Adults who are eligible for continuing healthcare (CHC) funding and children who receive continuing care have had a legal right to a personal health budget (PHB) since 2014. The aim of PHBs is to give individuals more choice and control over the money spent on their health and wellbeing needs. By 2020, NHS England expects an uptake of <a href=\"https:\/\/www.sbs.nhs.uk\/PHBChoices-about\">at least 100,000 people<\/a>.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Uptake has been slow due to a lack of awareness about PHBs amongst patients and families, as well as amongst care professionals. The system to apply for a PHB is also difficult to navigate. Currently only people with a <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2018\/03\/25\/anya-de-jongh-how-can-healthcare-services-deliver-on-the-vision-for-personalised-care\/\"><span style=\"font-weight: 400\">high level of complexity<\/span><\/a><span style=\"font-weight: 400\"> qualify for a PHB. <\/span><\/p>\n<p><span style=\"font-weight: 400\">A service user seeking a PHB is given a social care assessment with the help of a care manager. \u00a0A support plan and budget is calculated based on the information given by the service user. It is means tested. Service users may choose to have their PHB paid directly to them, or managed by the NHS (notional budget) or a third party.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Recently, <\/span><a href=\"https:\/\/sharedlivesplus.org.uk\"><span style=\"font-weight: 400\">Shared Lives Plus<\/span><\/a><span style=\"font-weight: 400\">, a UK network which matches people who want extra support at home with people who have a spare room and are able to help provide care, were commissioned by NHS England to <a href=\"https:\/\/sharedlivesplus.org.uk\/information-and-publications\/item\/475-personal-health-budgets-guide-2018\">publish a guide to PHBs<\/a> for the voluntary, community, and social enterprise (VCSE) sector<\/span><span style=\"font-weight: 400\">. It states that the VCSE sector could and should play a big part in helping people to access and use PHBs, as well as providing and brokering new kinds of support. It claims that VCSEs have \u201ca track record of reaching local groups or communities\u201d which \u201cNHS organisations have struggled to reach consistently.\u201d The guide gives recommendations for VCSEs to work with service users and to coordinate with CCGs and the NHS, and illustrates this with some inspirational case studies. <\/span><\/p>\n<p><span style=\"font-weight: 400\">However, the ideas it promotes raise questions about the training needs of volunteers. From the public\u2019s point of view, voluntary services are being exploited to plug the gaps in statutory care services, often without NHS funding. Certainly, there is a <\/span><span style=\"font-weight: 400\">discrepancy between increasing expectations on the voluntary sector and decreasing funding.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Evidence presented by the <a href=\"https:\/\/www.nao.org.uk\/report\/personalised-commissioning-in-adult-social-care\/\">National Audit Office (NAO) in 2016 <\/a><\/span><span style=\"font-weight: 400\">suggested that PHBs were not working. There was, and continues to be, insufficient funding, set against increasing service users\u2019 needs. This is a familiar story. This year, <\/span><a href=\"http:\/\/www.disabledunited.com\"><span style=\"font-weight: 400\">Disability United<\/span><\/a><span style=\"font-weight: 400\">, an online support network for the disabled community, found at least 37 CCG\u2019s putting restrictions on CHC funding, limiting people\u2019s right to receive care at home. This is happening against a background of NHS England seeking \u00a3855 million of efficiency savings in CHC and <\/span><a href=\"https:\/\/publications.parliament.uk\/pa\/cm201719\/cmselect\/cmpubacc\/455\/455.pdf\"><span style=\"font-weight: 400\">NHS-funded nursing care<\/span><\/a><span style=\"font-weight: 400\"> spending by 2020-21. The current system needs to be replaced and the PHB funding gap needs to be acknowledged and corrected. <\/span><\/p>\n<p><span style=\"font-weight: 400\">At a time when the government is seeking efficiency savings, increased funding looks unlikely. The Public Accounts Committee of the House of Commons has made recommendations\u00a0to the Department of Health and NHS England which address the shortcomings of CHC. They will <a href=\"https:\/\/publications.parliament.uk\/pa\/cm201617\/cmselect\/cmpubacc\/74\/74.pdf\">report back to the Committee by April 2018<\/a><\/span><span style=\"font-weight: 400\">. While <a href=\"https:\/\/www.england.nhs.uk\/publication\/national-expansion-plan-for-personal-health-budgets-and-integrated-personal-commissioning\/\">NHS England\u2019s vision for PHBs\u00a0<\/a><\/span><span style=\"font-weight: 400\">focuses on the mechanics of delivery, together with <a href=\"https:\/\/www.england.nhs.uk\/publication\/harnessing-technology-for-personalised-care-final-report-on-the-national-it-challenge-process\/\">developing effective digital systems<\/a><\/span><span style=\"font-weight: 400\">, the patient\u2019s voice needs to be heard above all, especially the voices of those who have received PHB\u2019s in the last four years. <a href=\"https:\/\/www.peoplehub.org.uk\">Peoplehub<\/a>, a PHB network affiliated to NHS England, takes a step in the right direction. <a href=\"https:\/\/www.thinklocalactpersonal.org.uk\/\">Think Local Act Personal<\/a> (TLAP), a national partnership transforming health and care through personalisation and community-based support, have written a report \u201c<a href=\"https:\/\/www.thinklocalactpersonal.org.uk\/Latest\/Gathering-the-Evidence-Making-Personal-Budgets-Work-for-All-report\/\">Gathering the Evidence: Making PHBs Work for All<\/a>\u201d<\/span> <span style=\"font-weight: 400\">which reinforces this prime need. TLAP have also written a guidance, \u201c<a href=\"https:\/\/www.thinklocalactpersonal.org.uk\/Latest\/Beyond-direct-payments\/\">Beyond Direct Payments<\/a>,\u201d<\/span><span style=\"font-weight: 400\">\u00a0which offers alternative options to direct payments.
