The adjective <\/span><span id=\"E58\" style=\"font-size: 1rem\">tectonic<\/span><span id=\"E59\" style=\"font-size: 1rem\"> is older than modern geology; it comes from the Greek, meaning pertaining to building or construction in general (OED). We are familiar with it for its application to the tectonic plates underlying the oceans and continents, which, driven by deep currents in the mantle, cause earthquakes and volcanoes. The idea of <\/span><span id=\"E60\" style=\"font-size: 1rem\">these <\/span><span id=\"E61\" style=\"font-size: 1rem\">mobile <\/span><span id=\"E62\" style=\"font-size: 1rem\">tectonic plates developed during the 1960s, although the term <\/span><span id=\"E63\" style=\"font-size: 1rem\">tectonics<\/span><span id=\"E64\" style=\"font-size: 1rem\"> had already entered geology, referring simply to the structure of the Earth\u2019s crust or changes affecting it. <\/span><span id=\"E65\" style=\"font-size: 1rem\">Paradoxically,<\/span><span id=\"E66\" style=\"font-size: 1rem\"> a <\/span><span id=\"E67\" style=\"font-size: 1rem\">tectonic shift<\/span><span id=\"E68\" style=\"font-size: 1rem\"> occurs very slowly, but the subsequent events can be catastrophically sudden.<\/span><\/p>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<p id=\"E70\"><span id=\"E71\">The geological term has been borrowed by medicine in ophthalmology: it is a form of corneal surgery. And<\/span><span id=\"E72\"> also<\/span><span id=\"E73\">, as very many terms have been, in genetics, where Tectonic is a protein in the Hedgehog-mediated patterning of the neural tube. Anyone searching for metaphorical uses will have to wade through a large number of these uses, as well as geol<\/span><span id=\"E74\">ogical and ecological articles. Most metaphor<\/span><span id=\"E75\">ical uses ignore<\/span><span id=\"E76\"> the slow process and the catastrophic result, and mere<\/span><span id=\"E77\">ly mean<\/span><span id=\"E78\"> very big. There is often an overtone of difficulty, although there is nothing in the dictionary definition (COD: a very significant or considerable change or development) <\/span><span id=\"E79\">to imply that<\/span><span id=\"E80\">. <\/span><span id=\"E81\">Thus, i<\/span><span id=\"E82\">n its occasional application in medicine, the slowness is usually ignored; the <\/span><span id=\"E83\">tectonic shift<\/span><span id=\"E84\"> is just, like the <\/span><span id=\"E85\">quantum leap<\/span><span id=\"E86\"> and the <\/span><span id=\"E87\">big bang<\/span><span id=\"E88\">, something from which authors want you to infer that something terribly important <\/span><span id=\"E89\">is<\/span><span id=\"E90\"> happen<\/span><span id=\"E91\">ing<\/span><span id=\"E92\">.<\/span><\/p>\n<p id=\"E94\"><span id=\"E95\">Here are three examples: \u201c<\/span><span id=\"E96\">The advent of epigenetics brought in a tectonic shift in the understanding of molecular basis of complex diseases<\/span><span id=\"E97\">\u201d; \u201c<\/span><span id=\"E98\">our understanding of the intricacies of gene regulation has undergone tect<\/span><span id=\"E99\">onic shifts almost every decade\u201d; \u201c\u2026<\/span><span id=\"E100\">has resulted in a tectonic shift in the way physicians and the general population perc<\/span><span id=\"E101\">eive infertility and ethics\u201d. Is <\/span><span id=\"E102\">tectonic shift<\/span><span id=\"E103\"> any better than<\/span><span id=\"E104\"> <\/span><span id=\"E105\">\u201cgreat change\u201d or \u201cimportant advance\u201d? <\/span><\/p>\n<p id=\"E107\"><span id=\"E108\">But sometimes <\/span><span id=\"E109\">the<\/span><span id=\"E110\"> metaphor is necessary. Rudolf Klein <\/span><span id=\"E111\">used it well when he <\/span><span id=\"E112\">wrote about how to reform <\/span><a id=\"E113\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12511387\" target=\"_blank\" rel=\"noopener\"><span id=\"E114\" class=\"qowt-stl-Hyperlink\">US healthcare<\/span><\/a><span id=\"E115\">:<\/span><span id=\"E116\"> <\/span><span id=\"E117\">his<\/span><span id=\"E118\"> abstract <\/span><span id=\"E119\">ends<\/span><span id=\"E120\">, \u201c<\/span><span id=\"E121\">Importing systems from countries with different histories and institutions would require a tectonic shift in the American political landscape<\/span><span id=\"E122\">\u201d<\/span><span id=\"E123\">;<\/span><span id=\"E124\"> and <\/span><span id=\"E125\">his<\/span><span id=\"E126\"> article <\/span><span id=\"E127\">ends <\/span><span id=\"E128\">with, \u201c<\/span><span id=\"E129\">For the United States, the lessons learned are therefore likely to depend on whether increasing inequality, linked to increasing economic turbulence, produces a tectonic shift in the country\u2019s political scenery<\/span><span id=\"E130\">\u201d. <\/span><span id=\"E131\">Yes, a tectonic shift: big forces, consummate effort, unshakeable resolve.<\/span><\/p>\n<p id=\"E133\"><span id=\"E134\">I quite liked a <\/span><a id=\"E135\" href=\"http:\/\/journals.sagepub.com\/doi\/10.1177\/0141076816643332\" target=\"_blank\" rel=\"noopener\"><span id=\"E136\" class=\"qowt-stl-Hyperlink\">description<\/span><\/a><span id=\"E137\"> of changes to the Declaration of Helsinki as significant but not tectonic<\/span><span id=\"E138\">, although it implied to me that the authors were\u2014to use a word the OED labels as <\/span><span id=\"E139\">humorous<\/span><span id=\"E140\">\u2014underwhelmed. However, I have no idea what the authors\u00a0<\/span><span style=\"font-size: 1rem\">of an article on training the brain meant when they wrote that their paper provided \u201c<\/span><span id=\"E141\" style=\"font-size: 1rem\">a tectonic integration and synthesis of cognitive training approaches.<\/span><span id=\"E142\" style=\"font-size: 1rem\">\u201d<\/span><\/p>\n<\/div>\n<\/div>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<p><em><strong>Neville Goodman<\/strong>\u00a0is a retired consultant anaesthetist and a writer.
