Referring to a person as a \u201csubstance abuser\u201d rather than \u201ca person with substance use disorder\u201d <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20005692\">evokes stigma<\/a> and <a href=\"http:\/\/journals.sagepub.com\/doi\/pdf\/10.1177\/002204261004000403\">there is evidence<\/a> that it reduces patients\u2019 access to appropriate treatment.<\/p>\n<p>Moreover, imprecise terminology may result in misunderstanding of the nature of pain treatment and the management of substance use disorder. In turn, politicians and administrators may establish irrational public health policies, and patients may decide not to take their medicines.<\/p>\n<p>In <a href=\"http:\/\/www.bmj.com\/content\/356\/bmj.j309\">many countries<\/a> the <a href=\"http:\/\/www.who.int\/medicines\/areas\/quality_safety\/ACMP_BrNote_Genrl_EN_Apr2012.pdf\">barriers to accessing pain treatment<\/a> and treatment of opioid dependence are huge.\u00a0Yet non-medical use of psychoactive substances and untreated moderate and severe pain are considered public health priorities. Clear, unambiguous, non-stigmatising terminology <a href=\"http:\/\/www.who.int\/medicines\/areas\/quality_safety\/guide_nocp_sanend\/en\/\">is recommended by the World Health Organization<\/a> (WHO) as one way to ensure access to controlled medicines.<\/p>\n<p>Everyone should use appropriate terminology, including patients, governments, and international bodies. Yet journals read by healthcare professionals have a particular responsibility to communicate clearly in ways that benefit patients. Indeed, we consider the use of appropriate terminology in scientific journals of utmost importance to enhancing the quality and clarity of scientific communication and improving patient access to medicines used to treat both dependence and pain.<\/p>\n<p>The call to abandon the use of terminology that can stigmatise <a href=\"http:\/\/journals.sagepub.com\/doi\/pdf\/10.1515\/nsad-2015-0051\">is not new<\/a>, but most medical journals continue to accept manuscripts with inappropriate wording. Efforts to\u00a0use appropriate terminology should be widespread and extend to all patients who need controlled medicines, including patients with pain, in palliative care, or with substance use disorder. Moreover, the focus should be on a range of negative wording. In this article we note only the most obvious.<\/p>\n<p>The word \u201cdrug\u201d is ambiguous because it could mean \u201cmedication\u201d or \u201cnon-medically used psychoactive substance\u201d. Most parents and politicians do not like children to use \u201cdrugs,\u201d but they are in favour of them\u00a0receiving \u201cmedications\u201d when ill. Therefore, the ambiguity of \u201cdrug\u201d could hamper access to controlled (psychoactive) medicines in cases where their use is fully rational, and the unambiguous terms \u201cmedication\u201d (or \u201cmedicine\u201d) and \u201cpsychoactive substance\u201d should be used instead.<\/p>\n<p>Doctors and medical journals should use \u201cperson first\u201d language\u2014for example, \u201cperson with substance use disorder\u201d instead of language that defines people by an illness, such as \u201caddict.\u201d Two other unacceptable examples are \u201cabuse\u201d and \u201cmisuse\u201d because these express judgment. The alternatives are, for instance, \u201cnon-medical use\u201d or just \u201cuse.\u201d<\/p>\n<p>\u201cPhysical dependence\u201d for describing tolerance and withdrawal is also not acceptable language because it suggests that this is true dependence, but does not meet the ICD-10 diagnostic criteria for dependence, which includes at least one psychological component.<\/p>\n<p>Also, if used in a medical context, the term \u201cnarcotic\u201d: this is a legal term from the Single Convention on Narcotic Drugs, but it does not accurately represent the character of psychoactive substances, which can also be stimulants\u2014such as cocaine.<\/p>\n<p>Finally, the terms \u201csubstitution treatment\u201d or \u201cmedication assisted treatment\u201d ignore that this treatment is based on medicines that are proven to be effective. This may lead to government policies that exclude pharmacological treatment for substance use disorder.
