{"id":40388,"date":"2017-10-16T15:16:07","date_gmt":"2017-10-16T14:16:07","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=40388"},"modified":"2017-10-23T10:47:36","modified_gmt":"2017-10-23T09:47:36","slug":"william-cayley-focus-on-investing-in-health-not-on-cost-containment","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2017\/10\/16\/william-cayley-focus-on-investing-in-health-not-on-cost-containment\/","title":{"rendered":"William Cayley: Focus on investing in health, not on cost containment"},"content":{"rendered":"<p class=\"standfirst\">No one wants a bankrupt healthcare system, but we need to care for those who are sick or in need<\/p>\n<p><!--more--><\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/07\/bill_cayley_2.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-31912\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/07\/bill_cayley_2-243x300.jpg\" alt=\"bill_cayley_2\" width=\"134\" height=\"151\" \/><\/a><span style=\"font-weight: 400\">It was <a href=\"http:\/\/www.npr.org\/sections\/health-shots\/2017\/10\/13\/557541856\/halt-in-subsidies-for-health-insurers-expected-to-drive-up-costs-for-middle-class\">announced last week<\/a> that the Trump administration will end <\/span><span style=\"font-weight: 400\">cost sharing reduction (CSR) payments to health insurance companies\u2014<\/span><span style=\"font-weight: 400\">the latest in an increasingly long line of efforts to repeal, replace, reform, or otherwise revise the Affordable Care Act (ACA). The ACA has been criticised for burdening both physicians and patients with increased regulation and paperwork, yet it&#8217;s been <a href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMhpr1503614#t=article\">clearly documented<\/a> that it has also led to a reduction in the number of uninsured individuals in the US.<\/span><\/p>\n<p><span style=\"font-weight: 400\">While the ACA is far from perfect, efforts to change it based purely on cost considerations are misguided. It is often claimed that the United States has the \u201cbest\u201d healthcare system in the world. That assertion can clearly <a href=\"http:\/\/www.commonwealthfund.org\/grants-and-fellowships\/grants\/2016\/nov\/commonwealth-fund-international-profiles-of-health-care-systems-2017\">be challenged by looking at various outcomes<\/a><\/span><span style=\"font-weight: 400\">, but if by the \u201cbest\u201d one means the capacity to deliver advanced care or to deal with complicated medical conditions with \u201cstate of the art\u201d or \u201chigh tech\u201d medical approaches, then the claim has merit. In the US, we have the ability to provide a level of technically advanced medical care that is not available in much of the rest of the world.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The percentage of our gross domestic product (GDP) spent on healthcare is not inherently a problem\u2014the problem is that <a href=\"http:\/\/www.commonwealthfund.org\/publications\/press-releases\/2015\/oct\/us-spends-more-on-health-care-than-other-nations\">we spend that amount relatively ineffectively<\/a><\/span><span style=\"font-weight: 400\">. It is vitally important for clinical and health systems research to explore how we can provide quality care <a href=\"https:\/\/blogs.bmj.com\/bmj\/2015\/05\/18\/william-cayley-less-is-more\/\">in simpler or less expensive ways<\/a><\/span><span style=\"font-weight: 400\">. Yet it is also important for us to recognise as a society that caring for our neighbours is going to cost us money: we cannot care for those in need without footing the bill for the cost. <\/span><\/p>\n<p><span style=\"font-weight: 400\">While legislators and politicians may fight over political agendas and budgetary priorities, it is imperative that those of us who care for patients strive to keep the focus on our patients. No one wants a healthcare system that goes bankrupt, but neither should anyone want a system that balances its budget on the backs of those in need. It costs to provide medical care, and it may cost more to provide state of the art care for those who are sick or disadvantaged. <\/span><\/p>\n<p><span style=\"font-weight: 400\">We need to change the conversation from one about cost reduction or cost containment to a conversation about values, priorities, and effective, efficient provision of care.
