{"id":40908,"date":"2017-12-18T18:13:26","date_gmt":"2017-12-18T17:13:26","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=40908"},"modified":"2017-12-20T10:43:07","modified_gmt":"2017-12-20T09:43:07","slug":"karen-b-born-et-al-learning-how-to-choose-wisely","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2017\/12\/18\/karen-b-born-et-al-learning-how-to-choose-wisely\/","title":{"rendered":"Karen B Born et al: Learning how to choose wisely"},"content":{"rendered":"<p><span style=\"font-weight: 400\">Across the<\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2017\/04\/07\/choosing-wisely-around-the-world-professionalism-as-a-force-to-reduce-unnecessary-care\/\"><span style=\"font-weight: 400\">\u00a0countries with Choosing Wisely campaigns<\/span><\/a><span style=\"font-weight: 400\">, thousands of recommendations of &#8220;Things clinicians and patients should question&#8221; have been published. These lists and recommendations, familiar to many practising clinicians, outline tests, treatments, or procedures that evidence suggests are overused, unnecessary and may cause harm to patients. <\/span><a href=\"http:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2457401\"><span style=\"font-weight: 400\">As some have pointed out,<\/span><\/a><span style=\"font-weight: 400\"> implementing these recommendations into practice can be <\/span><a href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp1516803#t=article\"><span style=\"font-weight: 400\">challenging<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"http:\/\/www.bmj.com\/content\/351\/bmj.h6760\"><span style=\"font-weight: 400\">slow<\/span><\/a><span style=\"font-weight: 400\">. <\/span><\/p>\n<p><span style=\"font-weight: 400\">However, there are a new cluster of recommendations which are being published, not by national clinical specialty societies, but by national medical student associations. These lists of recommendations do not focus on specific tests, treatments, and procedures that are overused, but rather on the training environment and culture that foster overuse. Medical student leaders are sharing the same message that reducing overuse begins in training. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Choosing Wisely lists for medical students and trainees create opportunities for students to initiate conversations about unnecessary tests, treatments, and procedures. The longstanding tradition of generating a lengthy differential diagnosis as a way of demonstrating medical knowledge has led to students incorrectly assuming that the best way to impress their faculty is to order an exhaustive list of investigations. However, the <\/span><a href=\"http:\/\/journals.lww.com\/academicmedicine\/Citation\/2016\/10000\/Choosing_Wisely_for_Medical_Education___Six_Things.19.aspx\"><span style=\"font-weight: 400\">Choosing Wisely medical education lists<\/span><\/a><span style=\"font-weight: 400\"> encourage a more mindful approach, and urge students to consider displaying their knowledge by demonstrating restraint, thinking twice or asking the question &#8220;will the result of this test actually change how I manage this patient\u2019s condition?&#8221; The lists represent student leadership in driving a <\/span><a href=\"http:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/1367571\"><span style=\"font-weight: 400\">shift in medical education<\/span><\/a><span style=\"font-weight: 400\">, where thoroughness is appropriately balanced with thoughtfulness. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The Canadian Federation of Medical Students (CFMS) and the F\u00e9d\u00e9ration m\u00e9dicale \u00e9tudiante du Qu\u00e9bec (FMEQ) who together represent Canadian medical students surveyed nearly 2000 students to develop the <\/span><a href=\"https:\/\/choosingwiselycanada.org\/students\/\"><span style=\"font-weight: 400\">first Choosing Wisely medical education list<\/span><\/a><span style=\"font-weight: 400\">. The Resident Doctors of Canada (RDoC) representing postgraduate medical trainees also released a <\/span><a href=\"https:\/\/choosingwiselycanada.org\/residents\/\"><span style=\"font-weight: 400\">Choosing Wisely list for residents and trainees<\/span><\/a><span style=\"font-weight: 400\">. Similar initiatives have followed in other countries. For example, the <\/span><a href=\"http:\/\/choosingwisely.org.nz\/professional-resource\/nzmsa\/\"><span style=\"font-weight: 400\">New Zealand Medical Students Association recently<\/span><\/a><span style=\"font-weight: 400\"> released a list of six recommendations as part of the country\u2019s Choosing Wisely campaign. Accompanying the list is a mnemonic, <a href=\"http:\/\/choosingwisely.org.nz\/professional-resource\/nzmsa\/\">WISE<\/a>, which helps students to recall these &#8220;key concepts&#8221; in medical education, suggesting that prior to ordering tests, treatments, or procedures, students should be reflexive. <\/span><\/p>\n<ul>\n<li><span style=\"font-weight: 400\">Why? What will this test, treatment or procedure change?<\/span><\/li>\n<li><span style=\"font-weight: 400\">Is there an alternative? Less invasive, less resource intensive?<\/span><\/li>\n<li><span style=\"font-weight: 400\">Seek clarification. Clarify why the doctor ordered this test <\/span><\/li>\n<li><span style=\"font-weight: 400\">Explore\/explain. Be the patient\u2019s advocate. Explore concerns, take time to explain why a test, treatment or procedure is\/isn\u2019t necessary<\/span><\/li>\n<\/ul>\n<p><a href=\"http:\/\/choosingwisely.jp\/\"><span style=\"font-weight: 400\">Choosing Wisely Japan<\/span><\/a><span style=\"font-weight: 400\"> launched in October 2016. There is an active group of student leaders engaged in medical education from 19 of the country\u2019s 82 medical schools. The &#8220;<\/span><a href=\"http:\/\/onlinelibrary.wiley.com\/doi\/10.1002\/jgf2.96\/pdf\"><span style=\"font-weight: 400\">Choosing Wisely Japan Student Committee&#8221;<\/span><\/a><span style=\"font-weight: 400\"> has advocated for student leadership in the campaign in order to influence the clinical training environment, public engagement in the campaign, and their own future practice as physicians. The student committee has also developed a Choosing Wisely list, which encourages students to think twice, employ critical appraisal when ordering tests, treatments or procedures, and to not miss opportunities for conversations with patients or clinical supervisors. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Beyond list development, students are also demonstrating leadership at their medical schools and promoting resource stewardship education. Choosing Wisely Canada launched <\/span><a href=\"https:\/\/uottawa.scholarsportal.info\/ojs\/index.php\/uojm-jmuo\/article\/view\/1818\/1784\"><span style=\"font-weight: 400\">STARS (Students and Trainees Advocating for Resource Stewardship)<\/span><\/a><span style=\"font-weight: 400\"> in November 2015 to address a gap in undergraduate curricula with respect to resource-stewardship education across the country\u2019s 17 medical schools. Grassroots, student-led activism has advocated for more inclusion of Choosing Wisely recommendations or training on resource stewardship in local curricula so that students are exposed to these concepts prior to their clinical rotations. \u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">As we reach the end of this year, over 150 students across these 3 countries will call themselves STARS. This nascent group of student leaders are just beginning to implement efforts to influence medical training and culture, and recruiting their fellow students to join in their cause.
