{"id":39906,"date":"2017-08-21T14:43:44","date_gmt":"2017-08-21T13:43:44","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=39906"},"modified":"2017-09-01T15:18:49","modified_gmt":"2017-09-01T14:18:49","slug":"richard-smith-journals-surgeons-and-sexist-language","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2017\/08\/21\/richard-smith-journals-surgeons-and-sexist-language\/","title":{"rendered":"Richard Smith: Journals, surgeons, and sexist language"},"content":{"rendered":"<p><a href=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/richard_smith_2014.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-33037\" src=\"https:\/\/blogs.bmj.com\/bmj\/files\/2014\/12\/richard_smith_2014-150x150.jpg\" alt=\"richard_smith_2014\" width=\"128\" height=\"128\" \/><\/a>Much to my amusement and countering the stereotype of surgeons, the <em>Annals of Surgery<\/em> has \u201cfollowing an uproar\u201d\u00a0<a href=\"http:\/\/retractionwatch.com\/2017\/07\/24\/following-uproar-surgery-journal-retracts-paper-male-pronouns\/\">retracted a paper<\/a> that used only male pronouns to describe surgeons.\u00a0It\u2019s counter to the stereotype of surgeons because they still are overwhelming male and, in our imaginations if not reality, casually sexist. But there are an increasing number of female surgeons, and they presumably led the uproar (or is that supposition sexist?).<\/p>\n<p>(Since writing that first paragraph I have by pure chance encountered the surgeon who started the uproar with her Tweet: <a href=\"https:\/\/twitter.com\/natalieblencowe\">Natalie Blencowe<\/a>, a trainee surgeon in the Severn Deanery, who has with Jane Blazeby, professor of surgery in Bristol, <a href=\"https:\/\/blogs.bmj.com\/bmj\/2016\/04\/20\/beyond-buxton-establishing-when-the-times-right-for-a-surgical-trial\/\">written for <em>The BMJ<\/em><\/a>. They are about <a href=\"http:\/\/www.bmj.com\/content\/356\/bmj.i6463\">transforming surgical culture<\/a>, and we briefly discussed the words used to describe assertive female surgeons: feisty, bossy, abrasive, outspoken.)<\/p>\n<p>The offending article was a presidential address given in Polish, and the journal has a lame excuse that \u201cthe pronoun \u2018his\u2019 can include women in Polish.\u201d That would, of course, have been the case in English until recently, and it still is for many English speakers.<\/p>\n<p>I find the episode amusing because it allows us to calculate just how far behind the time surgeons are. The answer is 30 years. I say this because <em>The BMJ<\/em> introduced <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1257479\/pdf\/bmjcred00051-0007.pdf\">a policy of non-sexist or inclusive language in 1987<\/a>. I was then an assistant editor. We already did our best when editing to avoid sexist language, but an editorial made it explicit policy and asked authors to avoid sexist language in the articles they submitted. Examples of sexist language from medical writing included in the editorial were not only sexist but absurd, inaccurate, and funny: \u201cDevelopment of the uterus in rats, guinea pigs, and men\u201d and \u201cSubjects were 16 boys and 16 girls. Each child was to place a car on his board\u2026\u201d<\/p>\n<p>But <em>The BMJ<\/em> was not wholly successful in avoiding sexist language. <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1882777\/\">A reader examined 469 <em>BMJ<\/em> articles<\/a> for sex-biased language in 1992, when I was the editor, and found it in 35 (7%). Tim Usherwood, the author observed that 34 of the 35 examples of sex-biased language were androcentric. He wrote: Thus medical staffing issues were described as issues of medical &#8220;manpower.&#8221; The chairs of many committees were described as &#8220;chairmen,&#8221; even when women; and train drivers, doctors, surgeons, gastroenterologists, inventors, children, and chief executives were all alluded to with masculine pronouns. The average person was &#8220;the man in the street,&#8221; and women were presumably immune to environmental factors &#8220;harmful to man.&#8221;<\/p>\n<p>Somebody should repeat the audit. As a starter, I read every word of three pages of the <em>BMJ<\/em> of 15 July and noted the use of \u201cchair\u201d instead of chairman, \u201cworkforce\u201d rather than manpower, and the use of the plural \u201cthey\u201d rather than the singular \u201che or she,\u201d which avoids the problem English creates by not having something other than \u201che,\u201d \u201cshe,\u201d or it. One possible example of sex-biased language was \u201ca spokesman said,\u201d but perhaps the spokesman was a man; but as he or she is anonymous it might have been better to say spokesperson.<\/p>\n<p>Then\u2014by chance not design, I assure you\u2014the three pages include a piece on sex with robots. The piece reports that \u201c17% of 1002 adults questioned would be willing to go on a date with a robot, and that increased to 26% for a robot that looked like a human,\u201d (creating an image in my mind of a person sitting in a bar chatting with a piece of machinery), but there is no assumption in the words of the piece that it is men who will have sex with robots; but all the examples\u2014including a \u201cman\u2026[who] left his wife and family for\u2026[a robot]\u2014are of men, and the one picture shows a woman. Is that sexist?
