{"id":41906,"date":"2018-04-20T17:44:30","date_gmt":"2018-04-20T16:44:30","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=41906"},"modified":"2018-04-27T17:04:18","modified_gmt":"2018-04-27T16:04:18","slug":"kailash-chand-racism-is-still-rife-in-the-nhs","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2018\/04\/20\/kailash-chand-racism-is-still-rife-in-the-nhs\/","title":{"rendered":"Kailash Chand: Racism is still rife in the NHS"},"content":{"rendered":"<p class=\"standfirst\">Staff from black and minority ethnic backgrounds have been essential to the NHS throughout its history, yet there&#8217;s still much to be done to ensure they&#8217;re treated equally<\/p>\n<p><!--more--><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-41908\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/04\/kailash_chand.jpg\" alt=\"\" width=\"160\" height=\"160\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2018\/04\/kailash_chand.jpg 160w, https:\/\/blogs.bmj.com\/bmj\/files\/2018\/04\/kailash_chand-150x150.jpg 150w\" sizes=\"auto, (max-width: 160px) 100vw, 160px\" \/>On the 25<span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> anniversary of Stephen Lawrence\u2019s death, racism and discrimination are still rife in the UK, including in the NHS. For too many, the mindset has not changed much since the time of Stephen\u2019s death. We still witness many cases of overt and institutional racism<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">the case of <a href=\"https:\/\/www.bmj.com\/bawa-garba\">Hadiza Bawa Garba<\/a>, the nonsensical move to try to deport GP trainee <a href=\"https:\/\/www.bmj.com\/content\/361\/bmj.k1693\">Luke Ong<\/a>, and the Windrush saga are all glaring examples. You cannot help but think that the improvements we so wanted to see were only skin deep. The <\/span><a href=\"https:\/\/www.theguardian.com\/uk-news\/2018\/apr\/09\/rights-groups-decry-theresa-may-hostile-environment-immigration-policy\"><span style=\"font-weight: 400\">\u201chostile environment\u201d<\/span><\/a><span style=\"font-weight: 400\"> in the UK, which prime minister Theresa May <\/span><a href=\"https:\/\/www.theguardian.com\/uk-news\/2018\/apr\/17\/theresa-mays-hostile-environment-policy-at-heart-of-windrush-scandal\"><span style=\"font-weight: 400\">has rightly been castigated for, has become the norm<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In 1997 Jack Straw, the then Labour home secretary, asked Sir William Macpherson to lead an inquiry into how public institutions could address the needs of an ethnically diverse society and judge how well they were doing.\u00a0<\/span><span style=\"font-weight: 400\">The <\/span><a href=\"http:\/\/www.archive.official-documents.co.uk\/document\/cm42\/4262\/4262.htm\"><span style=\"font-weight: 400\">inquiry concluded that institutional racism<\/span><\/a><span style=\"font-weight: 400\"> was rife in the police service and in all probability in other public institutions.<\/span><\/p>\n<p><a href=\"https:\/\/www.theguardian.com\/uk\/1999\/feb\/24\/lawrence.ukcrime12\"><span style=\"font-weight: 400\">The Macpherson report<\/span><\/a><span style=\"font-weight: 400\"> demonstrated how <\/span><a href=\"http:\/\/tinyurl.com\/oscvkap\"><span style=\"font-weight: 400\">deeply<\/span><\/a><span style=\"font-weight: 400\"> racial discrimination had become embedded in the culture of public services. <\/span><span style=\"font-weight: 400\">Stephen Lawrence\u2019s tragic story led to great national introspection about the persistence of deep rooted racism and discrimination in the UK\u2014and the NHS was not exempt. By all accounts, <\/span><span style=\"font-weight: 400\">institutional racism is still a major problem in the health service. Surveys show that<\/span><span style=\"font-weight: 400\"> significant numbers of NHS staff from black or ethnic minority backgrounds still <a href=\"http:\/\/www.bbc.co.uk\/news\/health-36427577\">experience more discrimination and bullying<\/a> in the workplace than their white colleagues<\/span><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">It&#8217;s worth observing that the NHS has a poor track record in tackling other forms of discrimination too. Disabled staff and LGBT staff <\/span><a href=\"http:\/\/www.nhsstaffsurveys.com\/Page\/1019\/Past-Results\/Staff-Survey-2016-Detailed-Spreadsheets\/\"><span style=\"font-weight: 400\">experience higher levels<\/span><\/a><span style=\"font-weight: 400\"> of bullying, and women are still seriously under-represented in senior posts.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Yet it\u2019s been previously found that doctors from a black and minority ethnic (BME) background are more likely to be involved in disciplinary procedures; they are <\/span><a href=\"https:\/\/books.google.co.uk\/books?id=mygDBQAAQBAJ&amp;pg=PA305&amp;lpg=PA305&amp;dq=BMJ++General+Medical+Council;+they+are+three+times+more+likely+to+be+charged+with+serious+professional+misconduct&amp;source=bl&amp;ots=v54w160elL&amp;sig=tePnEcUpJO64GSoC_WagpvFNAV0&amp;hl=en&amp;sa=X&amp;ved=0ahUKEwiapNGJzsjaAhWsB8AKHbdyBcsQ6AEIUTAI#v=onepage&amp;q=BMJ%20%20General%20Medical%20Council%3B%20they%20are%20three%20times%20more%20likely%20to%20be%20charged%20with%20serious%20professional%20misconduct&amp;f=false\"><span style=\"font-weight: 400\">three times more likely to be charged with serious professional misconduct<\/span><\/a><span style=\"font-weight: 400\"> by the General Medical Council. And BME doctors are <\/span><a href=\"http:\/\/www.pulsetoday.co.uk\/trainee-pulse\/rcgp-fails-to-stop-bme-trainees-falling-behind-in-exam-results\/20034913.article\"><span style=\"font-weight: 400\">still more likely to fail the MRCGP final exam<\/span><\/a><span style=\"font-weight: 400\"> than white doctors, despite this being identified as <\/span><a href=\"https:\/\/www.bapio.co.uk\/bapio-response-to-the-independent-review-of-membership-of-the-royal-college-of-general-practitioners-mrcgp-examination-by-prof-a-esmail-and-prof-c-roberts\/\"><span style=\"font-weight: 400\">an issue for many years<\/span><\/a><span style=\"font-weight: 400\">. It would be hard to argue that this marked difference in failure rates is a matter of chance, diligence, or intelligence.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Well over <\/span><a href=\"https:\/\/www.ethnicity-facts-figures.service.gov.uk\/work-pay-and-benefits\/public-sector-workforce\/nhs-workforce\/latest\"><span style=\"font-weight: 400\">200 000 staff in the NHS<\/span><\/a><span style=\"font-weight: 400\"> (around a third of doctors and a fifth of nurses and midwives) are from BME backgrounds, and the NHS is the largest employer of<\/span><a href=\"https:\/\/www.opendemocracy.net\/ournhs\/roger-kline\/racism-in-nhs-don-t-let-unspeakable-become-acceptable\"><span style=\"font-weight: 400\">\u00a0BME staff in the UK<\/span><\/a><span style=\"font-weight: 400\">. These staff have been essential to the functioning of the <\/span><a href=\"http:\/\/www.historyandpolicy.org\/policy-papers\/papers\/immigration-and-the-national-health-service-putting-history-to-the-forefron\"><span style=\"font-weight: 400\">NHS since its inception<\/span><\/a><span style=\"font-weight: 400\">. Even now, with Brexit looming and increasingly difficult working conditions, the Department of Health and Social Care <\/span><span style=\"font-weight: 400\">is <\/span><a href=\"http:\/\/www.independent.co.uk\/news\/uk\/home-news\/nhs-recruitment-drive-foreign-doctors-overseas-gps-100-million-investment-hiring-staff-shortage-a7921511.html\"><span style=\"font-weight: 400\">recruiting all grades and specialties<\/span><\/a><span style=\"font-weight: 400\"> of doctors and nurses from outside of the EU. But how do we treat BME staff already working in the NHS?<\/span><\/p>\n<p><a href=\"https:\/\/www.england.nhs.uk\/wp-content\/uploads\/2014\/08\/edc7-0514.pdf\"><i><span style=\"font-weight: 400\">The snowy white peaks of the NHS<\/span><\/i><\/a><span style=\"font-weight: 400\">, a study by Roger Kline, examined BME progression in the health service in London and exposed the <\/span><span style=\"font-weight: 400\">lack of representation of BME staff in the upper echelons of the NHS<\/span><span style=\"font-weight: 400\">. <\/span><span style=\"font-weight: 400\">This was reflected nationally across other NHS bodies. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Don Berwick rightly <\/span><a href=\"http:\/\/tinyurl.com\/q6a7zkh\"><span style=\"font-weight: 400\">said that\u00a0<\/span><\/a><span style=\"font-weight: 400\">at its core, &#8220;the NHS remains a world leading example of commitment to health and healthcare as a human right\u2014the endeavour of a whole society to ensure that all people in their time of need are supported, cared for, and healed.\u201d <\/span><\/p>\n<p><span style=\"font-weight: 400\">The NHS demonstrates, more clearly than anything else, the positive contribution that staff<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">from all nationalities and all backgrounds<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">make to the national good.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Without staff from other countries, the health service would come to a standstill. BME staff have been essential to the NHS throughout its history, yet there&#8217;s still much to be done in terms of fairness and equality. As we remember Stephen Lawrence on the 25<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> anniversary of his death, we need to pledge that all arms of the NHS, and other medical bodies, take a hard look in the mirror and reflect on how transparency, fairness, and equality should frame the NHS.<\/span><\/p>\n<p><em><strong>Kailash Chand<\/strong> has been a GP for the past 30 years.
