{"id":40405,"date":"2017-10-17T11:27:44","date_gmt":"2017-10-17T10:27:44","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=40405"},"modified":"2017-10-24T10:37:46","modified_gmt":"2017-10-24T09:37:46","slug":"niall-dickson-the-cqcs-annual-report-should-cause-everyone-to-sit-up-and-listen","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2017\/10\/17\/niall-dickson-the-cqcs-annual-report-should-cause-everyone-to-sit-up-and-listen\/","title":{"rendered":"Niall Dickson: The CQC&#8217;s annual report should cause everyone to sit up and listen"},"content":{"rendered":"<p class=\"standfirst\">The CQC&#8217;s\u00a0annual <em>State of Care<\/em> report for 2017<span id=\"E89\">\u00a0says <\/span><span id=\"E90\">future NHS quality is precarious<\/span>\u2014but is anyone listening?<\/p>\n<p><!--more--><\/p>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<p id=\"E43\"><span id=\"E55\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-40409\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/10\/niall_dickson2.jpg\" alt=\"\" width=\"160\" height=\"160\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/10\/niall_dickson2.jpg 160w, https:\/\/blogs.bmj.com\/bmj\/files\/2017\/10\/niall_dickson2-150x150.jpg 150w\" sizes=\"auto, (max-width: 160px) 100vw, 160px\" \/>It is worth remembering that the CQC has the strong backing of the government. Even in its darkest days ministers decided reform not abolition was the right answer. In a sense the CQC is much more this Secretary of State\u2019s creation than say NHS England, which was a direct product of the Lansley reforms.<\/span><\/p>\n<p id=\"E56\"><span id=\"E57\">That does not mean the CQC is under direct government control\u2014it has its own statutory powers and up to a point can say and do its own thing. But the fact that its carefully crafted and worded annual report on the state of care in England described the system as precarious should cause everyone to seek a moment\u2019s reflection.<\/span><\/p>\n<p id=\"E58\"><span id=\"E59\"><a href=\"http:\/\/www.bmj.com\/content\/359\/bmj.j4672\">The report is deliberately not alarmist<\/a>. <\/span><span id=\"E60\">Indeed,<\/span><span id=\"E61\"> it argues that<\/span><span id=\"E62\"> the quality of care has been maintained<\/span><span id=\"E63\">, <\/span><span id=\"E64\">and claims <\/span><span id=\"E65\">that m<\/span><span id=\"E66\">ost of us are receiving good<\/span><span id=\"E67\"> and<\/span><span id=\"E68\"> safe care<\/span><span id=\"E69\">. In a nod towards its own role it points out that <\/span><span id=\"E70\">many services previously rated <\/span><span id=\"E71\">as <\/span><span id=\"E72\">inadequate have improved.<\/span><span id=\"E73\"> While true, it is worth noting that some services previously rated as good have slipped back.\u00a0<\/span><span id=\"E74\">Twenty-six per cent o<\/span><span id=\"E75\">f mental health services <\/span><span id=\"E76\">ori<\/span><span id=\"E77\">ginally rated &#8220;<\/span><span id=\"E79\">good&#8221;<\/span><span id=\"E81\">\u00a0<\/span><span id=\"E82\">had <\/span><span id=\"E83\">dropped at least one rating, as did two out of the 11 NHS acute hospitals.<\/span><\/p>\n<p id=\"E84\"><span id=\"E85\">The context of course is increasing demand\u2014more patients and more complicated patients. The word which should cause everyone, including the politicians to sit up is &#8220;<\/span><span id=\"E87\">precarious&#8221;<\/span><span id=\"E88\">\u2014<\/span><span id=\"E89\">the report says <\/span><span id=\"E90\">future quality is precarious.<\/span><\/p>\n<p id=\"E91\"><span id=\"E92\">And that applies all over the system. Inevitably there is a lot of concern about the NHS front door\u2014<\/span><span id=\"E93\">attendances at A&amp;E, ambulance calls, emergency admissions <\/span><span id=\"E94\">have all been rising. <\/span><span id=\"E95\">Like so many targets, the one whereby 95% of patients are treated within four hours is simply not being met. <\/span><\/p>\n<p id=\"E96\"><span id=\"E97\">But <\/span><span id=\"E98\">the knock<\/span><span id=\"E99\">&#8211;<\/span><span id=\"E100\">on effect of this relentless pressure can be seen throughout the system. <\/span><span id=\"E101\">Bed occupancy is at the highest level ever recorded (91.4 per cent between January and March 2017, compared to the recommended maximum rate of 85 per cent). <\/span><span id=\"E102\">C<\/span><span id=\"E103\">ancer patients are having to wait longer for treatment, with only 81 per cent of the 36,000 being referred for treatment able to access it within two months.<\/span><\/p>\n<p id=\"E104\"><span id=\"E105\">It also found bed occupancy levels for acute mental health wards too high while the number of detentions is up 20% in two years.<\/span><\/p>\n<p id=\"E106\"><span id=\"E107\">The CQC <\/span><span id=\"E108\">is <\/span><span id=\"E109\">full of praise for staff who are delivering in this environment but conclude\u2014there is a limit to <\/span><span id=\"E110\">their <\/span><span id=\"E111\">resilience and that <\/span><span id=\"E112\">far<\/span><span id=\"E113\"> t<\/span><span id=\"E114\">oo much care needs to improve. <\/span><span id=\"E115\"> <\/span><\/p>\n<p id=\"E116\"><span id=\"E117\">As for the NHS\u2019s sister service<span id=\"E109\">\u2014<\/span>social care, the situation is even more severe, <\/span><span id=\"E118\">despite<\/span><span id=\"E119\"> some extra <\/span><span id=\"E120\">funding<\/span><span id=\"E121\"> in the budget. Last year the CQC said it was <\/span><span id=\"E122\">\u201cat tipping point,<\/span><span id=\"E123\">\u201d and<\/span><span id=\"E124\"> it has not retreated from that. The <\/span><span id=\"E125\">number of beds in nursing homes has reduced by 4,000 (two per cent) over two years and the volume of local authority funded domiciliary care has decreased.\u00a0<\/span><span style=\"font-size: 1rem\">\u00a0<\/span><\/p>\n<\/div>\n<\/div>\n<div id=\"contentsContainer\">\n<div id=\"contents\">\n<p id=\"E132\"><span id=\"E133\">So,<\/span><span id=\"E134\"> what is to be done? Well, <\/span><span id=\"E135\">there is doubtless more to be done by the service, by NHS leaders and professionals to continue to develop a much more <\/span><span id=\"E136\">joined-up and person-centred care\u2014the\u00a0<\/span><span id=\"E137\">CQC is surely right to say <\/span><span id=\"E138\">this <\/span><span id=\"E139\">needs to happen with \u201cmore consistency and urgency, and national leaders need to support this\u201d.<\/span><\/p>\n<p id=\"E140\"><span id=\"E141\">But the money question will not go away. We already have an underlying <\/span><span id=\"E142\">deficit <\/span><span id=\"E143\">which has been estimated <\/span><span id=\"E144\">at \u00a33.7 billion in 2016\/17<\/span><span id=\"E145\">\u00a0and the prospects for the next two years are even more grim than the last two. <\/span><\/p>\n<p id=\"E146\"><span id=\"E147\">Governments always regard the constant calls for more funding with a sceptical eye and believe that much more can be done to make the most <\/span><span id=\"E148\">of<\/span><span id=\"E149\"> what is already on offer. This tends to be even more the case when the state\u2019s finances are <\/span><span id=\"E150\">far <\/span><span id=\"E151\">from healthy. But <\/span><span id=\"E152\">somehow,<\/span><span id=\"E153\"> we have got to convey an <\/span><span id=\"E154\">uncomfortable<\/span><span id=\"E155\"> truth<span id=\"E117\"><span id=\"E109\">\u2014<\/span><\/span><\/span><span id=\"E156\">that <\/span><span id=\"E157\">the situation is more serious than is being acknowledged, that necessary reform is being hampered by lack of funding and that we need both short<\/span><span id=\"E158\">&#8211;<\/span><span id=\"E159\">term support and a comprehensive review of what we need over the next <\/span><span id=\"E160\">10 <\/span><span id=\"E161\">to <\/span><span id=\"E162\">15 <\/span><span id=\"E163\">years.<\/span><\/p>\n<p id=\"E164\"><span id=\"E165\">The budget in November will be an opportunity for the government to underline its support for the NHS and social care system.
