The day after his <a href=\"https:\/\/www.nytimes.com\/2017\/03\/24\/us\/politics\/health-care-affordable-care-act.html\">failed<\/a> attempt to repeal and replace the ACA on 24 March, he tweeted, \u201cObamacare will explode . . . Do not worry.\u201d Then, in mid-April, one way this might <a href=\"http:\/\/www.cnbc.com\/2017\/04\/12\/president-donald-trump-threatens-to-cut-obamacare-subsidies.html\">happen<\/a> was put forward: by cutting costs sharing reduction (CSR) payments to seven million <a href=\"http:\/\/www.commonwealthfund.org\/publications\/explainers\/2017\/apr\/cost-sharing-reductions\">lower income ACA enrollees<\/a>. These subsidy payments reduce out of pocket costs for lower income families by reducing or eliminating deductibles.<\/p>\n<p>More recently, over the past two weeks the administration has sought to appeal to hardline conservatives: firstly, by giving more power to individual\u00a0<a href=\"http:\/\/www.politico.com\/story\/2017\/04\/25\/republicans-obamacare-repeal-237609\">states<\/a> on how move forward on reform, and secondly, by removing safeguards for people with <a href=\"https:\/\/www.washingtonpost.com\/powerpost\/gop-health-care-push-faces-new-obstacles-as-concerns-about-preexisting-conditions-grow\/2017\/05\/02\/d25fc760-2f47-11e7-8674-437ddb6e813e_story.html?hpid=hp_hp-top-table-main_healthcare-0802pm%3Ahomepage%2Fstory&amp;utm_term=.eae3b8400cc2\">pre-existing conditions<\/a>. Both proposed changes have been met with fierce resistance this week by moderate Republicans.\u00a0But with the <a href=\"http:\/\/money.cnn.com\/2017\/05\/03\/news\/economy\/high-risk-pools-obamacare-pre-existing\/\">recent promise to inject an additional $8bn spending<\/a> into Trump\u2019s reform package, the outcome hangs by a thread.<\/p>\n<p>Dismantling Obamacare is\u00a0part of Trump\u2019s goal to \u201cMake America Great Again.\u201d Yet in the health sector, America was never \u201cgreat\u201d\u2014at least as measured by the criteria of equity and efficiency.<\/p>\n<p>Eight years ago (that is, two presidential terms ago) the groundbreaking study <em><a href=\"http:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMbkrev0900609\">The Measure of America<\/a><\/em>\u00a0was released. Modeled on the United Nations Development Program&#8217;s Human Development Report, it asked a fundamental populist question, \u201cHow are people in the United States doing?\u201d It did not ask \u201chow are we doing in terms of economic growth, trade, and investment?\u201d\u2014the question normally asked by the establishment to measure progress. It introduced the American Human Development Index: a single measure of wellbeing for all Americans based on life expectancy, access to knowledge, and income, which was disaggregated by geographical location (by state and by congressional district), race, ethnic group, and sex.<\/p>\n<p>The <a href=\"http:\/\/ssrc-static.s3.amazonaws.com\/moa\/ahdr-execsumm.pdf\">index data<\/a> provided new detail on the US healthcare system, showing that it was a poor performer compared to its peers, spending on average three times more on healthcare than other industrialized nations while leaving tens of millions of people without adequate health insurance. At the heart of this failure were inequalities. When Barack Obama assumed office in January 2009, he was confronted by two striking sets of inequalities: those defined by race and by place of residence.<\/p>\n<p>For example, African-Americans with vascular disease or diabetes were four times more likely to have a leg amputated than white people. Data showed that infant mortality rates among African-Americans were more than two times, and in some southern states three times, higher than the national average.\u00a0Nine states shared the worst levels of health and human development and these were in the south, where African-American populations are 70% larger than the national average.<\/p>\n<p>Set against this background and the social architecture that supported these large inequalities, Obama sought to make good on his commitment to create a level playing field for all Americans, especially in healthcare. Yet although he began his presidency with the \u201c<a href=\"http:\/\/www.pbs.org\/wgbh\/frontline\/article\/watch-how-obamacare-became-a-symbol-of-americas-divide\/\">promise of change<\/a>, unity, and bipartisanship\u201d (and indeed, 80% of the public described themselves as optimistic about the new president), he soon hit a brick wall with his proposed healthcare bill. According to a recent documentary series <em><a href=\"http:\/\/www.pbs.org\/wgbh\/frontline\/article\/watch-how-obamacare-became-a-symbol-of-americas-divide\/\">The Divided States of America<\/a><\/em>, as he embarked upon health reform he \u201cquickly collided with political realities, including unified Republican opposition to his agenda and racially charged resistance.\u201d<\/p>\n<p>After a year of intense struggle, Obamacare passed the House of Representatives <a href=\"http:\/\/www.washingtonpost.com\/wp-dyn\/content\/article\/2010\/03\/21\/AR2010032100943.html\">on 21 March 2010 by a vote of 219 to 212<\/a>, but without a single Republican vote. A general theme of the PBS report was that Obamacare \u201ccontributed to years of polarization and a wave of anti-establishment sentiment that helped fuel Trump\u2019s road to victory\u201d (see <a href=\"http:\/\/www.pbs.org\/wgbh\/frontline\/film\/divided-states-of-america\/transcript\/\">transcripts<\/a>\u00a0online). Drawing from dozens of experts and Congressional leaders, the\u00a0documentary reported that &#8220;Obamacare came to symbolize the divisions in America.&#8221;\u00a0It claimed that \u201cwhat happened after it was passed, and because of the way it was passed, it became the symbol of the divide, and the reality of it in many ways.\u201d This portrayal of Obamacare as a bill forced through\u00a0amid discord was a view that Trump successfully <a href=\"http:\/\/thehill.com\/policy\/healthcare\/324228-trump-makes-healthcare-pitch-during-rally\">exploited on his campaign\u00a0trail<\/a>.<\/p>\n<p>The documentary\u00a0concluded that the future of Obamacare is uncertain since it is unclear \u201cwhether or when the country might finally accept what he has done\u201d (by passing the ACA). Trump\u2019s efforts to repeal and replace Obamacare are also based on the notion that most Americans do not accept Obamacare. Yet these assessments do not seem consistent with the evidence, or at least are no longer consistent with the latest evidence.<\/p>\n<p>For example, a <a href=\"http:\/\/kff.org\/health-reform\/poll-finding\/kaiser-health-tracking-poll-april-2017-the-fall-of-the-ahca-and-next-steps-for-the-aca\/\">Kaiser<\/a> tracking poll done in early April and then a Washington Post-ABC News <a href=\"https:\/\/www.washingtonpost.com\/national\/health-science\/public-pans-republicans-latest-approach-to-replacing-affordable-care-act\/2017\/04\/25\/25355eb0-26aa-11e7-bb9d-8cd6118e1409_story.html?utm_term=.bd8b63c93e7d\">poll<\/a> done at the end of April contradict this conclusion. Kaiser showed that 64% of the American public thought it was a good thing that Trump\u2019s American Health Care Act\u00a0failed to pass. Seventy five per cent of the public think that Trump and his administration \u201cshould do what they can to make the current law work.\u201d The Washington-Post-ABC News <a href=\"https:\/\/www.washingtonpost.com\/national\/health-science\/public-pans-republicans-latest-approach-to-replacing-affordable-care-act\/2017\/04\/25\/25355eb0-26aa-11e7-bb9d-8cd6118e1409_story.html?utm_term=.bd8b63c93e7d\">poll<\/a>\u00a0found that 61% of the public thought Trump should keep and improve Obamacare. Other noteworthy findings are that 70% of the public (including a majority of Republicans) want a nationwide mandate for pre-existing conditions rather than Trump\u2019s notion of <a href=\"http:\/\/www.politico.com\/story\/2017\/04\/25\/republicans-obamacare-repeal-237609\">leaving it up to states<\/a> to decide\u2014as was under consideration last week and this week.<\/p>\n<p>One explanation for the mismatch between the PBS assessment (and indeed, Trump\u2019s misreading of the public) and the results of these two polls is that the tone and quality of leadership and decision making of the past 100 days may have disenchanted many Americans. The executive orders aimed at deregulation\u2014especially of the finance and banking sectors\u2014and the proposed enormous tax cuts appear to benefit the privileged 1%. Consistent with this is a Kaiser poll finding that the percentage of Americans who believed that Trump could deliver better and cheaper healthcare dropped from 47% before the election to 37% three months later.<\/p>\n<p>Behind these poll results may also be a growing perception that Obamacare is actually a populist law and that Trumpcare is anti-populist.
