{"id":39999,"date":"2017-09-04T10:38:01","date_gmt":"2017-09-04T09:38:01","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=39999"},"modified":"2017-09-12T12:03:37","modified_gmt":"2017-09-12T11:03:37","slug":"taher-qassim-yemenis-are-being-left-to-rot-in-a-forgotten-war","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2017\/09\/04\/taher-qassim-yemenis-are-being-left-to-rot-in-a-forgotten-war\/","title":{"rendered":"Taher Qassim: Yemenis are being left to rot in a forgotten war"},"content":{"rendered":"<p class=\"standfirst\">The cholera epidemic in Yemen is raging in the midst of one of the world\u2019s worst humanitarian crises<\/p>\n<p><!--more--><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-thumbnail wp-image-40001\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/09\/taher_qassem-150x150.jpg\" alt=\"\" width=\"150\" height=\"150\" srcset=\"https:\/\/blogs.bmj.com\/bmj\/files\/2017\/09\/taher_qassem-150x150.jpg 150w, https:\/\/blogs.bmj.com\/bmj\/files\/2017\/09\/taher_qassem.jpg 160w\" sizes=\"auto, (max-width: 150px) 100vw, 150px\" \/>Three UN agencies, WHO, UNICEF, and the World Food Programme (WFP) have united to\u00a0<a href=\"http:\/\/www.who.int\/dg\/speeches\/2017\/yemen-joint-mission\/en\/\">work together<\/a>\u00a0to fight the outbreak of cholera and the humanitarian crisis in Yemen. The crisis in Yemen is in its third year. The inability of the\u00a0<a href=\"http:\/\/www.reuters.com\/article\/us-yemen-security-un-idUSKBN18L18I\">UN envoy for Yemen<\/a>\u00a0to bring the two fighting parties to the negotiating table has meant more suffering and likely, the exacerbation of the cholera outbreak. In an editorial, <em>The\u00a0<a href=\"http:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(17)32210-9\/fulltext\">Lancet<\/a><\/em>\u00a0has described the situation as the &#8220;world&#8217;s worst cholera outbreak in the context of the world&#8217;s worst humanitarian crisis.\u201d<\/p>\n<p>There is no indication that either the Saudi-led coalition nor the Saleh\/Houthi coalition are willing to halt the devastating destruction of the country. A\u00a0<a href=\"http:\/\/www.un.org\/apps\/news\/story.asp?NewsID=57319#.WZGrF0-QyUk\">senior UN relief official<\/a>\u00a0has voiced concern about airstrikes on civilians that happened at the start of\u00a0August 2017. In fact,\u00a0<a href=\"https:\/\/www.chathamhouse.org\/publications\/twt\/yemen-and-business-war\">recent evidence shows<\/a>\u00a0that the two fighting parties are cooperating to keep the war going so that their profit-making will not be affected. This, of course, is music to the ears of the arms industry, who keeps selling ammunition to each of the fighting parties.<\/p>\n<p>The war, according to varying estimates, has so far killed more than ten thousand Yemenis and injured hundreds of thousands. But these are all estimates. The\u00a0<a href=\"http:\/\/who.int\/mediacentre\/news\/releases\/2017\/cholera-yemen-mark\/en\/\">cholera<\/a>\u00a0outbreak has\u00a0<a href=\"http:\/\/www.emro.who.int\/images\/stories\/Yemen_daily_epi_update_20170801.pdf?ua=1\">infected nearly half a million people<\/a>\u00a0and killed two thousand people. Again, these are all estimates by various organisations.<\/p>\n<p>If the war continues in Yemen, the world has to be prepared for the potential of other surprise outbreaks of deadly diseases. Shockingly, these could be diseases that have been nearly eradicated from modern times\u2014diseases such as polio\u2014which could make a strong comeback. The cholera outbreak might improve, but in the absence of a functioning health system, malnutrition, starvation and deadly infectious diseases will be the norm. The war has severely disrupted the functions of health facilities, the availability of medicine, and the absence of the workforce due to the risks of being killed or not being paid. An outbreak in the future will be extremely difficult to control. The impact of war in Yemen was simplified in this\u00a0<a href=\"http:\/\/www.who.int\/emergencies\/yemen\/infographic.jpg?ua=1\">WHO infographic<\/a>.<\/p>\n<p>At the time of writing, Yemen\u2019s crisis is the least of the world\u2019s worries. Yemenis are being left to rot in a forgotten war. Some of the Yemeni diaspora in the UK and elsewhere are less interested in the cholera outbreak, but care more about who is going to win the war. However, it is fast dawning on many that there will be no winners and losers. In reality, everyone will be losers, because they have failed their own people and destroyed the basic infrastructure that was there before the war. It is easy to blame the other as a wrong-doer, but it is difficult to take responsibility and make compromises for the sake of all.<\/p>\n<p>Cholera in Yemen is a man-made outbreak. Campaigners highlighting the cholera outbreak are careful not to mention politics when they talk about cholera. In other words, humanitarian agencies are comfortable talking about the cholera outbreak, but they are unable to speak about why it happened in the first place. Hence, the diplomacy of the gun remains the strongest and cholera is a shadow that comes and goes, making insignificant headlines, if any, and mostly on social media.<\/p>\n<p>The cholera outbreak has affected almost all the governorates in Yemen.\u00a0<a href=\"http:\/\/www.emro.who.int\/images\/stories\/Yemen_daily_epi_update_20170801.pdf?ua=1\">The top ten governorates<\/a>\u00a0affected by cholera were all in areas that are controlled by the Saleh\/Houthi coalition, except Adalea. There could be several explanations as to why this is the case. Many health facilities are either destroyed or closed, health workers are not paid, and the transport of medicine is limited due to tight military control by both sides of the conflict. Amanat Al Asimah in the capital Sanaa, followed by Al Hudaydah, Hajjah, and Amran had the highest number of cholera cases. How the local communities have responded to the outbreak in these varied geographical areas is difficult to tell at the moment. The\u00a0<a href=\"http:\/\/www.yemenpolling.org\/advocacy\/upfiles\/YPCPublications_YPC-Data---Perceptions-of-the-Yemeni-public-on-living-conditions-and-security-related-issues---May-2017.pdf\">Yemen polling report<\/a>\u00a0presents some evidence about peoples\u2019 opinion on the war, security, and gender related security issues. In relation to the top 10 governorates affected by cholera, the report might give a proxy explanation of why these governorates were affected most.<\/p>\n<p>The world is witnessing a man-made cholera outbreak in Yemen as a result of a conflict that has entered its third year. No matter what happens to reduce the spread of the outbreak, other outbreaks of deadly infectious diseases are assured, as was predicted earlier by different experts. This is due to the total collapse of the health system in Yemen and the continued conflict which shows no sign of abating at the time of writing.<\/p>\n<p>On Wednesday 6 September, I will be at the\u00a0<a href=\"http:\/\/www.medact.org\/\">Medact<\/a>\u00a0&#8220;<a href=\"http:\/\/healththroughpeace.org\/\">Health Through Peace&#8221;<\/a>\u00a0Conference, where I will be conducting a workshop about the need for alternative ways of supporting individuals,\u00a0community groups and what is left of the health workers network.\u00a0The workshop will consider:<\/p>\n<ul>\n<li>How could public health and other health professionals in the UK, interested in Yemen, use existing technologies to support Yemen\u2019s collapsed health services?<\/li>\n<li>How could the international health community encourage and build local health networks in Yemen where the war is not as intensive as in other parts of the country?<\/li>\n<\/ul>\n<p><em><strong>Taher Qassim<\/strong> works for the department of public health in Liverpool Local Authority.
