There are no single, non-metaphorical words to replace them. <em>Ultimate<\/em>, <em>late<\/em> and <em>final<\/em> appear in lists of synonyms, but they don\u2019t give the sense. <em>Last minute<\/em> and <em>eleventh hour<\/em> are synonymous: something done before time has run out. <em>Last minute<\/em> has always been more popular than <em>eleventh hour<\/em>, increasingly so in recent years: four times more common on Ngram in 1960, it is now nine times more common, as its prevalence has increased and that of <em>eleventh hour<\/em> has decreased.<\/p>\n<p><em>Last minute<\/em> is also more common in PubMed, but many of its occurrences are literal: measurements made in the last minute of a period of observation. <em>Last minute<\/em> is barely a metaphor at all, but \u201cDespite the best of intentions, we often act at the last minute when we are faced with a deadline\u201d does not mean\u2014probably\u2014that it is literally the last minute; it is\u2014probably\u2014the last day. So, \u201ca last minute concession by DoH led to a deferment\u201d, \u201ca \u2018near miss&#8217; where harm is avoided at the last minute\u201d, and\u00a0\u201cgiving a \u2018last minute\u2019 choice to the patients\u201d are all good, clear descriptions.<\/p>\n<p>They could be rephrased\u2014<em>where ultimately harm is avoided<\/em> could give the sense; but <em>ultimately<\/em> might be taken as <em>eventually<\/em>, i.e., a drawn-out process without any time constraint. <em>Last minute<\/em> removes any ambiguity.<\/p>\n<p>Sometimes\u2014as above\u2014authors enclose metaphorical expressions in inverted commas. It is usually not necessary, but <em>last-minute concession<\/em> might then need the hyphen to avoid misunderstanding as a <em>last, minute concession<\/em>, a misunderstanding unlikely to happen with <em>last minute choices<\/em>.<\/p>\n<p><em>Eleventh hour<\/em> is less prevalent, but \u201cAt the eleventh hour, the invitation to speak was withdrawn\u2026\u201d, \u201cAt the eleventh hour, bishops continue to launch concerns\u2026\u201d, and \u201c\u2026every 2 hours and 30 minutes until the eleventh hour\u201d\u2014the only literal eleventh hour that I found. <em>Last minute<\/em> is an obvious metaphor; no doubt somebody was the first to use it, but it\u2019s unlikely that it was recorded. <em>Eleventh hour<\/em> is not so obvious, and, according to the <em>Oxford English Dictionary<\/em>, is an allusion to the parable of the labourers (<em>The Book of Matthew<\/em>, chapter 20).<\/p>\n<p><em>Last ditch<\/em> is more to do with effort than time, has military connotations, and is often linked with futility: \u201cwith \u2018last ditch\u2019 efforts of resuscitation&#8221;, \u201cFirst introduced as \u2018last ditch\u2019 therapy in the most critically ill\u201d, \u201c\u2026such \u2018last ditch surgery\u2019 invariably fails\u201d. Like <em>last minute<\/em> and <em>eleventh hour<\/em>, it is a useful metaphor, although prone to overuse by groups lobbying for expensive treatments. I did find one, not strictly medical, literal use, in an article about the treatment of waste water: \u201cThe content of heavy metals in suspended solids decreased along the course of treatment\u2026, through the first ditch to the last ditch.\u201d Unfortunately, metals were also found in subsequent ditches, so the last ditch failed.<\/p>\n<p><em><strong>Neville Goodman<\/strong> is a retired consultant anaesthetist and a writer.
