=\"font-weight: 400\"><strong>Conclusion<\/strong><\/p>\n<p style=\"font-weight: 400\">This unreported trial was sponsored by Dr.\u00a0Michael Mattay of the University of California, San Francisco who also took responsibility for the trial entry.
