Kohl was used as a cosmetic for blackening the eyelids, and the habit is even mentioned in the Bible: \u201ckakhalt ainayikh\u201d, you painted your eyes (Ezekiel 23:40), from the protoSemitic root kx\u0323l, powdered antimony. The Greek word for any salve, and particularly eye salve, was \u03ba\u03bf\u03bb\u03bb\u03bf\u1f7b\u03c1\u03b9\u03bf\u03bd (incidentally a dimunitive of the word for a pessary or suppository), in English collyrium. This may have been connected with kohl, in Greek \u03c0\u03bb\u03b1\u03c4\u03c5\u03cc\u03d5\u03b8\u03b1\u03bb\u03bc\u03bf\u03bd (literally \u201cwide-eye\u201d) or \u03c3\u03c4\u1f77\u03b2\u03b9 or \u03c3\u03c4\u1f77\u03bc\u03bc\u03b9, which is the same as the Latin stibium, again powdered antimony. Antimonious ores were sometimes confused with lead ores, and alquifou was the name of a Cornish lead ore that looked like antimony, used by potters to give a green glaze to earthenware.<\/span><\/p>\n<p><span style=\"font-weight: 400\">By extension, any fine powder produced by sublimation became known as al-koh\u2019l, and the idea was further extended to fluids produced by sublimation or distillation, as in what Paracelsus called \u201calcool vini\u201d, \u201calcohol of wine\u201d, or just alcohol for short. When alcohol was characterised chemically and named ethyl alcohol (ethanol), the term was applied to similar compounds in the series, such as methyl alcohol (methanol) and propyl alcohol (propanol).<\/span><\/p>\n<p><span style=\"font-weight: 400\">It is useful to know a little about the pharmacokinetics of alcohol. It is well absorbed throughout the gut, including the stomach. The rate of absorption depends on the concentration, with a maximum at about 15%, and is delayed by food. So if you want to get drunk quickly, take your gin with tonic or your whisky with Highland spring water (but not if it\u2019s a high class malt\u2014that would be sacrilege) and do it on an empty stomach. Alcohol is metabolised by alcohol dehydrogenase to aldehyde, which is metabolised to CO<sub>2<\/sub> and water by aldehyde dehydrogenase. Women have less alcohol dehydrogenase in their stomachs than men, and so more alcohol enters the systemic circulation. They also have a lower apparent volume of distribution, so their blood alcohol concentration is higher at any dose. The consequences of these differences are obvious. Elimination is slow, about 6 g\/h, and you can still be over the legal limit for driving the next morning after a heavy night. To do the calculations, use the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1701796\/pdf\/brmedj02306-0067.pdf\"><span style=\"font-weight: 400\">nomogram<\/span><\/a><span style=\"font-weight: 400\"> below, but remember that wines are stronger these days.\u00a0Alcohol is distributed in body water, which has a volume of about 40 L\/70 kg, so to calculate the approximate blood alcohol concentration in mg\/dL (mg%) divide the amount of alcohol in the body in grams by 0.4.<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-40840 alignnone\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol.png\" alt=\"\" width=\"671\" height=\"672\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol.png 671w, https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol-150x150.png 150w, https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol-300x300.png 300w\" sizes=\"auto, (max-width: 671px) 100vw, 671px\" \/><br \/>\n<span style=\"font-weight: 400\">Inhibit alcohol dehydrogenase with chloral hydrate say, and you have a <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11614798\"><span style=\"font-weight: 400\">Mickey Finn<\/span><\/a><span style=\"font-weight: 400\">. Inhibit aldehyde dehydrogenase with disulfiram or metronidazole, for example, and you get a very unpleasant <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24741856\"><span style=\"font-weight: 400\">reaction<\/span><\/a><span style=\"font-weight: 400\"> due to aldehyde, with flushing, headache, nausea, vomiting, sweating, vertigo, chest pain, hypoventilation, and in severe cases cardiac arrhythmias, myocardial infarction, congestive heart failure, and seizures.<\/span><\/p>\n<p><span style=\"font-weight: 400\">You don\u2019t necessarily have to get your alcohol from a bottle; some people can produce it themselves. In the <\/span><a href=\"https:\/\/file.scirp.org\/pdf\/IJCM_2013070411021392.pdf\"><span style=\"font-weight: 400\">gut fermentation syndrome<\/span><\/a><span style=\"font-weight: 400\">, also known as endogenous ethanol fermentation, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/1027959\"><span style=\"font-weight: 400\">autobrewery syndrome<\/span><\/a><span style=\"font-weight: 400\">, and plain drunkenness disease, gut organisms ferment ingested carbohydrate to alcohol. In 1931, Hurst &amp; Knott <\/span><a href=\"https:\/\/academic.oup.com\/qjmed\/article-abstract\/os-24\/94\/171\/1567383?redirectedFrom=fulltext\"><span style=\"font-weight: 400\">described<\/span><\/a><span style=\"font-weight: 400\"> what they called \u201cintestinal carbohydrate dyspepsia\u201d (figure), attributed to fermentation of dietary carbohydrate. Then, in <em>The BMJ<\/em> in <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18911055\"><span style=\"font-weight: 400\">1948<\/span><\/a><span style=\"font-weight: 400\"> Ladkin and Davies described an African child whose stomach ruptured because of distension by alcohol due to fermentation of sweet potatoes by a mixture of Gram negative cocci and bacilli, although they did not give the syndrome a name. The terms \u201cdrunken syndromes\u201d and \u201cauto-brewery syndrome\u201d <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/1027959\"><span style=\"font-weight: 400\">emerged<\/span><\/a><span style=\"font-weight: 400\"> in the 1970s.<\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-40841 size-full\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol2.png\" alt=\"\" width=\"531\" height=\"559\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol2.png 531w, https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol2-285x300.png 285w, https:\/\/blogs.bmj.com\/bmj\/files\/2017\/12\/aronson_alcohol2-300x316.png 300w\" sizes=\"auto, (max-width: 531px) 100vw, 531px\" \/><\/p>\n<p><span style=\"font-weight: 400\">Most cases have been attributed to fermentation of dietary carbohydrates by yeasts, such as <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16720504\"><i><span style=\"font-weight: 400\">Candida kefyr<\/span><\/i><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/6520589\"><i><span style=\"font-weight: 400\">Candida albicans <\/span><\/i><span style=\"font-weight: 400\">and<\/span><i><span style=\"font-weight: 400\"> Candida krusei<\/span><\/i><\/a><span style=\"font-weight: 400\">, and <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11568528\"><i><span style=\"font-weight: 400\">Candida glabrata<\/span><\/i><span style=\"font-weight: 400\"> and <\/span><i><span style=\"font-weight: 400\">Saccharomyces cerevisiae<\/span><\/i><\/a><span style=\"font-weight: 400\">. The syndrome has been reported after surgery for <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/27161390\"><span style=\"font-weight: 400\">Crohn\u2019s disease<\/span><\/a><span style=\"font-weight: 400\"> and in children with <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11568528\"><span style=\"font-weight: 400\">short bowel syndrome<\/span><\/a><span style=\"font-weight: 400\">. It has been <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/3997687\"><span style=\"font-weight: 400\">suggested<\/span><\/a><span style=\"font-weight: 400\"> that it might be a cause of sudden infant death syndrome and it has also been <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10976182\"><span style=\"font-weight: 400\">used<\/span><\/a><span style=\"font-weight: 400\"> as a defence in cases of drunk driving. It may respond to a low carbohydrate diet, use of antifungal drugs, and avoidance of antibacterial drugs.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Fermentation of carbohydrates to alcohol may also occur in the absence of gut pathology, for example, after eating <\/span><a href=\"http:\/\/www.tandfonline.com\/doi\/abs\/10.3109\/13590849009003132\"><span style=\"font-weight: 400\">chocolates<\/span><\/a><span style=\"font-weight: 400\">, although the resultant blood alcohol concentrations are generally below 20 mg\/dL (4.35 mmol\/L). This gives a new meaning to the word \u201c<\/span><a href=\"http:\/\/www.bmj.com\/content\/312\/7027\/373\"><span style=\"font-weight: 400\">chocaholic<\/span><\/a><span style=\"font-weight: 400\">\u201d.<\/span><\/p>\n<p><em><strong>Jeffrey Aronson<\/strong>\u00a0is a clinical pharmacologist, working in the Centre for Evidence Based Medicine in Oxford&#8217;s Nuffield Department of Primary Care Health Sciences.
