{"id":41642,"date":"2018-03-15T13:15:17","date_gmt":"2018-03-15T12:15:17","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=41642"},"modified":"2018-03-20T16:26:00","modified_gmt":"2018-03-20T15:26:00","slug":"chris-simms-the-british-and-canadian-health-systems-at-70-and-60","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2018\/03\/15\/chris-simms-the-british-and-canadian-health-systems-at-70-and-60\/","title":{"rendered":"Chris Simms: The British and Canadian health systems at 70 and 60"},"content":{"rendered":"<p class=\"standfirst\">As both healthcare systems celebrate landmark years, they also share similar threats to their continued survival<\/p>\n<p><!--more--><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/11\/Chris_sims.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-32780\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/11\/Chris_sims-248x300.jpg\" alt=\"Chris_sims\" width=\"104\" height=\"125\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/11\/Chris_sims-248x300.jpg 248w, https:\/\/blogs.bmj.com\/bmj\/files\/2014\/11\/Chris_sims-849x1024.jpg 849w\" sizes=\"auto, (max-width: 104px) 100vw, 104px\" \/><\/a><span class=\"c1\">This July the British NHS and <a href=\"https:\/\/en.wikipedia.org\/wiki\/Hospital_Insurance_and_Diagnostic_Services_Act\">Canadian hospital insurance<\/a><\/span><span class=\"c1\">\u00a0systems<\/span><sup class=\"c1 c6\"><a id=\"ftnt_ref1\" href=\"#ftnt1\">[1]<\/a><\/sup><span class=\"c1\">\u00a0will celebrate their 70<\/span><span class=\"c1 c6\">th<\/span><span class=\"c1\">\u00a0and 60<\/span><span class=\"c1 c6\">th<\/span><span class=\"c1\">\u00a0anniversaries, respectively. Both healthcare systems\u2014like many in high income countries\u2014are in crisis, although neither is about to collapse. <\/span><\/p>\n<p class=\"c8\"><span class=\"c1\">Health systems<\/span><span class=\"c1 c10\">\u00a0<\/span><span class=\"c1\">are<\/span><span class=\"c1 c10\">\u00a0<\/span><span class=\"c2 c1\">known as complex adaptive systems (CASs) and this leaves them vulnerable to a whole host of weaknesses that can prey upon them. There is typically no single point of control within them, and the behaviour of groups and individuals belonging to these structures (including politicians, bureaucrats, providers, patients, and their communities) can be unpredictable, random, and chaotic. They tend to have their own values, rules, and objectives, and are often in conflict with one another\u2014sometimes without knowing precisely why or with whom. Normally CASs adapt and self-adjust to internal changes or outside pressures\u2014what the NHS select committee calls \u201cits daily struggles\u201d\u2014through so called \u201csingle loop learning.\u201d But when ongoing pressures accumulate or new ones threaten, simple corrective measures are no longer adequate. \u00a0 <\/span><\/p>\n<p class=\"c8\"><span class=\"c1\">T<\/span><span class=\"c1\">hese <\/span><span class=\"c1\">pressures<\/span><span class=\"c1\">\u00a0take the form of the very factors that drive health systems: a bulging demographic cohort (increasingly burdened by chronic disease, comorbidities, and fragility) attempting to pass through a structure rife with bottlenecks. <\/span><span class=\"c1\">The existence <\/span><span class=\"c1\">of these bottlenecks (at the input, throughput, and output stages) <\/span><span class=\"c1\">indicates<\/span><span class=\"c1\">\u00a0an underfunded, understaffed, and under-performing healthcare system<\/span><span class=\"c1\">, which is<\/span><span class=\"c1 c2\">\u00a0weakened by a highly polarised and divisive political environment. \u00a0<\/span><\/p>\n<p class=\"c8\"><span class=\"c1\">A common (and not unexpected) thread to both crises is the link<\/span><span class=\"c1\">s<\/span><span class=\"c1\">\u00a0between austerity, access to effective healthcare<\/span><span class=\"c1\">,<\/span><span class=\"c1\">\u00a0and outcomes. In Canada \u00a0for example, total health expenditure as a percentage of GDP ha<\/span><span class=\"c1\">s<\/span><span class=\"c1\">\u00a0fallen since <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/secure.cihi.ca\/free_products\/NHEX-Trends-Narrative-Report_2016_EN.pdf&amp;sa=D&amp;ust=1521116394629000&amp;usg=AFQjCNFdYPWA5E8YKQCwcBsArCQrpa9m7A\">2010<\/a><\/span><span class=\"c1\">, while <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.fraserinstitute.org\/article\/canadas-health-care-wait-times-hit-new-record-high-again\">waiting times<\/a><\/span><span class=\"c2 c1\">\u00a0have hit all time highs and performance indicators all time lows. The UK has witnessed similar trends. \u00a0<\/span><\/p>\n<p class=\"c8\"><span class=\"c1\">Another commonality is the failure to plan ahead and to rely instead on short term, reactive, ad hoc decisions. A recent NHS select committee <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/www.publications.parliament.uk\/pa\/ld201617\/ldselect\/ldnhssus\/151\/151.pdf&amp;sa=D&amp;ust=1521116394630000&amp;usg=AFQjCNGISlPqWJVGebxgHduZpFx6YuL3LQ\">report<\/a><\/span><span class=\"c1\">\u00a0concluded that a \u201cculture of short termism seems to prevail in the NHS and adult social care. The short-sightedness of successive governments is reflected in a Department of Health that is unable or unwilling to think beyond the next few years.\u201d For example, rather than embrace the recommendations of the <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/www.kingsfund.org.uk\/projects\/commission-future-health-and-social-care-england&amp;sa=D&amp;ust=1521116394631000&amp;usg=AFQjCNFZoKJcdUsA6fPiOa31A_zTvAhSiQ\">Barker Commission<\/a><\/span><span class=\"c1\">\u00a0(the antithesis of short termism in the long span of its review, which looked back to 1948 and forward through the rest of the 21st century), the UK government has let social care decline to the point that it is little more than a \u201c<\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/www.kingsfund.org.uk\/publications\/priorities-nhs-social-care-2017&amp;sa=D&amp;ust=1521116394631000&amp;usg=AFQjCNEeDukNg7hQso9C5TV2CNne-64mtA\">threadbare safety net<\/a><\/span><span class=\"c1\">.\u201d Elsewhere, seemingly r<\/span><span class=\"c2 c1 c5\">andom promises of \u00a3350 or \u00a3100 million injections into the NHS are revealing indicators of this short termism.<\/span><\/p>\n<p class=\"c8\"><span class=\"c1 c5\">In Canada where there is no national health plan and services are delivered by the provinces and territories, short termism is often the hallmark of cash strapped governments<\/span><span class=\"c1\">\u2014<\/span><span class=\"c1 c5\">a reality exemplified in <\/span><span class=\"c0 c5\"><a class=\"c3\" href=\"https:\/\/globalnews.ca\/news\/3874371\/nova-scotia-doing-a-poor-job-explaining-its-family-doctor-plans-auditor-general\/\">workforce planning,<\/a><\/span><span class=\"c1 c5\">\u00a0or rather the lack of it<\/span><span class=\"c1 c5\">. <\/span><span class=\"c1\">Increased demand, staff shortages, and overwork manifest in physicians leaving the profession or retiring early, low morale, and very high levels of <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/www.facebook.com\/notes\/we-are-your-ontario-doctors\/doctors-burning-out-in-a-failing-healthcare-system\/835170386617069&amp;sa=D&amp;ust=1521116394633000&amp;usg=AFQjCNHEqJqo9kTAvLzbWP67uEJwr55kOA\">burnout<\/a><\/span><span class=\"c1\">. GPs\u2019 feelings of being undervalued and not respected are worsened by <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/torontolife.com\/city\/toronto-politics\/health-minister-eric-hoskins-doctors-revolt\/&amp;sa=D&amp;ust=1521116394633000&amp;usg=AFQjCNGuD2m868aLeSuKNWTEhn4z0L7NdA\">deep inequalities<\/a><\/span><span class=\"c1\">, which see some specialists (such as radiologists and ophthalmologists) earn salaries five times greater than GPs. <\/span><span class=\"c2 c1 c5\">The NHS is similarly plagued by the linked phenomena of low morale and burnout, and is struggling to hold onto its workforce.<\/span><\/p>\n<p class=\"c8\"><span class=\"c1\">Too often the complexity of these health systems is met with ill conceived efforts to improve them, which take the form of centralisation of structures or concentration of power. In the UK this was seen in the unfolding of the 2012 Health and Social Care Act, which was put together by a <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/www.kingsfund.org.uk\/sites\/default\/files\/field\/field_publication_file\/never-again-story-health-social-care-nicholas-timmins-jul12.pdf&amp;sa=D&amp;ust=1521116394634000&amp;usg=AFQjCNEtmWWuVO2FIEJLhbri3O35D9xEVA\">handful of politicians<\/a><\/span><span class=\"c1\">\u00a0in 12 days, and which ultimately breached the trust between the medical profession and politicians. Over the past <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=http:\/\/www.bmj.com\/content\/356\/bmj.j1026&amp;sa=D&amp;ust=1521116394635000&amp;usg=AFQjCNH6y-21Jj91MoTSLs5mSoCXLL2RUA\">four years,<\/a><\/span><span class=\"c2 c1\">\u00a0the government has played a blame game with the NHS crisis, alternately pointing the finger at NHS management, patients, and GPs and hospital doctors. <\/span><\/p>\n<p id=\"h.gjdgxs\" class=\"c8\"><span class=\"c1\">In Canada, an almost identical situation exists. Governments have sought to deal with complexity and the reality that there\u2019s \u201c<\/span><span class=\"c1\">no single point of control\u201d<\/span><span class=\"c1 c10\">\u00a0<\/span><span class=\"c1\">by <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/globalnews.ca\/news\/1889639\/seven-years-after-centralizing-health-care-alberta-decentralizes-system\/&amp;sa=D&amp;ust=1521116394636000&amp;usg=AFQjCNEeyAjef2szaRWhenpNmiLoHrIJlA\">alternately<\/a><\/span><span class=\"c1\">\u00a0<\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/globalnews.ca\/news\/3896216\/saskatchewan-health-authority-launches\/&amp;sa=D&amp;ust=1521116394636000&amp;usg=AFQjCNG4muk6fxZPQWSjP7UvaZCzQcl2-w\">centralising<\/a><\/span><span class=\"c1\">\u00a0and decentralising services, assembling or <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=https:\/\/www.winnipegfreepress.com\/local\/province-to-centralize-health-care-services-to-new-govt-entity-431414333.html&amp;sa=D&amp;ust=1521116394636000&amp;usg=AFQjCNGIL1YdztTI1DzhJcZXEnqddGuj4g\">dismantling<\/a><\/span><span class=\"c2 c1\">\u00a0local health authorities, all the while bemoaning the bureaucracy, professional associations, and irresponsible patients. <\/span><\/p>\n<p id=\"h.30j0zll\" class=\"c8\"><span class=\"c1\">The paradox is that this July both the NHS and Canadian Medicare have a number of reasons to celebrate their successes. Medicare is still seen to embody Canadian values and the NHS remains a \u201cnational religion.\u201d Cross country <\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=http:\/\/www.commonwealthfund.org\/interactives\/2017\/july\/mirror-mirror\/&amp;sa=D&amp;ust=1521116394637000&amp;usg=AFQjCNH9m1bUaKLZIM7eYiqeUcajnX3nQw\">data<\/a><\/span><span class=\"c2 c1\">\u00a0also show that for most indicators the UK performs well against its peers. <\/span><\/p>\n<p id=\"h.3znysh7\" class=\"c8\"><span class=\"c1\">However, neither the NHS nor Medicare<\/span><span class=\"c1 c10\">\u00a0<\/span><span class=\"c1\">is<\/span><span class=\"c1 c10\">\u00a0<\/span><span class=\"c1\">sustainable. Instead of being stuck on the endless loops of detecting and correcting errors, both healthcare services need a systems approach (<\/span><span class=\"c0\"><a class=\"c3\" href=\"https:\/\/www.google.com\/url?q=http:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/lrh2.10024\/full&amp;sa=D&amp;ust=1521116394638000&amp;usg=AFQjCNEel9fhZ9wa_WrSfURhrcey7pE0ZA\">double-loop<\/a><\/span><span class=\"c2 c1\">\u00a0learning), which occurs when the error is \u201cdetected and corrected in ways that involve the modification of an organisation\u2019s underlying norms, policies, and objectives.\u201d <\/span><\/p>\n<p id=\"h.tyjcwt\" class=\"c8\"><span class=\"c1\">The idea of taking a systems approach <\/span><span class=\"c1\">to these healthcare services <\/span><span class=\"c1\">is not new, yet <\/span><span class=\"c1\">its implementation has so far failed to materialise, perhaps falling at the hurdles of\u00a0<\/span><span class=\"c2 c1\">tackling difficult questions and confronting powerful stakeholders. It takes discipline to avoid knee jerk reactions to a crisis\u2014the temptation is to concentrate power or eschew responsibility.
