Stroke prevention with anticoagulants has long been known to be highly effective and new drugs <\/span><span id=\"E84\" class=\"qowt-font1-Calibri\">are <\/span><span id=\"E85\" class=\"qowt-font1-Calibri\">also <\/span><span id=\"E86\" class=\"qowt-font1-Calibri\">available<\/span><span id=\"E87\" class=\"qowt-font1-Calibri\">. Historically many patients with atrial fibrillation were not treated. <\/span><\/p>\n<p id=\"E88\"><span id=\"E89\" class=\"qowt-font1-Calibri\">For several years w<\/span><span id=\"E90\" class=\"qowt-font1-Calibri\">e have investigated the management of atrial fibrillation, analysing a large database of UK electronic primary care records. Such databases are an unparalleled resource for understanding how the health service delivers care. We found confusion in understanding the importance of contraindications to anticoagulants<\/span><span id=\"E91\" class=\"qowt-font1-Calibri\"> and <\/span><span id=\"E92\" class=\"qowt-font1-Calibri\">that stroke prevention generally seemed to be better in patients who are on chronic disease registers and therefore followed up regularly. [1,2,3,4]<\/span><\/p>\n<p id=\"E93\"><span id=\"E94\" class=\"qowt-font1-Calibri\">A<\/span><span id=\"E95\" class=\"qowt-font1-Calibri\"> <\/span><span id=\"E96\" class=\"qowt-font1-Calibri\">big question for the health service is whether we should focus efforts on finding more people with undiagnosed atrial fibrillation or on treating more of those <\/span><span id=\"E97\" class=\"qowt-font1-Calibri\">with a<\/span><span id=\"E98\" class=\"qowt-font1-Calibri\"> diagnosis<\/span><span id=\"E99\" class=\"qowt-font1-Calibri\">. It was thinking about this that led us to another quite different question.<\/span><\/p>\n<p id=\"E100\"><span id=\"E101\" class=\"qowt-font1-Calibri\">We initially wanted to describe trends in the prevalence of diagnosed atrial fibrillation and the proportion of <\/span><span id=\"E102\" class=\"qowt-font1-Calibri\">patients with a diagnosis <\/span><span id=\"E103\" class=\"qowt-font1-Calibri\">who were treated. In our primary care records <\/span><span id=\"E105\" class=\"qowt-font1-Calibri\">database<\/span><span id=\"E107\" class=\"qowt-font1-Calibri\"> we identified <\/span><span id=\"E108\" class=\"qowt-font1-Calibri\">those<\/span><span id=\"E109\" class=\"qowt-font1-Calibri\"> <\/span><span id=\"E110\" class=\"qowt-font1-Calibri\">with a documented diagnosis, excluding those in whom the diagnosis was subsequently <\/span><span id=\"E111\" class=\"qowt-font1-Calibri\">recorded<\/span><span id=\"E112\" class=\"qowt-font1-Calibri\"> as resolved. We counted the number of patients <\/span><span id=\"E113\" class=\"qowt-font1-Calibri\">with a diagnosis<\/span><span id=\"E114\" class=\"qowt-font1-Calibri\"> <\/span><span id=\"E115\" class=\"qowt-font1-Calibri\">and the number of those <\/span><span id=\"E116\" class=\"qowt-font1-Calibri\">with a diagnosis<\/span><span id=\"E117\" class=\"qowt-font1-Calibri\"> <\/span><span id=\"E118\" class=\"qowt-font1-Calibri\">who were then treated. <\/span><span id=\"E119\" class=\"qowt-font1-Calibri\">Y<\/span><span id=\"E120\" class=\"qowt-font1-Calibri\">ear on year <\/span><span id=\"E121\" class=\"qowt-font1-Calibri\">t<\/span><span id=\"E122\" class=\"qowt-font1-Calibri\">here were <\/span><span id=\"E123\" class=\"qowt-font1-Calibri\">increases in recorded prevalence and in the proportion of patients treated. [5]<\/span><\/p>\n<p id=\"E124\"><span id=\"E125\" class=\"qowt-font1-Calibri\">But our method prompted a series of questions<\/span><span id=\"E126\" class=\"qowt-font1-Calibri\">.<\/span><span id=\"E127\" class=\"qowt-font1-Calibri\"> Did we exclude many participants when we excluded those in whom atrial fibrillation <\/span><span id=\"E128\" class=\"qowt-font1-Calibri\">seemed<\/span><span id=\"E129\" class=\"qowt-font1-Calibri\"> <\/span><span id=\"E130\" class=\"qowt-font1-Calibri\">to have resolved? Yes, about 8%. How can a clinician really know if atrial fibrillation has resolved? We already know it can be intermittent. Can you really tell if it has gone completely? And doesn\u2019t it often recur? Would it be possible to determine from patient records if resolved atrial fibrillation had really resolved? <a href=\"https:\/\/www.bmj.com\/content\/361\/bmj.k1717\">Then we hit on the right question<\/a>. What is the risk of stroke in those whose condition is documented as resolved? Treatment of atrial fibrillation is determined by stroke risk. Whether the arrhythmia is intermittent, recurs silently, or has gone for good are distractions from what should inform clinical decisions. If stroke risk in resolved atrial fibrillation falls to the levels of those without atrial fibrillation then most anticoagulants aren\u2019t necessary, but if risk remains as high as those with atrial fibrillation then they probably are. <\/span><\/p>\n<p id=\"E131\"><span id=\"E132\" class=\"qowt-font1-Calibri\">We found that stroke risk was higher in those with resolved atrial fibrillation than those without atrial fibrillation. A further analysis strengthen<\/span><span id=\"E133\" class=\"qowt-font1-Calibri\">ed <\/span><span id=\"E134\" class=\"qowt-font1-Calibri\">the conclusions. In terms of their stroke risk<\/span><span id=\"E135\" class=\"qowt-font1-Calibri\">,<\/span><span id=\"E136\" class=\"qowt-font1-Calibri\"> people with resolved atrial fibrillation were becoming more like those with atrial fibrillation. In recent years the risk of stroke in those with resolved atrial fibrillation was the same as in those with atrial fibrillation. This is important because the frequency of resolved atrial fibrillation seems to have increased markedly after 2005. It was also clear that once atrial fibrillation is considered resolved, the prescriptions of anticoagulants almost always stop. We had discovered a group of patients at high risk of stroke who aren\u2019t on the chronic disease register. Because they aren\u2019t counted as at high risk, they won\u2019t appear as untreated <\/span><span id=\"E137\" class=\"qowt-font1-Calibri\">in any analysis of anticoagulant use. Ironically this has happened at about the same time as dramatic improvements in the identification and <\/span><span id=\"E141\" class=\"qowt-font1-Calibri\">management of atrial fibrillation.<\/span><span style=\"font-size: 1rem\">\u00a0<\/span><\/p>\n<\/div>\n<\/div>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<p id=\"E142\"><span id=\"E143\" class=\"qowt-font1-Calibri\">W<\/span><span id=\"E144\" class=\"qowt-font1-Calibri\">ithout databases of electronic patient records<\/span><span id=\"E145\" class=\"qowt-font1-Calibri\"> <\/span><span id=\"E146\" class=\"qowt-font1-Calibri\">i<\/span><span id=\"E147\" class=\"qowt-font1-Calibri\">t is impossible to know what is really happening to patients. Our analysis did not rely on any innovative methods and we have to acknowledge constructive input from reviewers.
