{"id":40639,"date":"2017-11-15T16:54:59","date_gmt":"2017-11-15T15:54:59","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=40639"},"modified":"2017-11-27T11:36:14","modified_gmt":"2017-11-27T10:36:14","slug":"grania-brigden-the-first-step-on-a-long-road-to-tb-elimination","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2017\/11\/15\/grania-brigden-the-first-step-on-a-long-road-to-tb-elimination\/","title":{"rendered":"Grania Brigden: The first step on a long road to TB elimination"},"content":{"rendered":"<p class=\"standfirst\">The first step in eradicating TB is eliminating the funding gaps<\/p>\n<p><!--more--><br \/>\n<img loading=\"lazy\" decoding=\"async\" class=\" wp-image-38785 alignleft\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/03\/grania_brigden_2017.png\" alt=\"\" width=\"158\" height=\"173\" \/><span style=\"font-weight: 400\">There has been a recent flurry of reports on tuberculosis (TB) ahead of this week\u2019s <\/span><a href=\"http:\/\/www.who.int\/conferences\/tb-global-ministerial-conference\/en\/\"><span style=\"font-weight: 400\">Global Ministerial Conference on Ending TB in the Sustainable Development Era\u00a0<\/span><\/a><span style=\"font-weight: 400\">in Moscow. For the second year in a row, the World Health Organization\u2019s <\/span><a href=\"http:\/\/www.who.int\/tb\/publications\/global_report\/en\/\"><span style=\"font-weight: 400\">Global TB report<\/span><\/a><span style=\"font-weight: 400\"> showed that TB remains the leading infectious disease killer. While the <\/span><a href=\"http:\/\/www.stoptb.org\/assets\/documents\/resources\/publications\/acsm\/909090_PDF_LR.pdf\"><span style=\"font-weight: 400\">Stop TB Partnership progress report<\/span><\/a><span style=\"font-weight: 400\"> on the Global Plan to End TB 2016\u20132020 shows that the paradigm shift required to reach the WHO END TB strategy and the Sustainable Development Goal TB targets for 2030 is still not occurring. <\/span><\/p>\n<p><span style=\"font-weight: 400\">When these targets were made in 2015, they required the tools we had then to be implemented and scaled up. Yet they were also dependent on the necessary investment in research and development (R&amp;D) taking place, so that we&#8217;d have the new tools we need to change the narrative in TB from \u201ccontrol\u201d to a more ambitious \u201cending\u201d or eradication. This investment, like the paradigm shift in the TB approach, has not happened. The Treatment Action Group\u2019s latest <\/span><a href=\"http:\/\/www.treatmentactiongroup.org\/tbrd2017\"><span style=\"font-weight: 400\">R&amp;D funding report<\/span><\/a><span style=\"font-weight: 400\"> shows that despite a small increase in the total TB R&amp;D investment, it still remains only a third of the $2 billion a year required. The small print shows that the investment of private and pharmaceutical companies in TB R&amp;D was at its lowest level since 2009.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Should we surprised by this? <\/span><\/p>\n<p><span style=\"font-weight: 400\">The market&#8217;s failure to develop new\u00a0<\/span><a href=\"http:\/\/apps.who.int\/iris\/bitstream\/10665\/193736\/1\/9789241509763_eng.pdf?ua=1\"><span style=\"font-weight: 400\">antibiotics has been highlighted<\/span><\/a>,\u00a0<span style=\"font-weight: 400\">and TB antibiotics are no different. In fact, since we don\u2019t need a new drug to cure TB\u2014rather, we need new combinations of new drugs\u2014the challenges, barriers, and costs for developing new TB treatments are higher, making it an unattractive proposition for the private sector. This need for new combinations to treat TB means that there needs to be a bespoke solution for TB, which will overcome the market&#8217;s failure.<\/span><\/p>\n<p><a href=\"http:\/\/www.thelifeprize.org\"><span style=\"font-weight: 400\">The Life Prize<\/span><\/a><span style=\"font-weight: 400\"> (formerly the 3P Project) is a response to the unique challenges in TB R&amp;D and aims to create a new way of rewarding investment in TB drug development. It will create an open, collaborative research framework to promote the development of new regimens, with stewardship, quality, accessibility, and affordability key concerns that will be considered throughout the development pipeline. The Life Prize is an enabling framework that is open to all actors\u2014from private and pharmaceutical companies to academic and research groups. It promotes the collaboration and partnerships we need to develop the treatments of the future and ensure their availability to all. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Innovative ideas like the Life Prize, <\/span><a href=\"http:\/\/endtbrussia.org\/brics\"><span style=\"font-weight: 400\">the BRICS TB Research Network,<\/span><\/a><span style=\"font-weight: 400\"> and the <\/span><a href=\"http:\/\/bmi.icmr.org.in\/itrc\/index.php\"><span style=\"font-weight: 400\">India TB Research Consortium<\/span><\/a><span style=\"font-weight: 400\"> are looking at the problem of TB in a new way, and building collaborative networks and partnerships to develop the new tools of the future. <\/span><\/p>\n<p><span style=\"font-weight: 400\">The focus on R&amp;D at this week\u2019s Ministerial Conference in Moscow builds on recent declarations at <\/span><a href=\"http:\/\/www.stoptb.org\/news\/stories\/2017\/ns17_044.asp\"><span style=\"font-weight: 400\">the G20<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/www.brics2017.org\/English\/Documents\/Summit\/201709\/t20170908_2021.html\">BRICS<\/a> summit<\/span><span style=\"font-weight: 400\">s, which highlighted the importance of R&amp;D for developing new tools. And <a href=\"http:\/\/www.who.int\/tb\/features_archive\/Moscow_Declaration_to_End_TB_final_draft_ENGLISH.pdf?ua=1\">the declaration from Moscow<\/a> is an important next step on a long road to the elimination of TB. <\/span><\/p>\n<p><span style=\"font-weight: 400\">It is vital that discussions move beyond framing the problem to implementing the solutions. The collaboration and innovative ideas taking place in the field of TB must be matched by the political and financial commitment of countries and donors. It is not acceptable that a curable infectious disease is killing 1.3 million people every year. It is not acceptable that TB can be allowed to cause a quarter of all antimicrobial resistance related deaths in 2050, as is projected. And it is not acceptable that the current R&amp;D investment continues to stay at a third of what is required. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Elimination is the aim for TB. Countries at next year\u2019s UN High Level Meeting on TB have an opportunity to build on the progress made in Moscow and commit to the first step of eliminating TB: eliminating the funding gaps for TB. \u00a0<\/span><\/p>\n<p><strong><em>Grania Brigden<\/em><\/strong><em>\u00a0is the Life Prize project lead with The International Union Against TB and Lung Disease (The Union).
